There is a total of 5 wells spudded on the pad, none of which are completed at this time. No production equipment is present on the well pad.
0396196702 2016-09-14 Drill / Deepen / Reopen No Violations On site status check inspection accompanied by ODNR Inspector Austin Gutridge found Patterson Drilling rig 801 on location and the 8.5 production well bore total drilling depth recently reached at 20.989 and the 5.5 production casing ran and cemented. The rig is skidding to the next well on the pad.
0711994813 2016-09-13 Casing String Witnessed No Violations My inspection today finds the drilling contractor, Patterson Drilling rig 801, is attempting to circulate out a gas kick. Casing pressure is steady around 400 to 600 pounds per square inch on 3/4 choke. Pason gas measures 3500 to 5000 parts per million. While on location I witnessed water exit the flare stack and fall to the ground around the flare stack. I instructed Steve Bishop, Drilling Manager, to test the water as soon as safely possible to ensure the area is properly remediated if needed. 5.5 casing has been installed to a depth of 21,016. The company was instructed to condition the well until static prior to beginning cementing operations.

0924743902 2016-08-18 Casing String Witnessed No Violations I contacted CNX company representative Ryan Coffman. Mr. Coffman indicated the new joint of pipe was on location and the cement job would be starting in about two hours. Upon arrival I found the drilling rig attempting to re-enter the section of the well below the mine void. The casing could not be realigned with the wellbore and I departed.

Express Energy cement service cemented the casing using 275 sacks of class A -2% calcium .50 pound per sack loss circulation material 50% open hole excess cement with good cement returns witnessed to surface and the cement mix water tested for chlorides and PH.
1795535528 2017-08-16 Production Wells No Violations The purpose of my inspection was a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure gauges present read 5696 pounds per square inch on the tubing, 6389 pounds per square inch on the 5.5 inch diameter casing, 1011 pounds per square inch on the 13.375 inch diameter first intermediate casing, 627 pounds per square inch on the 9.625 inch diameter second intermediate casing. The production equipment consisted of four 400 barrel tanks, five sand traps, and five gas production units. Identification was posted at the wellhead.
1944916277 2019-05-02 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells , (4) gas processing units with sand traps, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance, tanks, and wellheads. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 458 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 458 psi. The production records are current.
