0667523099 2016-11-28 Drill / Deepen / Reopen No Violations The purpose of my inspection today is a Temporary inactive status check. There are 5 wells total on pad. There is no production equipment on location. No violations present. During my inspection I was accompanied by Cody Craker with CONSOL Energy.
0726464445 2015-07-22 Drill / Deepen / Reopen No Violations Status check inspection found the well drilling process has been completed with the well bore total drilling depth reached at 21,124 and 5.5 production casing ran into the well to a set depth of 21,103. CalFrac cement service cemented the said well casing using a Class A lead and tail cement mix with 90 barrels of gel spacer returns to surface. The drilling rig Patterson 801 has been released and the remainder of he wells on the pad will not be completed at this time. All drilling field records have been received and recorded.
1050944050 2019-05-02 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells , (4) gas processing units with sand traps, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance, tanks, and wellheads. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 457 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 204 psi. The production records are current.

I discussed with the company men on site the DV tool and Inflatable Packer isolation tools to be ran.
1426712055 2015-05-28 Casing String Witnessed No Violations 05/28/2015 -On site inspection to witness the conductor cement job found Express Energy has drilled a 36 x 100 conductor borehole and set 100 of 30 .375 wall casing with Express Energy Cement service on location to cement the casing using 275 sacks of class A-.50 pound per sack loss circulation material along with 2% calcium and 50% open hole excess cement with the cement displaced with 74.8 barrels of fresh water and good cement returns to surface with a constant lift pressure shown during displacement. 2 wells were cemented on this day with a 4 hour wait time between jobs due to only one rig on the location to hold the pipe.

A formation integrity test was done indication a breakdown tolerance good for 15 # mud weight.

Addressing the well - The well has been drilled but not stimulated at this time, with 4 additional wells on the pad that have the conductor only set in place. No production equipment is present on the well pad.
1471948353 2016-08-22 Drill / Deepen / Reopen No Violations Temporary inactive status check inspection found the well drilling process has been completed and the well is awaiting stimulation. There is a drilling rig on location completing the remainder of the permitted wells on the pad. Annular testing has been completed and monitored on the said well. No violations noted and the drilling permit is good until 05/18/2017.
1491460900 2017-08-16 Production Wells No Violations The purpose of my inspection was a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure gauges present read 6352 pounds per square inch on the tubing, 6830 pounds per square inch on the 5.5 inch diameter casing, 541 pounds per square inch on the 13.375 inch diameter first intermediate casing, 1049 pounds per square inch on the 9.625 inch diameter second intermediate casing. The production equipment consisted of four 400 barrel tanks, five sand traps, and five gas production units. Identification was posted at the wellhead.

1909386502 2015-06-25 Casing String Witnessed No Violations On site status check inspection found Patterson Drilling rig # 801 on location with a 24 mine void coal seam borehole recently drilled to a depth of 800 with a coal seam encountered at a depth of 688 to 698. The company is preparing to begin running the 18.625 mine string to be set at a depth of 42 below the mine seam floor with a 2 stage tool to be set at 562 and a swell packer to be set below the 2 stage tool at 566 to isolate the coal seam. CalFrac Well Service is on location to cement the casing in a 2 part process with the bottom section from the casing shoe to the mine void cemented using 305 sacks of class A 1.5% calcium .25 pound per sack flake 50% open hole excess cement, the top section was cemented after deploying the hydraulic packer and 2 stage cement tool using 1336 sacks of class A .5% calcium .25 pound per sack flake 100% open hole excess cement with 20 barrels of viscous spacer ran ahead along with 21 barrels of bentonite gel, the casing was pressure tested to 500 psi over lift pressure upon the landing of the DV closure tool.
