I was relieved by Inspector Stolarik.

0840943183 2019-05-06 Production Wells No Violations On this day, I am conducting a preliminary restoration inspection. The site meets ORC 1509.072(A). There is signage at the pad entrance. There are (3) producing wells, (2) gas processing units with sand traps, (5) 400-barrel steel production tanks within steel containment, and a flare stack present. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The production records are current.
0906978522 2016-05-16 Drill / Deepen / Reopen No Violations On site inspection of the current drilling activity. Upon inspection it was observed that a drilling rig identified as 1st Class Energy rig # 10 was on the hole and has drilled a 24 borehole to a total depth of 770 and has penetrated a coal seam at 69 to 697 with the company reporting they have not encountered a mine void. The Division Permitting section has issued a permit to set the mine string and not utilize isolation tools if no mine string is encountered. The company is currently rigging up to run the 18.625 mine string and will notify me of a approximate time for the upcoming cement job.
1463159651 2016-05-13 Casing String Witnessed No Violations The said well was spudded on 05/12/2016 by Express Energy with a 36 x 95 conductor borehole being drilled and 95 of 30 conductor being set. Express Energy cement service cemented the casing using 260 sacks of class A cement with circulation established prior to cementing and cement returns witnessed to surface inside the tin horn cellar.
1463463826 2016-05-17 Casing String Witnessed No Violations On site inspection beginning at 21:00 hours on 05/16/2016 with my arrival onto the well location to find Inspector Eric Conrad reporting that during the initial attempt to cement the mine string circulation was lost and the company has indicated the coal seam in the mine works not reported as initially encountered during drilling has broken down during the initial attempt to gain circulation before cementing. Between the hours of 21:00 hours and 23:00 hours a 47 barrel class A bottom balance plug was set using Class A cement with 3% calcium and 5% gypsum and .125 lb/sk lcm with the 18.625 casing shoe at 742 and the coal seam at 692 to 697. Fluid level was marked prior to cementing the bottom at 699. At 2:00 am on 05/17/16 - (2) 1 diameter grout strings were ran into the annular space of the 18.625 casing to a measured depth of 600 and a 10 barrel plug spotted on top of the cement basket set at 657, the cement was then given 2 hours to cure before attempting to tag the top of cement with the 1 tubing string and setting a 2nd 20 barrel plug at 500 at 4:30 am. The 1 tubing string was then pulled up to 300 and a 100 barrel plug pumped. The tubing was pulled and the casing topped off with 57 barrels of class a cement. The total annular calculated volume was 185 barrels of cement with 187 barrels used on this day. The company will initiate a bond log and casing shoe test before proceeding with the well bore drilling process. The current rig on the hole First Class Energy rig # 10 has completed their assigned drilling and will move off the hole.
1464451632 2016-05-28 Casing String Witnessed No Violations On site inspection to witness the 18.625 casing shoe integrity test. The top of cement was tagged inside the 18.625 at 671 and the cement was drilled out below the casing shoe set at 742 with a 17.5 diameter drill bit. The casing was then loaded with 8.8 lb/gl processed fluid and the drilling rig Kelly bushing and rotating rubber was pulled allowing visibility to check the fluid top inside the casing, The calculated fluid hydrostatic weight was 339.5 lb on the shoe with no fluid loss observed and the company granted permission to proceed. Also note the company has directional drilling tools on the drill string and there are live completed wells on this location.

1519751934 2018-02-27 Preliminary Restoration No Violations Status check inspection found the affected area of the well site construction meets ORC 1509.072 with vegetation established and erosion control in place. The well was drilled in addition to the (2) previous producing wells on the pad. Addressing the well- The well is in the initial flow back stages and is producing through the 5.5 diameter production casing. The manual master valve is still in place on the well head with 2 smaller production valves sitting atop the master valve. Downstream from the vertical completion stack is an additional master valve and (2) emergency shut down valves that can be controlled remotely by the production management by the means of a computer. There is 2 electrically controlled manual buttons on location, both at the production facility. The emergency shut down devices are also pressure controlled with upper and lower set limits. Also on location is 2 additional well head previously completed along with a production facility consisting of a total of (5) production tank of steel design and 400 barrel fluid capacity. The production tanks also have fire censors that will automatically shut the wells in. The production tanks are situated inside a polymer lines steel containment dike area.

2109614553 2017-01-04 Completion Testing No Violations Status check inspection on the well completion process found Kean Group pressure pumping service standing by to stimulate stage 25 out of 52 total stages to be stimulated. Schlumberger Wireline Service is on location and has perforated the 24th stage by 1st setting a composite bridge plug at 18,482 and the perforating the stage in 5 clusters staggered out in 5 shots between the depths of 18,444 and 18,264. Cactus well head is on location and providing well control with a 15,000 pound pressure rated well head and completion iron. The company is running a composite cage ball bridge plug every 10 stages and re pressure testing the 5.5 diameter casing after every 10 stages.
