0544051759 2018-02-26 Preliminary Restoration No Violations Preliminary restoration found the area affected by the well pad construction as described in the pre-site construction plans meets ORC 1509.072. The entrance of the lease road has signage posted identifying each of the 4 wells present on the well pad. The well pad entrance has an electrically controlled emergency shut down button on the right side of the lease road. The lease road is gravel and in good repair. As soon as you enter the well pad there are 4 steel tanks that are of steel construction and of 400 barrels capacity each. The tanks are situated inside a polymer lined steel containment dike area with an emergency shut down button at the loading site of the tanks. Situated beside the production facility is 4 gas processing units one supporting each well on the pad, each gas processing unit has an emergency shut down device present. In the center of the well pad is 4 well heads with the tubing valves open and annular pressure monitoring in place. Each well head is also equipped with an electrically controlled shut down device.

1428362466 2015-04-06 Casing String Witnessed No Violations 04/06/2015 Patterson Drilling rig 538 is now on the hole and has drilled a 17.5 x 358 surface borehole and ran 358 of 13.375 surface casing with a guide shoe and float collar ran and proper centralizer placement demonstrated. Stingray cement service is on location to cement the casing using 479 sacks of class A-2% calcium .125 pps flake with 50% open hole cement excess ran and the casing pressure tested to 500 psi upon the landing of the cement latch down plug, with good cement returns witnessed to surface.
1428519783 2015-04-08 Casing String Witnessed No Violations Patterson drilling rig 538 and Wyoming casing crew ran 2353 of 36lb/ft. of 9.625casing in 12.25open hole. There are 23 centralizers ran on the casing string. The company ran all range 3 casing j-55 buttress thread. Sting ray is cementing back the casing to the surface. Stingray Cementing ran Class H and 1%CaCl. They ran .15%FL-HV AND .25LB/sack of celluseal. They ran .15%C-41-P. 16 barrels of returns. Good circulation. Landed the plug at 1000 psi. Then took up to 1500 psi and released. Floats are holding.
1478581597 2015-11-24 Preliminary Restoration No Violations Status check inspection of the well head found the well equipped with a production tree and the well in line and producing into a gas processing unit. Proper annular pressure surface monitoring gauges are present at the well head. Also present on location supporting the 4 existing wells is a total of 4 gas processing units and a total of 4-400 barrel production tanks. Proper well identification is posted. Addressing the restoration, it was observed that the area affected by the well pad construction has been restored with erosion control and vegetation established.
1763948349 2019-05-28 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. There are (4) producing wells equipped with a plunger lift system, (4) gas processing units (GPU) with sand traps, and (4) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility, wellheads, and entrance. Additionally, concrete barriers surround the perimeter of the wellheads and GPUs. Identification and annular pressure monitoring devices are present at the wellheads. The intermediate casing has a pressure of 400 pounds per square inch (psi). No gauge is available for the surface casing. On 5/28/2019, I spoke with Company Representative Steve Habig to inquire about the presence of a surface string gauge.
1974092581 2015-04-21 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson Drilling rig 531 on location and the 8.750 production borehole drilling process complete with a total measured depth drilled to 17,702 in the directional azimuth of 329.54 degrees with a borehole inclination of 91.02 degrees and a true vertical measured depth of 9,708.17. The 20# P-110 HP casing has been ran in the wellbore and Stingray Cement service is on location to cement the casing.
