0560872956 2014-09-25 Casing String Witnessed No Violations The said well was spudded on 09/25/14 by Butches Rat hole top hole rig with a 110 x 32 conductor borehole drilled and 113 of 26 .375 wall pipe set with Stingray Cement services on location to cement the casing using 416 sacks of Class A-2% calcium .25 pps flake and 30% calculated open hole cement excess ran.
0634363567 2018-02-26 Production Wells No Violations Producing well status check inspection found the well producing off the tubing well head assembly. The tubing well head has (2) lower manually operated master valves between the production head flow cross section and the tubing head flow cross. The production flow cross outlet also has (2) manual operated valves and one valve that is hydraulically controlled remotely and is part of the well pad emergency shut down system (ESD) that can be remotely controlled from the (ESD) switches found at the well pad entrance - each gas processing unit and the production fluid storage facility, each of the (4) well heads also have (ESD) devices present. All (4) well heads have annular pressure gauges present. At the left rear corner of the well pad is the production facility consisting of a total of (4) steel construction tanks of steel design which are located inside a polymer lined steel containment dike area. The lease road and well pad are of gravel design and is in good repair.
0822986291 2019-05-28 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. There are (4) producing wells equipped with a plunger lift system, (4) gas processing units (GPU) with sand traps, and (4) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility, wellheads, and entrance. Additionally, concrete barriers surround the perimeter of the wellheads and GPUs. Identification and annular pressure monitoring devices are present at the wellheads. The 9.625 inch diameter intermediate casing has a pressure of 350 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 65 psi.
0884744502 2015-07-22 Preliminary Restoration No Violations Status check inspection on the well completion process found the well drilling process is now complete and the company awaiting Stingray Pressure pumping stimulation service to complete the well. Addressing the restoration the company has finalized the production equipment placement with a total of 4-400 barrel production tanks present on location along with 4 gas processing units. In review of the affected area by the well pad construction it was observed that adequate vegetation and erosion control is in place and the company will continue to monitor the surface water created on the pad throughout the well completion process.

1419054119 2014-12-20 Casing String Witnessed No Violations 12/19/14 On site for a 22:00 pm call out to cement the surface casing upon arrival it was noted Sidewinder rig 209 is now on the hole and has drilled a 17.5 x 342 surface borehole and ran 342 of 13.375 54.5# casing with CalFrac cement service on location to cement the casing using 458 sacks of class A-3% calcium .25 pps flake 50% calculated open hole excess cement with circulation established prior to cementing and good cement returns to the surface, the casing was pressure tested to 500 psi over lift pressure. Off site 12/20/14 2:00 am.
1716985293 2015-08-11 Completion Testing No Violations Status check inspection on the well completion process found Stingray Pressure pumping service on location standing by to stimulate stage 12 while Nine Energy completes the perforation phase on well # 1 and Stingray stimulates stage 12 on the said well. All 4 wells on the pad have 38 proposed total stages. Oil States service is providing well head control. All stimulation support equipment is located on secondary polymer containment while the earthen well pad perimeter serves as the primary site containment with the drain valves in the close position.
