0174611071 2015-07-22 Preliminary Restoration No Violations Status check inspection on the well completion process found the well drilling process is now complete and the company awaiting Stingray Pressure pumping stimulation service to complete the well. Addressing the restoration the company has finalized the production equipment placement with a total of 4-400 barrel production tanks present on location along with 4 gas processing units. In review of the affected area by the well pad construction it was observed that adequate vegetation and erosion control is in place and the company will continue to monitor the surface water created on the pad throughout the well completion process.

The tubing well head has (2) lower manually operated master valves between the production head flow cross section and the tubing head flow cross. The production flow cross outlet also has (2) manual operated valves and one valve that is hydraulically controlled remotely and is part of the well pad emergency shut down system (ESD) that can be remotely controlled from the (ESD) switches found at the well pad entrance - each gas processing unit and the production fluid storage facility, each of the (4) well heads also have (ESD) devices present. All (4) well heads have annular pressure gauges present. At the left rear corner of the well pad is the production facility consisting of a total of (4) steel construction tanks of steel design which are located inside a polymer lined steel containment dike area. The lease road and well pad are of gravel design and is in good repair.

2029426402 2019-05-28 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. There are (4) producing wells equipped with a plunger lift system, (4) gas processing units (GPU) with sand traps, and (4) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility, wellheads, and entrance. Additionally, concrete barriers surround the perimeter of the wellheads and GPUs. Identification and annular pressure monitoring devices are present at the wellheads. The 9.625 inch diameter intermediate casing has a pressure of 390 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
