0624773956 2015-11-10 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has been restored with erosion control and vegetation in place. Addressing the well- the well drilling and completion process has been completed and crews are on site preparing to switch out the completion well head stack and install the production well head tree. There is a total of 11- 400 barrel production tanks on location along with 9 gas processing units supporting the 5 existing wells on the pad.

0795306842 2014-10-12 Drill / Deepen / Reopen No Violations 10/11/14 I was notified that the company attempted to displace the cement with a wiper plug on the 18.625 mine void string with further review showing the casing not properly set to the 30 minimum depth through the mine void, instructions were given to drill out the plug and shoe and implement a casing shoe test to verify proper annular seal from the shoe to mine void. On 10/12/14 I visited the site and review all documents and laws with the company men on location explaining once again the mine void procedures required by law. District Supervisor Brent Bear Regional Supervisor Rocky King and Deputy Chief Dave Claus are all aware of the situation.
1206358954 2014-10-14 Drill / Deepen / Reopen Violations Noted Administrative inspection to document on 10/11/14 that the 18.625 mine void string was only set 12 through the mine void but the well bore had been drilled to 735 leaving the casing 18 short of the required law of 30 minimum pipe set through a mine void. Instructions are as follows properly encase any future strings according to OAC 1501:9-1-08 well construction rules pertaining to a mine void string as found in section (M) Casing strings (b) Casing through any underground mine void. After drilling through any underground mine void or rubble zone, casing shall be set at least thirty feet but no more than 50 feet below the base of the mine void or rubble zone and cemented at this point.
1412617119 2014-10-06 Casing String Witnessed No Violations Status check inspection on the drilling process found the 26surface casing borehole to a depth of 349, Patterson rig 277 is on location and has ran the 24 weld connection surface casing to a depth of 20 outside the 100 conductor pipe and the casing is setting down and will not go past 120. The company will pull out of the hole with the string and ream the hole with a tri cone drill bit to bottom.
1412809596 2014-10-08 Casing String Witnessed No Violations Patterson rig 277 is on location and has drilled a 26 surface borehole to a depth of 349 and ran 339 of 96.4# weld connection surface casing with a guide shoe ran along with 6 centralizer, Halliburton cement services is on location to cement the casing using 345 sacks of class A cement with 60% open hole cement excess ran and 40 barrels of gel ran ahead of the cement with circulation established prior to cementing and good cement returns to surface, no plug was ran and the casing shall be pressure tested before the next stage of drilling commences.
1413070861 2014-10-11 Drill / Deepen / Reopen No Violations Administrative inspection to document the recent setting and cementing of the 18.625 mine void string. On 10/09 & 10/10/2014 I was in contact with the company men on site multiple times in regards to the running -setting and cementing of the mine void string with the company notifying me they had drilled their borehole to a depth of 735 with the bottom of the mine void at 685 and top at 671, I thoroughly discussed the cementing procedures and requirements per ORC. On 10/11/2014 I was contacted at 6 am that the company was beginning to rig up the cement crew but was unable to attend and at 7:02 pm was notified that the company had completed the mine void cement job, in conversation I discovered the casing had only been set to 697 which is 18 short of the 30 minimum state requirement, I contacted my Regional supervisor Rocky King and he advised the company must complete a casing shoe test before drilling on. The company man on duty was contacted at 7:55 pm and advised to implement a casing shoe test after drilling out from under the casing shoe to confirm the mine void is properly cemented.
1414344282 2014-10-26 Plug / Plug Back No Violations On site inspection to witness the setting of a 800 class G plug in the 12.250 2nd intermediate well bore section due to a directional tool drilling failure causing the well bore to deviate 16 degrees for a total of 27 off centerline course in the directional azimuth of 97 degrees. The plug shall be set through the drill string and the Company will wait 12 hours before proceeding with the drilling operation. Plugging reports and directional survey reports were received.
1415795804 2014-11-11 Plug / Plug Back No Violations 11/09/2014 I was notified by the company men on site that they had lost tools in the 12.250 section at a depth of 9,118 and would be setting a 700 deviation kick plug with Halliburton through fiberglass tubing. (Inspectors Note-This is the 2nd deviation plug set in this 12.250 section on this hole).
1416425769 2014-11-19 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Patterson drilling rig 277 on the hole drilling the 12.250 2nd intermediate wellbore at a depth of 8,888 with compressed air and a true vertical depth of 8,720. The company is pulling out of the hole with the drill string to check the drilling assembly. A copy of the last drilling survey has been received.
1426156830 2015-03-10 Casing String Witnessed No Violations On this day I was on site from 15:00 pm until 18:00 pm with intention to witness the 9.625 casing bond log and pressure test. After standing by on location for 3 hours it was obvious that the procedure was going to take an extended period of time. I explained to the Company men on site that the Division would be awaiting the results by digital format through e mail once the procedures were complete. The records were received and reviewed around 12:00 noon the following day. The company (CNX) was granted a variance by the Deputy Chief Dave Claus and District Supervisor Brent Bear. With the understanding the companys casing cement procedures shall be reviewed in a formal meeting by both parties.
1427497620 2015-03-27 Drill / Deepen / Reopen No Violations Status check inspection on the recent issue of the drill string being stuck in the hole. The well has been drilled to a depth of 20,855 and they have pulled approximately 7 drill stands off bottom to a depth of 20,190. KLX Energy pipe recovery is currently rigged up on the hole and has ran a free point tool showing the pipe free @ 6,700 and tight @ 7,000. The drill string was pulling @ 400,000# hook load.
1427635341 2015-03-29 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling and drill pipe recovery issue. Upon inspection I was informed by the company man on site that they were able to back the casing string off at 6,500 and circulate the hole clean with the drill string being free to that point. They then back the drill string off at 7,000 and circulated the hole clean to that point with the wireline operator on site KLX Energy indicating with the free point log ran they are tight down to 9,000. At tis point the drill string is sitting in the hole separated by 500and the rig has ceased operations to change out their drill line.
1714530283 2016-03-07 Production Wells No Violations Status check inspection to document the well head casing annular pressure control devices. Upon inspection it was observed that the 5.5 & 9.625 annulis has been plumed into a 2 line that is being controlled by a positive choke device with a .375 diameter restrictor plate. There is pressure reducing devices present and installed into the annular pressure discharge line in such a way that it is isolated by a 2 valve and is not being used in normal day operations, also the pop off valves are also plumed to be vented into the atmosphere when in use. The well has no annular pressures greater than 10 psi present. The well head tubing operation pressure present on this day is 6,450 psi.
1849838205 2016-01-17 Production Wells No Violations Inspection report to document the well head pressure test completed this day by Cameron Well head service crews. The 13.375 lower section was pressure tested to 904 psi @ 80@ of the casing collapse pressure. The 9.625 was tested to 5,000 psi (full working pressure) the casing was set in the mandrel well head. The 5.5 casing well head was tested to full working pressure at 10,000 psi. (0) zero pressure was observed as present on the annular side.
1905146247 2019-05-20 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells, (6) gas processing units with sand traps, (11) 400-barrel and (1) 50 barrel steel production tanks within containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 50 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. I spoke with CNX Production Foreman Ron Kinsey onsite.

The 9.625 showed no annular pressure and the well is shut in.
1988504689 2014-08-16 Casing String Witnessed No Violations The said well was spudded on 08/16/2014 and I was on site for a 10:00 pm call out for a conductor cement job with Express Energys top hole rig on location with a 100 x 36 borehole being drilled and set 100 x 30 x .500 casing, Express Energy cement services will cement the casing using 275 sacks of class A cement. I was on site from 10:00 pm on 08/16/14 until 2:00 am on 08/17/2014 and witnessed the casing being ran but not the cement job due to Express Energy having a equipment failure prior to starting the cement job and undetermined repair time.
