0588118646 2014-08-20 Casing String Witnessed No Violations Onsite with Inspector Bill Stolarik to witness the cementing of the conductor job. Express energy augered and cemented the casing back to surface. See his report for this date.
0603012401 2016-01-21 Production Wells No Violations Producing well status check inspection to document the well head working pressures found the 5.5 internal structure has been isolated by the means of a back pressure valve set inside the 5.5 casing hanger and VR plugs set in the tubing head. The 5.5 annular is 850 psi and the 9.625 annular is (0) zero. A company identified as Bolt Tech Mannings has arrived and will be conducting the post weld heating of the 13.375 casing where the 13.375 slip on weld head lower weld has failed. Also on scene is the welding contractor Whites Welding and upon inspection of the weld point where the 13.375 well head was welded to the 13.375 casing a crack was observed in the well head, the worker also reported the well head main body had fractures visible to the eye and the detachable base plate gussets welds were also broken. The contractor had indicated that the multiple structural failures were in his opinion to be fro excessive force being exerted on the well head from an external source. I advised a Consol Representative on site named Cody Craiger that I was suggesting the 5.5 casing be isolated and the well head be changed immediately. My Direct Supervisor Brent Bear along with ODNR Deputy Chief was advised on this day of the well head defects and my recommendation to isolate the well bore and change the busted well head.
0724666495 2016-01-19 Production Wells No Violations Status check inspection to document the well head pressures found the 5.5 casing pressure to be 7,200 psi internally with 750 psi on the annular of the 5.5 and no pressure recorded on the 9.625 annular side. No production tubing is present in the well bore and the well is shut in.
0894205067 2015-06-15 Completion Testing No Violations Status check inspection on the well stimulation process found Halliburton Completion Service on location stimulating stage 10 to be completed with Horizontal Wireline Service on location providing the wireline service and perforating this stage in 4 12 shot clusters between the depths of 18,286 to 18,134 with a solid completion plug set at 18,318. The annular pressure is being monitored electronically with the upper limits being below the set limit of 5,000 psi for this well.
0899838287 2015-06-30 Completion Testing No Violations Status check inspection on the well completion process found Horizontal Wireline Service recently has completed perforating stage 39 to be stimulated with a solid completion plug set at 12,566 and 48 perforations shot in 5 clusters between 12,542 and 12,391. Halliburton completion service is on location to stimulate the well. Discussion was made with the company men on site in regards to the agreement between the company and the Division that the well bore was not to be stimulated past the 11,200 mark.
0928491086 2016-03-07 Production Wells No Violations Status check inspection to document the well head casing annular pressure control devices. Upon inspection it was observed that the 5.5 & 9.625 annulis has been plumed into a 2 line that is being controlled by a positive choke device with a .375 diameter restrictor plate. There is pressure reducing devices present and installed into the annular pressure discharge line in such a way that it is isolated by a 2 valve and is not being used in normal day operations, also the pop off valves are also plumed to be vented into the atmosphere when in use. The well has no annular pressures greater than 10 psi present. The well head tubing operation pressure present on this day is 5,800 psi.
1147446104 2019-05-20 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells, (6) gas processing units with sand traps, (11) 400-barrel and (1) 50 barrel steel production tanks within containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 8 psi. I spoke with CNX Production Foreman Ron Kinsey onsite.
1325172695 2015-05-11 Completion Testing No Violations On site status check inspection found Basic Well Service on location with their work over service rig. In conversation with the company man on site He indicated that during the production string cement job a total of 5,000 of cement had been left in the casing from the casing shoe to app 15,500. A bond log was provided and reviewed showing the top of cement at approximately 10,100 near the top of the beginning of the horizontal curve. Notification was made verbally that the Company management was to make notification to the Division of the defect and the course of action for repair. ODNR Regional Supervisor Brent Bear was made notice of the issue. The company man on site was also made notice that no notification was made to the Division of the casing pressure test of work over operations.
1378075509 2014-12-27 Casing String Witnessed No Violations On 12/27/14 notification was received from the company man on site by phone that they has reached a total drilling depth of their 12.250 2nd intermediate well bore of 9,425 and they have ran 9.625 casing to a depth of 9,327. Upon attempting to circulate it was discovered that their casing shoe track was plugged off and they would be perforating above the casing float collar. The company man indicated the shoe track was 45 which should put their perforations at 9,282. Permission was granted to perforate the casing to allow circulation to be established. A cement bond log and casing shoe test will be required before the production borehole drilling commences.
1408538631 2014-08-20 Casing String Witnessed No Violations The said well was spudded on 08/20/14 by Express Energys top hole rig with a 100 x 36 conductor borehole being drilled and 100 of .500 wall x 30 diameter conductor pipe being set. Express Energy Cement services cemented the conductor using 275 sacks of class A-2% calcium .25 pound per sack flake with 50% open hole excess ran with good cement returns to surface.
1418003043 2014-12-07 Casing String Witnessed No Violations 12/07/2014 On site inspection @ 20:00 hours to witness the surface casing cement job with the following observed. Patterson Drilling rig 277 is now on the hole and has drilled a 350 x 26 surface casing borehole with 333 of 24 97# casing ran and Halliburton cement service on location to cement the casing using 352 sacks of class A cement with 60% open hole cement excess ran and good cement returns witnessed to surface. The casing shall be pressure tested with the Blow Out Preventer installation, test chart received 7:00 am 12/08/14.
1418209545 2014-12-10 Casing String Witnessed No Violations 12/10/2014 -6:00 am On location to witness mine string cement job. 22 hole TD @ 787 top of mine @ 672- bottom of mine @ 687- 18.625 casing set @ 728- cement basket @ 623 guide shoe ran. Halliburton on location to cement the casing with bottom balance job implemented with 319 sacks of class A (Halcem) 2% calcium .25 pps flake 50% open hole cement excess ran. Backside topped off with 565 sacks of class A cement.
1424051308 2015-02-15 Drill / Deepen / Reopen No Violations On site inspection to evaluate the 9.625 Bond Log ran by Halliburton with the review of the CBL Log found the log to be inaccurate and the cement top inconclusive with the Halliburton Casing cement job on 12/28/2014. In discussion with ODNR Regional Supervisor Brent Bear and the Company men on site and after reviewing the cement calculations and cement job log a decision was made to not request any further testing or casing repair, this decision was based on the 1,500calculated top of cement ran and supported by the cement job log showing good circulation - lift pressure & gel spacer returns to surface during the cement job and no casing annular pressure present to date. The casing was initially perforated with my approval above the casing float collar @ 9,278 due to the casing shoe track being restricted with debris. With a casing integrity test being completed with no pressure loss shown also on this day. Upon drilling out from under the 9.625 to begin the drilling of the production borehole drilling a formation integrity test will be also implemented.
1425573487 2015-03-05 Casing String Witnessed No Violations Administrative report to document the notification received from the company man on site that while running their 5.5 production casing at a measured depth of approximately 20,000 the casing began to drag and they were having difficulty getting it to the well bore total depth of 21,204. It was also reported that while working the casing string the casing parted at a depth of 2,179. The company pulled the parted upper section and ran in the hole with a overshot pipe recovery grapple and was able to get ahold of the casing string left in the hole. They were going to attempt to pull the remainder of casing string back out of the hole. ODNR District Supervisor was advised of the situation on this day.

1438241473 2015-06-23 Completion Testing No Violations Status check inspection on the well completion process found Halliburton Pressure Pumping Stimulation service on location standing by to stimulate stage 21 of a total of 46 to be completed. Horizontal Wireline Service has recently completed perforating the stage with a total of 48 perforations shot in 4 clusters of 12 shots between the depth of 16,091 and 15,943 with a 15,000 # rated working capacity completion plug set at a depth of 16,115. The annular casing pressure is being monitored electronically and has recently been tested by the method of bleeding off the pressure at 12:30 AM showing a volume loss to 314 psi in a time of 4 minutes and a total maximum gain to 700 psi in 10 hours.
1452916353 2016-01-15 Production Wells No Violations On this day I received notification by phone at 19:31 hours from a Pete Nickell with CNX Energy in regards to well head gas leak on the Switz #6 well pad. I responded and was on location at 20:30 hours to find the well leaking gas from around the well head inside the cellar below the fluid level. 22:20 hours the fluid level was drained and the well is leaking from between the 13.375 and 18.625. The company men on site are to submit a plan to the Division.
1452957984 2016-01-16 Production Wells No Violations On site inspection as a response to a well head gas leak that occurred on 01/15/2016--evaluated the well head assembly and found the main weld connecting the 13.375 casing to the 13.375 well head sub base was cracked, this lower portion of the well head was noted to have a 5,000 psi working pressure maximum rating. Also noted were the well head inner perimeter seals were pressure tested and leaking pressure above 3,800 psi, the well head company applied a polymer seal by injecting a resin type material into ports into the well head. FMC well control flow back monitoring did arrive on site to begin testing of the well annulis pressures. The Division Chief and Deputy Chiefs along with my Direct Supervisor Brent Bear were part of 2 conference call discussions on this day updating the status of the well head.
1453066381 2016-01-17 Production Wells No Violations Inspection report to document the well head pressure test. The 13.375 lower slip on section failed between the 13.375 seal and the point where the well head is welded to the 13.375 due to a failed weld. The company has been instructed to repair the well and retest the assembly. The 9.625 was set in slips due to the casing not being landed in the mandrel hanger during well construction. The 9.625 annular tested to a top pressure of 3,360 psi working pressure before failing and having to be repacked, in which allowed the well head to achieve a 5,000 psi pressure test. The 5.5 well head casing point tested to 10,000 psi. All pressure test were at 80% of actual casing collapse pressures. Addressing the annular pressure issue- The well initially was found to have 3,400 psi annular casing pressure between the 5.5 annular and 9.625 internal areas of the well bore, the pressure was blown down and after a 22 hour period the pressure has built back up to 500 psi. The 5.5 master valve is shut in at the time of my report at 17:48 hours on 01/17/2016.
1454258662 2016-01-30 Production Wells No Violations On site to witness pressure testing of wellhead after it was repaired, Cameron applied 250 psi to affected area of wellhead and we monitored pressure for 30 minutes with no pressure loss company will continue to plumb up and monitor pressures on annuli of remaining wells on the pad and report to the division if they reach a pressure of 250 psi .
1454427181 2016-02-02 Production Wells No Violations Status check follow up inspection on the recent well head repair work and annular pressure documentation. On 01/30/2016 the weld that had failed where the 13.375 slip on well head is attached to the 13.375 casing was repaired and mag particle tested to confirm that no further cracks were present, the well head and casing was then pressure tested as a combined unit. Addressing the annular pressure - on this day at 3:00 am the annular pressure between the 5.5 casing and 9 casing was recorded at 246 psi the casing pressure was blead down manually to 0 (zero) takin a reported 4 minutes to subside. At 11:00 am the pressure had built back up to 248 psi and the company blead the pressure off to 0 (zero) in 10 seconds. The 5.5 production casing had an internal pressure of 7,552psi.
1465745287 2015-11-10 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has been restored with erosion control and vegetation in place. Addressing the well- the well drilling and completion process has been completed and crews are on site preparing to switch out the completion well head stack and install the production well head tree. There is a total of 11- 400 barrel production tanks on location along with 9 gas processing units supporting the 5 existing wells on the pad.
1908320228 2015-03-06 Casing String Witnessed No Violations Status check onsite inspection in regards to the recent issue encountered with the 5.5 23# P-110 casing being parted at a depth of 2,159.6 with a total of 20,181 casing being ran in the hole initially inside a 8.75 borehole drilled to a measured depth of 21,204. A total of 50-5.5 casing joints was recovered above the point it was parted @ 2,159 leaving a remainder of 18,021 of 5.5 production casing in the hole. The company has a Baker fisherman on location and has stabbed back into the casing string with a overshot and the drill string and is circulating out the casing shoe with 13.60# synthetic oil based mud. The proposed pipe repair plan is to run in the hole with Baker Hughes wireline service and shoot a collar buster shot below the separated casing at a collar joint and then run in the hole with a Bake Hughes Hydraulic operated casing back off tool and attempt to recover the next casing joint looking up. With a proposed plan to then run in the hole and screw the casing string back together. Patterson Drilling rig # 277 is on the hole.
1978705261 2016-01-16 Production Wells No Violations Inspection on this day indicated that 13 wellhead has a crack in it and it leaking gas and company personnel are on site trying to secure the other wells on the pad so they can relieve pressure front the annular space on this well safely.

Flare stack was rigged up and flow line to 500 bbl. tank was put in place and they bled 3400 psi off of annular space in four minutes, company is going to have 24 hr. monitoring of pressures on the wells on the pad while they are shut in waiting on order from the division on when they can place wells back online.
