0122834869 2016-03-08 Production Wells No Violations Status check inspection to document the well head casing annular pressure control devices. Upon inspection it was observed that the 5.5 & 9.625 annulis has been plumed into a 2 line that is being controlled by a positive choke device with a .375 diameter restrictor plate. There is pressure reducing devices present and installed into the annular pressure discharge line in such a way that it is isolated by a 2 valve and is not being used in normal day operations, also the pop off valves are also plumed to be vented into the atmosphere when in use. The well has no annular pressures greater than 10 psi present. The well head tubing operation pressure present on this day is 6,000 psi.
0345716938 2016-01-19 Production Wells No Violations Status check inspection to document the well head pressures found the 5.5 production casing to have 7,180 psi internally and no annular pressure on the 9.625 or 13.375. No production tubing is present in the well bore and well is shut in.
0556571144 2016-01-17 Production Wells No Violations Inspection report to document the well head pressure test completed this day by Cameron Well head service crews. The 13.375 lower section was pressure tested to 904 psi @ 80@ of the casing collapse pressure. The 9.625 was tested to 5,000 psi (full working pressure) the casing was set in the mandrel well head. The 5.5 casing well head was tested to full working pressure at 10,000 psi. Initially on 01/17/2016 the well was found to have a annular volume pressure of 2,400 psi between the 5.5 annular and 9.625 internal casing areas. This pressure was blead down on the evening of 01/16/2016 and after a 24 hour monitoring period (0) zero pressure was observed as present on the annular side.
0628655635 2019-05-20 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells, (6) gas processing units with sand traps, (11) 400-barrel and (1) 50 barrel steel production tanks within containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 8 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 8 psi. I spoke with CNX Production Foreman Ron Kinsey onsite.
1067274147 2015-02-02 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson Drilling rig 277 on location drilling the 8.750 production borehole at a current depth of 14,427 in the directional azimuth of 145 degrees using 12.7# synthetic oil based drilling mud with a projected total depth to be drilled of 21,318.
1093172803 2014-08-19 Drill / Deepen / Reopen No Violations The said well was spudded on 08/18/2014 by Express Energy with a 100 x 36 conductor borehole being drilled and 100 of 30 conductor being set and cemented by Express Energy Cement services with 275 sacks of class A-2% calcium .25 pound per sack flake with 50% open hole excess ran with good cement returns to surface.
1423579045 2015-02-10 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Patterson Drilling rig # 277 running the 5.5production casing at a depth of 11,000 the well bore has been drilled to 21,386. The company is demonstrating proper centralizer placement.
1453396759 2016-01-16 Production Wells No Violations On site to monitor repair work being performed on another well on the pad.

Current annular pressure on this well is 2400 psi recorded between 9 and 5.5 and 500 psi on 13.

1453403334 2016-01-21 Production Wells No Violations Status check inspection to verify the pressures of the well head found a reported 5.5 internal casing pressure along with a 5.5 annular pressure of 100 psi and a 9.625 annular pressure of 150 psi.
1835359709 2016-02-02 Production Wells No Violations Status check inspection to document the well bore annular pressures. The 5.5x 9 showed 210 psi, the 9x 13 showed 210 psi. The company is preparing to put the well back on line in the next 12 hours.
1970485410 2015-11-10 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has been restored with erosion control and vegetation in place. Addressing the well- the well drilling and completion process has been completed and crews are on site preparing to switch out the completion well head stack and install the production well head tree. There is a total of 11- 400 barrel production tanks on location along with 9 gas processing units supporting the 5 existing wells on the pad.
556571144 2016-1-17 0:00: PW No Violations
628655635 2019-5-20 0:00: PW No Violations 0.10
