0050571698 2015-06-10 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well drilling and completion phase is now complete and the well is now in the flow back process stage with 4 gas processing units on location, one to support each well on location. At the production storage facility there is 4-400 barrel production tanks present inside a plastic lined steel containment dike area. Addressing the preliminary restoration it was observed that there is adequate vegetation established within the boundaries of the limits of disturbance. Proper erosion control has been maintained. Preliminary Restoration passed on this day.

A follow up inquire was made to confirm the 1st intermediate was set through the Berea formation and the mud loggers report was showing the Berea depth at 1,934.

Otex was on location with 450 sacks to cement the casing back to surface with 30% excess. Class A 15.6ppg and yield of 1.20. 15 bbl of good cement was returned to the surface. 48 bbl of fresh water was used to displace the cement to 315.

1434389956 2015-06-15 Production Wells No Violations On this day I received notification by phone from Brad Burwell a Gulfport HSE Company Representative who indicated that a Gas Processing Heater had leaked an estimated 5 gallons of Ethylene Glycol (Antifreeze). Upon my arrival on loction at 1:15 pm it was observed that the Company had a total of 3 vaccum water truck mobilized with one truck irrigating the area and 2 truck recovering fluid with no visible product observed. All location drain valves were observed and confirmed to be in the closed position. A Material Safety Data Sheet was received and the company shall recover all standing fluids from the location.
1444835030 2018-02-26 Production Wells No Violations Status check inspection found the well pad equipped with a total of (4) producing wells with each well equipped with emergency shut down devices (ESD) there is also ESD Devices at the production facility and at the well pad entrance and well as the gas processing units. The well head is equipped with a casing head however the casing flow lines are not attached. The well has been recently set up to be produced by the means of a plunger lift system selling into the production flow line. The tubing flow cross section has been removed to install the plunger lift system. Annular pressure monitoring is in place. Also on location is the production facility consisting of 4 steel construction tanks capable of holding 400 barrels of fluid each. The tanks are situated inside a polymer lined steel containment dike area.

Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 20 psi. The production records are current.
