1033885007 2019-05-20 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. The site meets ORC 1509.072(A). I found the pad gated with signage at the entrance. There are (5) producing wells, (6) gas processing units with sand traps, (11) 400-barrel and (1) 50 barrel steel production tanks within containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 10 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 10 psi. It will be noted that a plunger lift system was recently installed. I spoke with CNX Production Foreman Ron Kinsey onsite.
1053116669 2014-06-03 Drill / Deepen / Reopen No Violations On 06/02/14 the mine void borehole was drilled to 723 and the casing ran with difficulties experienced the company attempted to pull the string and it hung up at 677 - 4 below the mine void- on 06/03/14 permission was granted by the Division to cement the string in place and run a 16 liner to the bottom of the original total depth drilled.

1396447361 2014-04-02 Casing String Witnessed No Violations The said well was spudded on 03/30/14 by Redstone Drilling rig RS-30 with 100 of 30 conductor being set inside a 36 borehole and cemented by Allied well services on 04/02/14 using 405 sacks of class A 2% calcium .125 pps flake.
1401923180 2014-05-29 Casing String Witnessed No Violations On 05/29/2014 Savanna Drilling rig # 640 is now on the hole and has drilled a 26 diameter surface borehole to a depth of 362 with Calfrac cement services on location to cement the casing with 353 sacks of class A -3% calcium .250 pound per sack flake with 50% calculated excess ran and no cement plug - the casing will have to be pressure tested at a later date and the cement job was delayed 5.5 hours after my arrival and 8 hrs after they called me out when cementers were hooking up.

1402144459 2014-06-07 Drill / Deepen / Reopen No Violations On the day of my inspection I was on site to witness the cementing of the backside of the 16 inch mine string. Savanna Rig #640 ran 3 joints of 1 inch pipe between the 18.625 inch casing and the 16 inch liner. CalFrac Well Services was on site and pumped 143 sacks of Class A cement 15.6ppg with a yield of 1.18cuft/sk. Cement stayed to surface.
1402602997 2014-06-12 Drill / Deepen / Reopen No Violations Status check inspection of the drilling process found Savanna Drilling rig # 640 on the hole currently drilling the 1st intermediate well bore at a depth of 1,500 using a 14.750 diameter drill bit, the company man reported they are having trouble with the red rock collapsing and have pulled the drill string out of the hole and are preparing to switch over from air to fluid drilling.
1403018705 2014-06-17 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I witnessed Savanna Rig #640 was drilling ahead at 2365 feet with a 14.375 inch tricone bit on 9.7# water based mud with a td of 2545 feet. Containment repairs were being made and remediation to the slip area was holding with further remediation and seeding to take place in July.
1403256439 2014-06-20 Casing String Witnessed No Violations On 06/20/2014- 2,495 of 11.750 intermediate casing was set and cemented by CalFrac well service using a lead and tail class A cement with a cement wiper plug ran and 297 barrels of H2O displacement.
1409226965 2014-08-28 Casing String Witnessed No Violations Status check inspection found the 2nd intermediate well bore total drilling depth has been reached at a depth of 9,465 using a 10.625 diameter drill bit and 9,434 of 8.625 casing has been ran with 151 centralizers a float shoe and guide collar ran on the casing, the company is currently circulating on the hole with drilling fluid and CalFrac cement services is standing by to cement the casing.
1410220936 2014-09-08 Plug / Plug Back No Violations Notification was received on 09/07/2014 that while drilling the 7.875 horizontal curve well bore section at a depth of 10,608the company was experiencing difficulty building the proper curve section of well bore and would be setting 2- 500 cement plugs back up into their last casing string being the 8.625 2nd intermediate set to a true vertical depth of 9,435, CalFrac Cement services was the cement contractor setting 2 plugs through the drill string both being of Class H 17 # -0.98 yield cement with only DCP-FR2 friction reducer as a additive, 15% cement excess was ran.
1417093794 2014-11-25 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Nomac rig #82 on location drilling the 12.250 2nd intermediate well bore at a current depth of 4,149. A 15 degree well bore nudge was implemented beginning at 1,100 at a direction of 163 degrees azimuth.
1441899595 2015-09-10 Completion Testing No Violations Status check inspection n the well completion process found Deep Well services tubing snubbing unit rigged up on the hole and currently drilling out the completion plugs at a depth of 15,600 with Cameron service well head tree and FMC company flow back services on location. There is 5 total wells on this location that have been drilled and are in the completion process phase.
1451303546 2014-06-04 Casing String Witnessed No Violations On 06/03/14 I was called out at 9:45 pm to witness the 18.625 mine string cement job, a bottom balance job was executed and then a backside grout job was also done with both cement jobs being set through 1.250 tubing, total on site time was 12 hours.
1561210742 2016-01-17 Production Wells No Violations Inspection report to document the well head pressure test completed this day by Cameron Well head service crews. The 13.375 lower section was pressure tested to 904 psi @ 80@ of the casing collapse pressure. The 8.625 was tested to 2,744 psi @ 80% of the casing collapse pressure due to the casing being set in slips and not the mandrel well head. The 5.5 casing well head was tested to full working pressure at 10,000 psi. Initially on 01/17/2016 the well was found to have a annular volume pressure of 2,250 psi between the 5.5 annular and 9.625 internal casing areas. This pressure was blead down on the evening of 01/16/2016 and after a 24 hour monitoring period (0) zero pressure was observed as present on the annular side.
1819654372 2016-03-07 Production Wells No Violations Status check inspection to document the well head casing annular pressure control devices. Upon inspection it was observed that the 5.5 & 9.625 annulis has been plumed into a 2 line that is being controlled by a positive choke device with a .375 diameter restrictor plate. There is pressure reducing devices present and installed into the annular pressure discharge line in such a way that it is isolated by a 2 valve and is not being used in normal day operations, also the pop off valves are also plumed to be vented into the atmosphere when in use. The well has no annular pressures greater than 10 psi present. The well head tubing operation pressure present on this day is 4,654 psi.
1839535464 2015-06-11 Completion Testing No Violations Status check inspection on the well completion process Found Horizontal Wireline Services on location with the 15 stage recent perforated with a completion type bridge plug set at a depth of 14,556 and 48 perforation shot in 4 -12 shot clusters between 14,380 and 14,532 with a 50 cluster spacing and a 200 stage spacing. Halliburton Pressure Pumping Service is standing by to complete the stage.
2100177935 2015-11-10 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has been restored and erosion control in place along with vegetation established. The well has had the drilling and stimulation process complete and has the production well head installed and is awaiting the installation of the production flowlines. There is 11-400 barrel capacity production tanks on location place inside a polymer lined steel containment dike area. Also on location is 9 gas processing units.
