0096337424 | 2013-03-20 | Drill / Deepen / Reopen | No Violations | Inspector on site @ 8:30 am to pressure test the surface cement casing remedial work squeeze job. Cement was tagged inside the casing at 270 with the perforations @ 276 to 280, at 282 we fell out of the cement and ran 3 Kellys past that point and circulated the hole clean. We then pulled the drill string out of the hole and put the cement reducing swage back on the 7 and pressure tested it to 350 psi with no recorded loss or communication to the annular side of the surface pipe. Inspector approved the remedial work as complete. |
0335907627 | 2013-03-14 | Drill / Deepen / Reopen | No Violations | A notification phone call was received by Kerry Brown from Monroe drilling asking permission to start the drill site excavation. I responded to the site and after reviewing the site and asking Kerry to try and stockpile and save as much top soil as possible permission was granted to excavate the site. Kerry stated He would be building Becks drill sites and would practice good site construction management trying to preserve the topsoil for reclamation purposes. |
0533223198 | 2013-07-30 | Preliminary Restoration | No Violations | Inspection of the well site found the drilling pits filled and the well in production with 4.5 production casing and 1.5 tubing and pumping rods and the production valves open and the well being produced by the means of a pumping unit. 2-100 barrel production tanks are present in a established dike along with a mechanical separator and production gas sales meter. Inspector will note that notification was made to Beck energy that final grade work still needs done around the well site as well as better vegetation needing established. |
0704109582 | 2013-04-02 | Drill / Deepen / Reopen | No Violations | Status check inspection of the current drilling progress. Notification was received by Becky Sims that the well pressure has been controlled and that they were running back into the hole with the drill string on fluid to complete the drilling of the well bore. |
1363376365 | 2013-03-15 | Drill / Deepen / Reopen | No Violations | Inspector on location to set the 9.625 conductor pipe and upon arrival I noticed they only had 64 of pipe on location. After speaking with the drilling contractor I made them aware that they did not have the minimum of 100 of conductor pipe as the permit required. The hole depth was then drilled to 140 and 130 of conductor was set and cemented back to surface. Inspector on site 6 hrs. |
1363695647 | 2013-03-19 | Drill / Deepen / Reopen | No Violations | Inspector on site for a 10PM callout on 03/18/1013 for a 7 surface job, Upon arrival the rig was still pulling drill pipe and not ready, 888 of 7 surface was set in 920 of borehole, The cement did not circulate and the plug stopped short of landing, The cementers and myself suspect the 1st red rock formation collapsed @ 625 which is where the plug sat down. The surface will be grouted on the long string cement job. Inspector off location 2 Am. |
1363720895 | 2013-03-19 | Drill / Deepen / Reopen | No Violations | On site status check on Surface job from the following evening found that once they run in the hole with the drill string the top of the cement is at 480 inside of the 7 surface pipe which is 888 in total depth. The cement will be drilled out within 40 of the casing shoe and the surface casing will be bond logged. Once we see the top of cement a remedial plan will be put into place if needed. Inspector will return @ 6PM to witness the bond log. |
1364314542 | 2013-03-26 | Drill / Deepen / Reopen | No Violations | Status up date of the drilling progress found the current drilling depth of 2230. The well has pressured up and the drilling company has pulled the drill string out of the hole and has a swage on the 7 surface and well head control in place. They have tried to kill the well with 10# mud unsuccessfully. With plans to mix heavier mud and try to kill the well with a pump truck. |
1364506744 | 2013-03-28 | Drill / Deepen / Reopen | No Violations | On this day I was on site with MRI 3 Pat Shreve to evaluate the well bore that has occurred a high amount of gas pressure during the drilling process. 3rd party fluid and high pressure pump contractors have been brought in to try and kill the well, but due to the unusual circumstances of the drill string being pulled out of the hole and a reducing swage being put on the 7 surface casing with a low pressure nipple and valve, the decision was made to put the well in line and sell the well down until proper pressure rated equipment can be installed. |
1366238325 | 2013-04-17 | Drill / Deepen / Reopen | No Violations | On this day I observed Universal Well Services- Completion Services Trucks traveling in the direction of the location. After attending to other business I traveled to the said well site and observed the stimulation process in progress. I observed and photographed the operation in progress then had a conversation on site with Ray Beck the company owner in regards to the no notification being made to me about the up and coming completion work on the well. Mr. Beck said His assistant Becky Sims had been very sick and most likely forgot to call me. I discussed the notification requirements per their permit conditions and advised Mr. Beck He must notify me as the permit required. I observed the complete stimulation process on this day. |
2036419706 | 2014-07-31 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the well site restoration meets ORC 1509.072 with proper vegetation established on location and the following equipment present at the well head 4.5 production casing- 1.5 tubing and pumping rods with the production valves open and the well set up to be produced by the means of a pumping unit, at the production fluid storage area 2-100 barrel production tanks are presen along with a mechanical separator and gas sales meter with a current sales chart all located in an established dike area with proper well identification posted. |
