0008224097 2013-11-06 Completion Testing No Violations The well bore drilling has been completed and the company is completing the water line supply line from the Ohio river, the well shall be stimulated in the near future.
0048681561 2013-06-10 Casing String Witnessed No Violations On this day remidial work was performed on the 13.375 casing string cement job. Grey wire line bond logged the well and showed a annular cement top at 556 with a casing TD of 807. We then filled the Annular of the 13.375 with 53 barrels of fresh water with a timed loss of 3 barrels per minuite the decision was made th grout with a 1 tubing string and not squeeze the cement through perforations. The total annular volume to be filled is 53 barrels, we spotted 5 barrels of cement and waited 1 hour and then topped the hole off with 50 barrels of Class A- 3% calcium- 1/4 # flake cement. ODNR Supervisor Rocky King and Division Deputy Chief Dave Claus was in communication with me and kept updated during the process.

There are (3) producing wells, (2) gas processing units with sand traps, (5) 400-barrel steel production tanks within steel containment, and a flare stack present. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The production records are current.
0221873774 2014-05-07 Completion Testing No Violations Status check inspection on the completion process found the well has been stimulated and the company is awaiting Halliburton Completion coil tubing unit to arrive and drill out the completing plugs, a site inspection was done and all 500 barrel flow back tanks have proper containment erected and are grounded, 4- 400 barrel production tanks are set up with proper containment in place.

There is 4- 400 barrel production tanks located in an plastic lined metal liner with 2 of the tanks present related to the said well along with 2 in line heaters on location and a positive pressure flair stack burning the excess gases of the production tanks. Preliminary Restoration was passed on this day.
0601024860 2018-02-27 Production Wells No Violations Status check inspection found the well equipped with a casing well head with (2) valves on one side of the casing head and no valves present on the opposite side. No flow line is attached the well casing well head and the well is set up to be produced through the production tubing head with (2) manual valves present and (2) (ESD) Emergency Shut Down valves present. The (ESD) valves can be remotely controlled by computer or are set up to be pressure deviation controlled, the ESD devices also have fire monitors on the production tanks and (2) manual shut offs at the production facility. The well is currently shut in. The production facility consists of a total of (5) tanks of steel construction which are situated inside a polymer lined steel containment dike area. On this day I contacted XTO Production Foreman Tracy VanScoy who indicated the crews on site were removing fluids and gravel from the tin horn cellars to check all annular pressures on the well heads.
1120523592 2013-06-07 Drill / Deepen / Reopen No Violations Notification was received by the company man on site that during the cementing of the 13.375 intermediate casing string that total circulation was lost. Remidial work is to begin on 06/10/2013.
1190320034 2013-06-13 Drill / Deepen / Reopen No Violations Inspection found First Class energys rig drilling at 2200 using a 12.500 bit on air. With the projected depth for the current well bore section to be 2454, The 9.625 casing will be set next with the projected casing tally received and reviewed on this day.
1368829186 2013-05-17 Casing String Witnessed No Violations On this day I witnessed Express Energy Services drill and set 100 of 26 conductor in a 30 borehole with Universal Well Services cementing the pipe in place. Cement used was 225 sacks of Type 1- 3% calcium-1/4# flake with cement returns to surface.
1369755568 2013-05-28 Drill / Deepen / Reopen No Violations Status check inspection on the drilling of the 18.625 surface casing found the drilling company First Class Energy drilling at 70 inside the conductor pipe. The company was mixing a Non-Biodegradable soap and I had to inform them that they were drilling their surface casing string used to protect the fresh water strata and only Bio-Degradable soap was allowed to be used. I also had to have XTO correct their permit because the original permit allowed the surface and the mine void string set as one casing string.
1369851105 2013-05-29 Casing String Witnessed No Violations On site inspection to witness the running and setting of 320 of 18 surface casing, The original permit conditions were deviated from 540 to 320 with permission from the ODNR Permitting Section to avoid drilling into the Mine void.

The drilling co First Class Energy rig # 6 has pulled out of the hole with the drill string and is preparing to pick up and run in the hole with directional drilling tools to begin a 5 degree nudge to the South West. The surface casing and Bop was pressure tested to 400 psi on air with no pressure loss.
1376585651 2013-08-15 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found the well bore has been drilled to the total drilling depth of 12,776 and the rig is running the 5.5 production casing. A casing tally was received and reviewed. The true verticle drilling depth is 6,203 and calculated cement top is 4,500 with 15% excess to be ran on both the lead and tail cement jobs.
