0386223558 2013-12-12 Drill / Deepen / Reopen No Violations Well has been spudded but not drilled to total depth. Rig is at location and shut down on this day. There is 7casing with a swedge and a 1plastic gas line hooked up coming from the field. There is no check valve in site. There is a gauge with no pressure on day of inspection. The drill pit has no free board and the fluid needs to be removed before the pit walls are breeched. If this happens the water will go down over in field and hit the lower creek. The county inspector Bill Stolarik was there on day of inspection that will reflect this.
0413021317 2013-07-01 Drill / Deepen / Reopen Violations Noted Inspection and compliance notice that the driller has not followed the Divisions Permit conditions and a improper unapproved casing string has been set. Inspection found a 9 casing string set at a unknown depth somewhere from 235 to 300 with no annular space and the string not cemented in place according to the laws of the division under 1509.17. The last known recorded casing was 10.750set at 106. I also will note that the driller is bailing the drill cuttings out on the ground surface and a proper flow ditch is not established to the drill pit. It was observed that the driller had set fire to the well bore at the time of my arrival and stated He did not want to smell the gas. I also observed a oil spill from under the rig that shall be cleaned up immediately. The driller shall cease all activity immediately contact the Permitting section of the Division. 0418180157 2016-01-04 Plug / Plug Back No Violations On site status check inspection as a follow up to Chiefs Order # 2015-344 found Harmons well service on location attempting to pull the unapproved 5.1875 casing string that was set in the well to a approximate depth of 610. The casing was ripped at the 2nd collar up from bottom at approximately 620 and recovered from the well bore. The rig then proceeded to run a 1.5 tubing string into the well bore and was unable to get past 620,

0666372810 2015-05-08 Production Wells No Violations Compliance Notice # 1428512346 follow up inspection, on this day I visited the Gatherco meter station the said well sells into and observed the Roots #8 -C rotary style meter with an rotary index reading of 0543 of gas sales being observed. Upon inspection of the well head it was observed that the surface casing gauge read 85 psi with isolated pressure between the un cemented 5 casing string and the 7 surface casing. No well identification has been posted and the production tank has yet to be relocated away from the stream bed and existing well known as 3411163157-0000.
0867272735 2015-12-29 Plug / Plug Back No Violations On site status check inspection as a follow up to Chiefs Order # 2015-344 found Harmons well service on location attempting to pull the unapproved 5.1875 casing string that was set in the well to a approximate depth of 610. It was observed that the rig was pulling 40,000# by measurement on the weight indicator and was unable to pull the casing string. The rig owner Troy Harmon had left site to and was in route to pick up a casing knife, directions were given to rip the casing as close to the 7 surface casing shoe as possible to provide maximum coverage when plugging. As a last resort the 5casing will be shot off with a explosive collar buster shot. The plugging contractor Harmons well service indicated the 5 string was drove in. My direct supervisor Brent Bear was updated on the status of the plugging progress.
0901057624 2012-12-10 Drill / Deepen / Reopen No Violations Review of the casing program on the drilling permit found the proposed casing program not in compliance with the ORC Laws. Notification was made to Division Permitting section and corrections were made accordingly with notification being made to Ed Crum of the revised casing program. I have been in contact with Mr. Crum on several occasions in regards to His drilling site and proposed program.
1002938823 2012-12-25 Drill / Deepen / Reopen No Violations Inspector was in contact with the well owner Ed Crum in regards to the procedure in which He has planned to run and set the conductor pipe on the said well. Per orders of the Division supervisor Rocky King the conductor pipe will be cemented as ORC requires. Mr.Crums intention to drill the conductor hole the same size as the diameter as the conductor pipe and drive the pipe into the well bore in sections and weld the sections together will not be accepted as a Division approved method.
1356706126 2012-12-28 Drill / Deepen / Reopen No Violations Well was spudded on 12/21/12. Status check on the current drilling progress found the current drilling depth to be 40 using a 10 bit, the driller says they plan to drill down app 100 and then ream the hole to 13 and set 10.750 conductor. Inspector went over the procedure of cementing the conductor and surface casings as well as the ODNR rules for lining the fluid return ditch to the drilling pit.
1359791959 2013-02-01 Drill / Deepen / Reopen No Violations On this day I was on location 4 hours to witness the running and cementing of the 10.750 conductor pipe with 106 of conductor being run in 108 of hole. Inspector will note that the pipe had to be grouted due to insufficient cement returns and the well is being drilled by the owners own rig and crew.

In phone conversation with the landowner John Ackerman it was stated that they are working on the weekends periodically.

1372688290 2013-06-28 Drill / Deepen / Reopen No Violations Inspection and follow up to prior notification that I have given verbally to Ed Crum that He was not following the proper casing program. Notification was received from Troy Harmon on 06/28/13 that they would be running 7 surface on Monday 07/01/13, In conversation I asked Troy if they had installed the 9 liner that Ed had previously mentioned in conversation and He indicated that they did. I advised Him that it was a unapproved casing string and it would need removed prior to running and cementing the surface casing.
1372725544 2013-07-01 Casing String Witnessed No Violations On site inspection to witness the running of the 7 surface casing and proper centralizer placement. The unapproved 9.625 casing string set at 365 shall be pulled the next day before cementing the surface casing.
1372858537 2013-07-02 Drill / Deepen / Reopen No Violations I stopped by location at 8:00 am and no one was on site, Upon phone conversation with Ed Crum at 8:30 am he stated they would not be working on removing the 9.625 unapproved casing string, only pulling up the rig floor to bring in casing jacks on 07/03/13. A violation has been issued and the driller is to remove the 9.625 unapproved casing string.
1373372363 2013-07-08 Drill / Deepen / Reopen No Violations Ed Crum notified me at 7:15pm that they would be welding a 9.625 short joint and collar to the unapproved casing string tomorrow in order to try and pull it out of the hole. He also stated they would not work on the well the rest of the week after completing that procedure.
1373574686 2013-07-11 Administrative Inspection No Violations Ed Crum called and stated His crews were on vacation and the would not be working on the well trying to remove the unapproved casing string. He also stated that He has been in contact with with Formation Cementing about mixing a Super Mud pill to put in the hole while they remove the casing to support the bore hole. I only advised Ed crum that any chemicals must be enviormentally friendly for this is the fresh water strata we are dealing with.
1374099595 2013-07-17 Drill / Deepen / Reopen No Violations Notification was received from Ed Crum that they had the casing jacks set up on the unapproved casing string and was able to move the casing up several feet. Upon arrival on site I discussed a plan of action and the company has plans to pull the casing the following morning and cement the 7 surface casing in place.
1374191318 2013-07-18 Administrative Inspection No Violations On this day I received a call from Ed Crum after the cementing of the 7 surface casing on the said well. It was explained to Mr. Crum that the surface casing was to be grouted to surface due to no cement returns. I also discussed with Mr. Crum that the casing was to be pressure tested before proceeding and that they were to establish a proper lined flow line ditch from the well head to pit. I also discussed the proper hauling and disposal of the drilling pit fluid along with stressing the fact that they were using a open hook with no safety latch on the blocks on the rig and they needed to correct all these issues before resuming drilling.
1374703177 2013-07-24 Casing String Witnessed No Violations On this day I witnessed the pressure testing of the surface casing, the casing was tested to 500 psi with no loss greater than maximum pressure requirements.
1378821543 2013-09-10 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found the current drilling depth to be 600 using a 6.5 bit. The well is being drilled with a sputter rig.
1379447780 2014-10-09 Production Wells No Violations Compliance notice # 1387375718 follow up found the drilling pit has been closed and the cutting removed prior to pit closure with vegetation starting to show establishment in the former pit area. The cuttings removed from the pit are however still remain placed on the earth surface on location and have not been properly removed. The owner has his swab rig on the hole and appears to be randomly swabbing the well bore. The issues in the initial violation have yet to be addressed to the level the violation can be resolved.
1386515188 2013-12-08 Drill / Deepen / Reopen No Violations On this day I returned a call made to me on 11/04/13 by the landowner John Ackerman, John stated that while hunting on His farm He observed the idle drilling pit overflowing down the hollow from the recent rains, He also requested that the 1 plastic lines buried randomly across the property be buried so He can plow, I asked John if He at this time would like to officially file a complaint and He said yes, Complaint 4554 was entered in the Database on this day and the well will once again be inspected on 12/09/13. John also confirmed my suspicion that they are drilling at night and on the weekends as to the reason I can not ever catch them at the site.
1386708211 2013-12-10 Drill / Deepen / Reopen No Violations Compliance agreement follow up found the company men on site drilling at a depth of 835 using a 4.250 spade bit with the un cemented 5 casing set as a liner at 606. I checked the chlorides in the drilling pit and they were found to be 200 parts per million along with a 1 layer of ice on the pit and a fluid level that is acceptable and not a threat to immediately overflow.

I will continue to monitor the drilling activities and pit level throughout the drilling process before quieting violation.
1389921305 2013-07-08 Drill / Deepen / Reopen No Violations Inspection found the rig crew preparing to attempt to pull the 9.625 unapproved casing string out of the hole. I spoke with Ed Crum in regards to the 7 surface casing in the hole and advised Him to remove the string before pulling the 9.625 to avoid the unstable material from collapsing in on the annular of the 7. I also spoke with my supervisor Rocky King about the ongoing issues with this well. Ed Crum has been ordered to remove the 9.625 string and properly set the 7 surface or plug the well.
1393263361 2014-02-24 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress and compliance notice follow up inspection found a 2.875 intermediate casing string has now been added to the well bore, the drilling pit is free of fluid but is still standing open. A 5 casing string that has been added was never cemented, I shall speak with my supervisor on the matter.
1398083282 2014-04-17 Administrative Inspection No Violations In conversation with the worker for CGO Inc at another field site he indicated that he has tried to clean out the production borehole and it keeps collapsing and the well is making no gas or oil.
1398702948 2014-04-28 Drill / Deepen / Reopen No Violations Status check inspection found the company tool rig still standing on the hole with the 2.875 tubing and packer that was installed in the hole now pulled from the well bore. The drilling pit is still open and the well has yet to be completed with my last report from the company man indication the red rock is collapsing and he was unable to get back to bottom with the drill tools.
1402336050 2014-06-09 Drill / Deepen / Reopen No Violations Compliance follow up inspection found a rotary drilling rig on the hole and the company man on site reported the well has been drilled to 1333 using a 4.5 diameter drill bit, the rig is pulling out of the hole laying down the drill string.
1402937125 2014-06-16 Drill / Deepen / Reopen No Violations Compliance notice follow up inspection found the last drilling rig to be on the hole has been removed and the well now has 2.875 tubing in the hole, the drilling pit has yet to be closed.
1405030750 2014-07-10 Preliminary Restoration No Violations Status check inspection with supervisor Brent Bear on the preliminary restoration found the drilling pit has not been closed, directly below the drilling pit chloride tests were executed using low level Quantab strip tests and chloride levels were measured at 643 parts per million and a iron scales/bacteria sheen was present in the puddles in the wet weather stream bed, a half round tank was present full of well fluid near the well head along with a swabbing rig set up on the hole, 2.875 tubing has been installed back in the hole since my last inspection, and the restoration of the well site has yet to be completed and is overdue.
1406821085 2014-07-31 Preliminary Restoration No Violations Compliance Notice # 1387375718 issued on 12/03/13 follow up inspection found workers removing the cuttings from the pit with a backhoe and hauling them up the bank and placing the material on a plastic liner at the rear of the drilling location. I notified my ODNR District supervisor Brent Bear and informed him that the drilling cuttings were being removed from the drill pit but were not being placed in a liquid tight container and the method being used could allow the product to further contaminate the area, Brent was to follow up on a recently prepared Suspension order that has been sent to the Divisions Legal staff and see if the order has been issued.
1407032906 2014-08-02 Administrative Inspection No Violations Administrative inspection to document the recent field enforcement action against the well owner Edward Crum. On 12/03/2013 a Compliance Notice was issued against the producer on 12/03/2014 and activities on the said well site due to the workers on site allowing the drill pit to overflow and the landowner filing a complaint against the producer. In recent inspections it was observed and document that the drilling process is complete and the drilling pit had not been properly closed and well related product was found outside the drilling pit. Mr. Crum has been notified by the Divisions District Supervisor Brent Bear that the company CGO Inc must properly solidify the drilling pit in place or remove the product from the drill site to a EPA approved landfill. At 5:40 pm Mr. Crum notified me and reported they had solidified the drilling pit with lime and conducted a paint filter pass through saturation test and he had approved the product to be hauled off site. Mr. Crum has been in contact with me on several occasions over the last 48 hours and I have informed him that the cuttings need solidified and buried on site or removed to a EPA approved landfill.
1416241381 2014-11-17 Preliminary Restoration No Violations Compliance Notice and Preliminary Restoration follow up inspection. On 10/29/2014 I met on site with the well owner Ed Crum and informed Him that the 1/2 round tank he is using to swab the well into must be kept empty and not allowed to overflow, and the drilling cuttings placed on location that has been removed from the drilling pit must be removed off site. On this day it was observed that the 1/2 round steel construction tank with an approximate fluid capacity of 50 barrels is full to the top and with the current raining weather conditions find the tank ready to overflow, chloride levels of the fluid taken with Quantab test strips indicated the chlorides exceeded both the high and low level readings. The drilling cutting is still on site stored on and under plastic but not liquid tight.
1416517232 2014-11-20 Preliminary Restoration No Violations Compliance Notice # 1387375718 follow up inspection. It was also observed that the company owned swabbing rig is still standing on the hole and the 50 barrel 1/2 round steel tank on location was still standing full of fluid such as I observed on 11/17/14. A through review was done of the area trying to find the gas metering chart related to the said well to confirm the well is in production with a gas compressor being found on a adjoining lease with multiple 1 plastic lines feeding into the compressor and one single 1 gas outlet line with no gas metering equipment being observed. The complaint issued against the said producer is considered still open and the violations issued are not resolved. The well location has not been reclaimed and properly restored.
1420029977 2014-12-30 Drill / Deepen / Reopen No Violations Compliance notice # 1387375718 issued on 13/03/2013 has been resolved on this day. Upon inspection it was observed that the drilling pits have been closed and the affected area by the drilling process restored. I will allow time for vegetation to establish before the preliminary restoration can be signed off on.
1422107877 2013-12-03 Drill / Deepen / Reopen No Violations Inspection on 12/03/13 observed that the drilling pit was once again being allowed to overflow, A complaint received by the landowner on 12/04/13 was also reported that the drilling pit was being allowed to overflow from the recent rains. Complaint number 4554 was filed against the operator on 12/08/13. It was also noted under 1501:9-10-05 Burial of Pipelines (1) The land across the pipeline is to be laid is not reasonably expected to be under cultivation. The landowner asked that the pipeline be buried so He can cultivate His fields, (when weather permits before the next growing season). Instructions are as follows- Evacuate the drilling pit immediately and monitor the pit at the beginning and end of every week as well at the end of every work day recording the pit level on paper form inside the drillers shack and keep accurate records of fluids removed.
1422296109 2015-01-26 Production Wells No Violations Status check inspection found the following equipment at the well head- 7surface casing and well head along with a 5casing string set as a liner and hung in the 7 well head and not cemented or on a packer. The 7 well head has 2 ports and a pressure gauge reading 50 psi reading on the gauge. There is 2.875 tubing set and a 1 sales line with a valve in the closed position and a pressure gauge reading 75 psi on the tubing. On this day it was observed that there is annular gas pressure on the 5 liner. No gas sales meter is present on location.
1423156228 2015-02-05 Production Wells No Violations Status check inspection to verify the well GPS point. On this day at the time of my inspection I witnessed Troy Harmon on location opening up the 1 sales line at the well head. I requested to be shown the location of the gas sales meter. We traveled to a gas sales meter located on Township road 56 where a Gatherco Meter station was pointed out as the sales meter for the said well. The sales meter digital gauge was reading but not the paper sales chart and the meter sold for approximately 2 minutes and stopped. Mr. Harmon indicated at least 2 other CGO wells no associated to the property the said well is located was selling into the same meter.
1428512346 2015-04-08 Production Wells Violations Noted Upon inspection on this day it was observed that the said well has now had 1.5 tubing and pumping rods installed along with a pumping unit with a 1 fluid production flow line present at the well head and 2 flow line going into the tank battery. Upon observation of the tank battery a newly set 100 barrel production tank was found inside a containment dike that has also been newly excavated. Upon taken measurements it was noted that the tank was set 20 from a existing well known as 3411163157--1--507-6592.
1429780085 2015-04-22 Production Wells No Violations Compliance notice # 1428512346 follow up inspection and administrative report to document a conversation that I had with Troy Harmon who is representing CGO Inc as a contractor over the field production and documentation. On this day I visited both the well site and tank battery. Upon inspection it was observed that the well has not had the corrections made to properly isolate the unapproved 5 casing string. Upon inspection of the tank battery it was also observed that the tank has not been relocated away from the stream and existing well known as 3411163157-0000. Also noted was a phone call received at 9:08 am and a conversation that took place between the contract field producer Troy Harmon in regards to the well having a un-cemented 5 casing string along with the tank battery needing moved and the lack of reported production for the last 2 years by CGO Inc. I indicated to Mr. Harmon that the well Owner Ed Crum had not contacted the Division in regards to the violation and unapproved casing string and needed to do so before the 30 days noted in the violation or I would move foreword with a Order. It was stressed to Mr. Harmon that CGO had no individual gas sales meters set at any of his wells to confirm the gas production.
1430844362 2015-05-05 Production Wells No Violations Compliance Notice # 1428512346 follow up inspection found the following present at the wellhead; 7 surface casing -2.875 intermediate string & 1.5 production tubing and pumping rods with the well set up to see gas off the 2.875 casing and all production valves in the open position. In addressing the unresolved violation issued on 04/08/2015 regarding the 5 unpermitted casing string set in the well bore it was observed that no corrections have been made in regards to the string and no communication has been received from the well owner concerning the violation, the surface casing was observed to have 95 psi on the gauge on the day of my inspection. At the production facility the 100 barrel production tank has not been relocated from the existing stream or wellhead a minimum of 50 as described in the violation. It was also observed at the Gatherco meter station a Roots #8-C-Rotary style meter with a pressure recorder is present showing a current reading of 0543, I shall monitor the sales activity to confirm the system the well is feeding into is actively selling gas. No well identification has been posted.
1430942212 2015-05-06 Production Wells No Violations Compliance Notice # 1428512346 follow up inspection, on this day I revisited the Gatherco meter station the said well sells into. On 05/05/2015 the Roots #8 -C rotary style meter was visited and rotary index reading of 0543 of gas sales was observed and on this day 05/06/2015 the meter was revisited at 11:00 am (26 hours later) and the meter was still reading 0543. The ambient air temperature is 85 degrees and sunny.
1431468816 2015-05-12 Production Wells No Violations Compliance Notice # 1428512346 follow up inspection, on this day I visited the Gatherco meter station the said well sells into and observed the Roots #8 -C rotary style meter with an rotary index reading of 0544 of gas sales being observed. Upon inspection of the well head it was observed that the surface casing gauge read 85 psi with isolated pressure between the un cemented 5 casing string and the 7 surface casing. No well identification has been posted and the production tank has yet to be relocated away from the stream bed and existing well known as 3411163157-0000. The producer has yet to contact the Division in regards to the compliance notice.
1434728462 2015-06-19 Production Wells No Violations Compliance Notice # 1428512346 follow up inspection, on this day I visited the Gatherco meter station the said well sells into and observed the Roots #8 -C rotary style meter with an rotary index reading of 0549 of gas sales being observed. Also noted that there is no individual gas sales meter or meters present for any of the producing wells selling into the Gatherco Gas sales system meter point. With the contract pumper Troy Harmon reporting multiple lease do feed into the sales meter. Addressing the 100 barrel production tank set near the existing well 3411163157-000- and nearby waterway found the tank has been set back to meet the minimum requirement of 50 from both mention items. Inspection of the well head found the following equipment present- 7 surface casing with a pressure valve in plane sight showing 90 psi. 2.875 tubing and 1.5 tubing and pumping rods with the production valves in the open position. Also noted and photographed the check valve has been removed from the 2.875 casing that could allow the gas to back feed into the well bore and contribute to the high surface casing pressure. No well identification has yet to be posted at the tank battery or well head. All documentation has been prepared and forwarded over to the Divisions Legal staff for the drafting of a Chiefs order.
1437403290 2015-07-20 Production Wells No Violations Status check inspection follow up to Order # 2015-344. Upon inspection of the well head the following equipment was found; A 7 surface casing string along with a 2.875 tubing string set on a packer with a 1.5 production tubing string with production flowlines hooked to the 1.5 and 2.875 and the production valves found in the open position with no check valves in place to avoid gas from back flowing into the well bore. Gas pressure is present on the 7 surface casing at the well head. Not visible at surface but present is the un cemented 5 string set in the well bore that was not on the drilling permit and has not been addressed. No sub gas meter reflecting the production output of the said well is present at the well or in the area. There is however a Roots 8-C meter present at the commercial purchase point for Gatherco reflecting the total gas sold into the system for all the combined wells owned by the producer in the area, the meter currently reads 544 on the rotary index meter. Addressing the 100 barrel production tank noted in the previous violation, It was observed that the producer has met the set back requirements for the existing well 34111263157-000 and the existing stream by relocating the said tank, but has yet to place proper well identification on the well or the tank, also noted the tank is located on an adjoining lease. Also noted I have not received any contact from the producer in regards to the violation noted as violation # 1428512346 issued on 04/08/2015. The producer also noted as part of his submitted restoration plan all vegetation was to be removed off site and to this day the trees and debris remain present and the landowner has requested the material be removed.
1438887927 2015-08-06 Production Wells No Violations Status check follow up to Order # 2015-344. Upon inspection of the well it was observed that the 5 un cemented casing string has not been addressed. At last record annular pressure was present at surface on the 5 casing string. The 5 string was set to stabilize the well bore due to the hole collapsing, with the ultimate outcome of not having the proper annular clearance as listed under 1501:9-1-08 well construction (H) Wellbore diameters (1) The diameter of each section of the wellbore in which casing will be cemented shall be at least one inch greater than the outside diameter of casing collar to be installed. Unless otherwise approved by the Chief. Also noted at the well head was the absence of a check valve on the 2.875 tubing set as the production string, the producer does have on the gas sales outlet a spring type orifice 046 field regulator with a 08 to 20 psi rated working discharge pressure. With no sub meter found present to document the well production, there is a Roots 8-C rotary type meter present at the Gatherco commercial purchase point for all the wells present in the area owned by the producer with a rotary index reading of 0558 present in plain sight. Addressing the tank battery- A single 100 barrel production tank is present located in an established dike area, the tank was repositioned on the property and in doing so was relocated within 96 feet of an existing dwelling. Under 1501:9-9-05 Production Operations. (2) Oil production tanks shall be set a minimum of (50) feet from the traveled portion of a public roadway and a minimum of one hundred (100) feet from an existing dwelling. Addressing the overdue Preliminary Restoration- The owner noted in his permit application well restoration section that any vegetation would be cut into firewood, to this day the trees created are piled on site and the landowner has requested the debris removed off the property.
1450801134 2015-12-22 Plug / Plug Back No Violations Status check inspection found Harmons well service rig on location and has pulled the 2.875 tubing and packer out of the hole and ran 1,303 of 1.5 tubing back in the hole. A total depth was confirmed by wireline of 1,330. A intermediate casing string was visible at surface inside the well head, the casing string diameter was confirmed at 5.1875.
1451335955 2015-12-28 Plug / Plug Back No Violations On site inspection to witness the well plugging process with the following observed- Harmons well service rig is on the hole and has pulled the 2.875 tubing liner and packer from the well along with the 1.5 tubing and pumping rods. The 1.5 tubing was ran back in to a depth of 1,325 and a 400 bottom plug was spotted through the tubing by Formation Cementing Service. No earthen pit has been constructed and the well fluid is being circulated back into a 1/2 round steel tank. The bottom plug has been set on this day and the 1.5 tubing will be pulled and the 5.1875 liner shall be pulled before setting the plug to cover the bottom of the surface casing. There is a standing Chiefs Order # 2015-344 against the said well.
1452017673 2016-01-05 Plug / Plug Back No Violations 01/05/2016 On site inspection to witness the final stage of the plugging process found the 5.187 casing was ripped at 580 and pulled out of the hole. Proceeding a 1.5 tubing string was ran to 580 and sat down indicating the well bore had collapsed from behind the 5.187 when the unapproved casing string was pulled. The well was plugged solid from 580 to surface in 2 intervals. The initial bottom hole plug set was tagged at 920 with the rig tools prior to pulling the 5.187 casing string. The casing was cutoff 30 below grade and a permanent marker installed on this day. Compliance notice # 1428512346 was considered resolved on this day however Chiefs Order # 2015-344 will need resolved pending a consent agreement.

1540304931 2015-09-22 Preliminary Restoration No Violations Preliminary Restoration follow up and Compliance notice # 1428512346 follow up inspection. On this day it was observed that the 100 barrel production tank has been moved back and has met the 100 set back requirements from a residence. Also noted was the vegetation created during the well pad construction has been removed from the area. With both the items being completed as addressed in the violation now allow the restoration to be completed. The Preliminary Restoration was passed on this day however the additional well construction items noted in the violation still need addressed, these items are also noted is a Chiefs order # 2015 # 344.
1560003327 2013-07-02 Drill / Deepen / Reopen No Violations I received phone notification from Ed Crum at 6:37 Pm that they tried to lift the 9.625 casing string out with the rig and a bull dozer and it would not budge. Ed stated that they were bringing in casing jacks tomorrow 07/03/13 and He would call me when they were going to lift on the casing, I stated I would be on site and He was to notify me when they began to try to remove the casing.
1576419547 2013-10-22 Drill / Deepen / Reopen No Violations I received a phone call today from Ed Crum who indicated that he had spoken with Steve Opritza with the Divisions Permitting section. Mr. Crum indicated He would be changing the intended producing zone to the 1st Cal run and possibly the 2nd Cal run, He indicated that the bore hole was collapsing badly at around 880 and He would like to run a liner and producer shallower. Mr. Crums phone call was prompted by a discussion I had with Steve Opritza in regards to the produce not completing the well bore drilling.

1676477514 2013-07-18 Casing String Witnessed No Violations On this day I witnessed the removal of the unapproved 9.625 casing string and cementing of the 7 surface casing. The cement mix water was tested with a low level chloride strip and show no unacceptable chloride levels the PH was 7.8. Circulation was established prior to cementing with 600 lbs of Bentonite Gel and 25# of LCM ran ahead of the cement, 50% cement excess was ran with 145 sks of class A cement total used, there was good circulation and returns of gel and lcm to surface but no cement, with the fluid level staying up on the annular side after stopping displacement. Permission was received to grout the backside from surface from my Supervisor Rocky King.

1788638372 2013-12-13 Drill / Deepen / Reopen No Violations Compliance agreement # 1422107877 follow up inspection with inspector Pat Shreve found the drilling pit fluid level still needed to be lowered and a 1 sales line hooked to the surface casing needs removed. The production valve was closed and no pressure was found on the surface casing at this time, I shall speak with my supervisor in regards to the well owner possible trying to sell gas off the production casing on the uncompleted well bore. Contact with the well owner will be made after a opinion is received from my supervisor.

The Division is discussing alternative remediation work. I will note that there has multiple conflicting reports from the well owner and drilling crew reported to the division in regards to work done and casing points.
1941190006 2013-09-24 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress and condition of the location found no company men on site. I called Ed Crum for a update and He said they had quit drilling due to hole conditions with caving formations, He questioned if He could produce at His current depth of between 600 & 700, I informed Ed that He was permitted for the Keener and the pressure gauge He has on the surface casing read zero when I looked at it. I also informed Ed that He needed to quit letting the drilling pit overflow and if He was done drilling it needed to be emptied.
1966483147 2012-12-17 Drill / Deepen / Reopen No Violations A status check on the drill sit has found the drilling rig moved in and the drilling pit excavated and lined. Inspector will note that contact has been made with the well owner Ed Crum in regards to potential problems with the choice of the well location and the potential of water run off infiltrating the drilling pit before the pit was constructed and Mr. Crum continued with the pit construction against the advise of the inspector to use an alternative pit method and location.
