At 5:40 pm Mr. Crum notified me and reported they had solidified the drilling pit with lime and conducted a paint filter pass through saturation test and he had approved the product to be hauled off site. Mr. Crum has been in contact with me on several occasions over the last 48 hours and I have informed him that the cuttings need solidified and buried on site or removed to a EPA approved landfill. |

Also noted was a phone call received at 9:08 am and a conversation that took place between the contract field producer Troy Harmon in regards to the well having a un-cemented 5 casing string along with the tank battery needing moved and the lack of reported production for the last 2 years by CGO Inc. I indicated to Mr. Harmon that the well Owner Ed Crum had not contacted the Division in regards to the violation and unapproved casing string and needed to do so before the 30 days noted in the violation or I would move foreword with a Order. It was stressed to Mr. Harmon that CGO had no individual gas sales meters set at any of his wells to confirm the gas production. |

No well identification has been posted. |

Addressing the 100 barrel production tank set near the existing well 3411163157-000- and nearby waterway found the tank has been set back to meet the minimum requirement of 50 from both mention items. Inspection of the well head found the following equipment present- 7 surface casing with a pressure valve in plane sight showing 90 psi. 2.875 tubing and 1.5 tubing and pumping rods with the production valves in the open position. Also noted and photographed the check valve has been removed from the 2.875 casing that could allow the gas to back feed into the well bore and contribute to the high surface casing pressure. No well identification has yet to be posted at the tank battery or well head. All documentation has been prepared and forwarded over to the Divisions Legal staff for the drafting of a Chiefs order. |

Upon inspection of the well head the following equipment was found; A 7 surface casing string along with a 2.875 tubing string set on a packer with a 1.5 production tubing string with production flowlines hooked to the 1.5 and 2.875 and the production valves found in the open position with no check valves in place to avoid gas from back flowing into the well bore. Gas pressure is present on the 7 surface casing at the well head. Not visible at surface but present is the un cemented 5 string set in the well bore that was not on the drilling permit and has not been addressed. No sub gas meter reflecting the production output of the said well is present at the well or in the area. There is however a Roots 8-C meter present at the commercial purchase point for Gatherco reflecting the total gas sold into the system for all the combined wells owned by the producer in the area, the meter currently reads 544 on the rotary index meter. Addressing the 100 barrel production tank noted in the previous violation, It was observed that the producer has met the set back requirements for the existing well 34111263157-000 and the existing stream by relocating the said tank, but has yet to place proper well identification on the well or the tank, also noted the tank is located on an adjoining lease. Also noted I have not received any contact from the producer in regards to the violation noted as violation # 1428512346 issued on 04/08/2015. The producer also noted as part of his submitted restoration plan all vegetation was to be removed off site and to this day the trees and debris remain present and the landowner has requested the material be removed. |

Also noted at the well head was the absence of a check valve on the 2.875 tubing set as the production string, the producer does have on the gas sales outlet a spring type orifice 046 field regulator with a 08 to 20 psi rated working discharge pressure. With no sub meter found present to document the well production, there is a Roots 8-C rotary type meter present at the Gatherco commercial purchase point for all the wells present in the area owned by the producer with a rotary index reading of 0558 present in plain sight. Addressing the tank battery- A single 100 barrel production tank is present located in an established dike area, the tank was repositioned on the property and in doing so was relocated within 96 feet of an existing dwelling. Under 1501:9-9-05 Production Operations. (2) Oil production tanks shall be set a minimum of (50) feet from the traveled portion of a public roadway and a minimum of one hundred (100) feet from an existing dwelling. Addressing the overdue Preliminary Restoration- The owner noted in his permit application well restoration section that any vegetation would be cut into firewood, to this day the trees created are piled on site and the landowner has requested the debris removed off the property. |
