0679458895 2012-08-20 Production Wells No Violations On the day of my inspection the well was shut in. The pit has not been closed yet. Monroe drilling house was still on site and there was also a frac tank on site. No violations were noted.
1344596905 2012-08-10 Drill / Deepen / Reopen No Violations Inspector on site to witness 4.5 casing being run in and cementing of the production casing. Well was TD @ 2700, and a total of 2651 of new 4.5 production casing was run into the well bore. Due to an equipment failure on one of the cementing trucks the long string cementing procedure was not witnessed.
1346348019 2012-08-30 Drill / Deepen / Reopen No Violations Inspector stopped by frac job for a status check. Universal well services on location and stimulating the 1st stage on this day, I checked the casing annuals and no communication pressure was noticed.
1372092596 2013-06-24 Preliminary Restoration Violations Noted Inspection per request of Carl E Conley found that both drill sites Brown/Conley #1&2 need extensive restoration work due to improper management practices during the initial site excavation. Inadequate methods to save proper amounts of topsoil to reestablish vegetation were not used along with proper erosion and sedimentation control. The Producer Beck Energy must make both locations suitable to establish and hold vegetation along with establishing proper long term erosion control paying attention to; and addressing any areas that are beginning to or already have slipped. I have talked with both Ray Beck and Becky Sims and encourage them to take better management practices when building locations. Initial Violation issued was # 1372092596.
1460647652 2016-04-14 Preliminary Restoration No Violations Preliminary Restoration inspection and complaint # 5300 inspection response. Addressing the complaint- On this day it was observed that a earthen slip had occurred in the general area of but not directly on the transportation sale pipeline at a GPS point of EPE 21 -39.60456 X 80.96778, contact was made with both the landowner and Don Coe with Beck Energy and both parties are working together to resolve the issue and both parties understand the issue is beyond the Divisions jurisdiction. Addressing the restoration of the well site. The well site meets ORC 1509.072 with vegetation and erosion control established. 2-100 barrel production tanks are present in an established dike along with a mechanical separator and gas sales meter with a current sales chart present. Proper well identification is posted and the production area is shared between the 2 wells present on the lease - the Brown Conley # 1 & #2.
1593211212 2012-08-20 Drill / Deepen / Reopen No Violations Inspection of drill site found the drilling pit filled and the well bore completed and waiting on the final completion process. Inspector will note that suggestion was made to Ray Beck in concern to the silt run off into the stream bed that the lease road crosses.
1877525989 2013-11-06 Preliminary Restoration No Violations Compliance agreement # 1372092596 was resolved on this day, top soil has been brought in and erosion control is in place, vegetation is beginning to be established and I will re inspect after ample time has been given to proper monitor vegetation growth and long term erosion control.
