1126295140 2018-04-12 Production Wells No Violations Status check inspection found the well head production valves in the open position with annular pressure monitoring in place. There is activity around the well pad supporting a second pad adjoining the well pad the said well is on. The company has submitted a simultaneous operations plan that indicated they will put a cage over the wells and establish a perimeter around the wells with barricades in place. Both are being followed out. The well produced into a midstream operation on a nearby pad.
1146396302 2014-09-09 Preliminary Restoration No Violations On the day of my inspection accompanied by ODNR staff and Production Supervisor Rodney Boron, I witnessed that the well was equipped with Cameron casing and tubing wellheads along with a Christmas tree valve assembly and a sand trap controlled manually to a 300bbl frac tank in containment. Production equipment included a GPS Unit, a compressor unit, dehydrator unit, and a fuel skid. Tank battery consisted of 8-500bbl tanks and 2 let down towers all within containment. Also on site were 4 combustors and a pig launcher. The site passed preliminary restoration according to ORC 1509.072A.
1232946617 2012-12-10 Drill / Deepen / Reopen No Violations Well was completed on 12/08/12 with the company providing a copy of the cement ticket to the inspector with verbal compliance that the 5.5 long string cement job went good with 15 Barrels of cement returns to surface.
1246821545 2012-11-30 Drill / Deepen / Reopen No Violations Status check on drilling progress found the current drilling depth to be 8,121 and the company currently pulled out of the hole with the drill string for a bit change, estimated total completion depth to be 10,100.
1349402473 2012-10-04 Drill / Deepen / Reopen No Violations Well was spudded on this day, 10-04-2012, with 70 of 20 comductor being set and cemented to surface.
1352890676 2012-11-12 Drill / Deepen / Reopen No Violations Notification was received on this day by phone that Warren Drilling Rig # 12 was rigged up and should be drilling by mid week.
1353947757 2012-11-26 Drill / Deepen / Reopen No Violations Inspection found the current drilling depth to be 5324 using a 8.750 Varrell bit and starting to build their curve. The switch was made to oil base drilling mud @ 4882 with a total proposed depth to be reached of approximately 10,220.
1354636202 2012-12-04 Drill / Deepen / Reopen No Violations Inspection and status check of drill site found that the well bore has been completed with a total depth reached of 10,063. With the drill string currently stuck in the hole @ 4947 in the vertical section, company is currently in the process of retrieving the stuck drill string.
1377041945 2013-08-20 Completion Testing No Violations Status check on the stimulation process to observe the 2nd stage to be stimulated with a solid composite 12,000 psi bridge plug set at 9,679 and 72 perforations staggered out in 5 different bursts of 6 shots in 12 using 3 hollow steel carriers between 9,479 and 9,650, with 2000 gl of 7.5% HCL used and a break pressure of 5892 and average treatment pressure of 6324 psi and rate of 80.6 barrels per minuite and shut in pressure of 4,480 psi. The annular pressure is being monitored electronically.
1410364324 2014-09-09 Production Wells No Violations The southeast meeting was held at the site. The well is a producing well with a large Texas well head. We looked at the process from the well head to the midstream line. Facility is in compliance with 1509 and 1501:9-9-05.
1455801350 2016-02-18 Preliminary Restoration No Violations Preliminary Restoration status check inspection fund the area affected by the initial well pad construction does have vegetation and erosion control established. There is a fresh water impoundment in place adjacent to the drilling pad. A separate production facility is located near the well pad, this facility will service multiple well pads in the direct area owned by the current registered owner (Triad). Documenting the well head - the well head is complete with the tubing production valve in the open position. There is a hydraulic emergency shut down device located in the production tree outlet. The company has annular pressure gauges on all annulis sections of the well head and the company has a authorized representative periodically checking and documenting all annuli pressures. Addressing the production facility- A total of (8) 400 barrel production tanks are located inside a polymer lined metal containment dike area with (2) low pressure vapor recover units also present inside the dike area. A (6) station commercial tank load out facility is located on the production pad along with a naturel gas compressor and gas processing equipment.
1466500552 2012-11-15 Drill / Deepen / Reopen No Violations On the day of my inspection Warren Rig 12 had drilled to 525 with a 17.5 bit and ran 13.375 pipe to 509. Express ran the casing and BJ was cementing. See casing ticket. No violations noted.

The sales line has a hydraulic Emergency Shut Down with no electrically controlled actuation solenoid. The well head is leaking gas from the B-section between the 9.625 & 13.375. On site was Triad Production Foreman Derik Higgins who was informed to install flow lines and pressure relief valves on all well annulis. Mr. Higgins was informed that the gas was not to dispersed into the atmosphere and a permanent recovery system must be installed. The well head assembly gauge showed 900 psi at surface.
