1421423892 2015-01-15 Plug / Plug Back No Violations Administrative inspection to document the plugging activities on this day not witnessed by the inspector due to being required to attend a monthly Division meeting. A -500- 2nd bottom plug set by perforating the 4.5 above TOC on annular side and breaking circulation and displacing a balance plug to cover the Marcellus formation with 6 perforations shot at 6,040. The top of cement was tagged with the wireline on the bottom plug set through tubing on the previous day at 6,040

1423137156 2015-02-05 Plug / Plug Back No Violations 02/05/2015 The 4.5 has been pulled and 1.5 ran into 2,480 with a 400 plug set to cover the Berea, the tubing was then pulled up to 1,636 and a 200 plug set to cover the bottom of the surface casing. A final 200 to surface plug was then set with the cement staying up to surface. Davis Well Service was the rig and Cemenco was the plugging contractor along with JET wireline Service throughout the job, Earl Barns was the company representative on site throughout the cementing process. H&L water truck service was the water hauler with the plugging pit being emptied at the end of each working day. The well casing will be cut off @ 30 and the production equipment removed along with the well site restored when the weather breaks in the spring.
1437523847 2015-07-21 Final Restoration No Violations Final Restoration status check inspection found the well casing has been cut off below grade and the area reclaimed with the production equipment removed and the roadway restored and noticeable vegetation established. The restoration meets ORC 1509.072.
1872212311 2015-01-22 Plug / Plug Back No Violations I am onsite to witness the rip plug and the first stage of the Devonian isolation plug. The company representative Earl Barns informed me that they believed the casing was plugged. Jet Wireline Services ran a chemical casing cutter in the casing to cut the casing off above the obstruction; however no obstruction was found. The decision was then made to attempt to pull a few joints of casing to determine if it was free prior to spotting the rip plug. Four joints of casing were pulled before the casing was free. A 57 sack class A cement plug was placed at 5036 with 14 sacks of gel spacer balanced above the plug. The casing was then pulled up to a depth of 3011. Cemenco then placed a 120 sack plug (400) to cover the bottom of the Upper Devonian formations. Circulation was established and maintained through out all plug emplacement.
