0153102801 2015-09-14 Production Wells No Violations Compliance Notice # 142845519 follow up inspection. On this day I visited the tank battery site with ODNR Inspector Eric Conrad with the following observed (2) additional 100 barrel production tanks have now been added with (4) total tanks now present. The production fluid knock out separator is no longer constructed in such a way as to allow the surface discharge of production fluid. A contract worker by the name of Troy Harmon was contacted on this day and informed to remove the plastic flow line out of the nearby stream coming from the inside of the tank battery. Also discussed was the production sales meter chart with the last date of record from 2014, Mr. Harmon indicated the meter was inoperable and needed repaired.
0365317764 2016-12-29 Production Wells No Violations On the day of my inspection it was observed that the producer has set additional production tanks to handle the production water and ceased surface discharge. There is now a total of (4) 100 barrel tanks present. The prior violation is considered resolved.
1319020149 2015-04-22 Production Wells No Violations Compliance Notice # 142845519 follow up inspection. Upon inspection of the tank battery it was observed that there was standing fluid inside the tank battery area. A field sample was taken and tested for chlorides with Quantab strip tests indicating brine present in the levels of 3,217 parts per million on the high level strip tests and exceeding the reading capability of 611 parts per million on the low level strip tests. Contact was made with the Troy Harmon who is a contract representative of CGO who indicated that no fluid had been drained from the storage tanks. I requested that the 1 line inside the dike be removed in such a way it no longer can communicate with the adjacent stream and the product once again be recovered from the dike. On 04/09/2015 I did inspect the storage facility and witness Harmons well svc on site remove any standing fluid and haul off any brine inside the storage tanks.
1428455519 2015-04-07 Production Wells Violations Noted Status check inspection found the following equipment present at the well head 4.5 production casing, 1.5 production tubing and pumping rods with the production valves open and the well set up to be produced by the means of a pumping unit. At the production fluid storage facility 2 mechanical separators and 3-100 barrel production tanks and a single 600 gallon plastic tank were found present in a dike area shared with 3411121995-0000. On this day it was observed that indications of surface contamination were visible inside the dike area below the 600 gallon plastic brine tank. The area was photographed and the fluid field tested for chlorides using both high and low range Quantab strip tests with the High range tests maximum reading 4,366 parts per million and exceeding the low level Quantab field strip tests at 611 parts per million. A sheen could be seen inside the dike are with hydrocarbon deposits present, the outlet drain line inside the dike area was standing open also. Contact was made to ODNR Regional Supervisor Brent Bear and OEPA Ken Mettler on this day regarding the reoccurring matter. Instructions are as follows - Immediately stop the surface discharge of the well production fluids and remediate affected area . ODNR Monroe County Inspector Bill Stolarik 740-507-6592.
1428615192 2015-04-09 Production Wells No Violations Compliance Notice # 1428455519 follow up inspection. On this day Harmons well service Employees were on site with the company owned vacuum truck recovering the product from within the tank battery. They had a total of 65 bbl of fluid recovered from within the tank battery along with product that had pulled out of the 3-100 barrel production tanks.
1430481053 2015-05-04 Production Wells No Violations Compliance Notice # 1428455519 follow up inspection. On Tuesday 04/28/2015 it was observed that a pumper for Harmons well service was on location draining the production fluid from the 3-100 barrel tanks over into the 600 gallon holding tank. Fluid was observed inside the containment dike pit area. On this day 05/04/2015 it was observed that there was no fluid inside the dike pit area and no fluid inside the 600 gallon tank. I shall review the brine hauling records to see if any product was received at the disposal well listed on Harmons UIC plan. The 1 drain line that runs from the pit inside the dike has yet to be removed.
1431044226 2015-05-07 Administrative Inspection No Violations Compliance Notice # 1428455519 follow up administrative inspection. On this day I received the Green Hunter Disposal billing records for the Month of April 2015 for the product billed to Harmons Well Service. No product has been documented as received by Green Hunter by Harmons for any C G O Inc well for the billing months of March or April 2015.Harmons brine hauling plan only reflects Green Hunter as a disposal site for their company. On the days of 04/09/2015 and 04/22/2015 I did witness Harmons well Service removing product from the Tank battery and from inside the Dike area, this product was field tested and has Chloride levels exceeding 3,217 parts per million on a high level chloride strip test.
1786027645 2015-05-08 Production Wells No Violations Compliance Notice # 142845519 follow up inspection. The initial timeframe set for the producer to comply was 04/17/2015 with the following observed at the production fluid facility on the day of my inspection. An addition 100 barrel storage tank has been set inside the tank battery and is being used to recover fluid from the gravity knock out system. A brine scald was observed in the pit area inside the dike and The 1 drain line inside the pit that runs to the stream has yet to be reconstructed in such a way as not to allow product to be discharged into the waters of the State. No contact has been received fro the producer regarding the violation.
