1077041700 2017-02-13 Production Wells No Violations Compliance Notice number 1287353237 follow up inspection found the well equipped with 4.5 diameter production casing and 1.5 diameter production tubing with both casing and tubing well heads installed and pumping rods installed in the 1.5 tubing with no pumping unit on the well. Attached to the 4.5 diameter casing well head is a 2 flowline and valve with the valve in the open position, further down line from the valve the sales line is reduced to 1 diameter and flows into a mechanical drip then into a domestic sales meter. The well appears to be capable of providing domestic gas and the previous violation is considered resolved on this day.
1287353237 2014-10-02 Production Wells Violations Noted Status check inspection per request of landowner Don Pollock found the well to be plotted off coordinates in the database. Inspection of the well head found 4.5 production casing 1.5 tubing and pumping rods with both production valves in the closed position and no pumping unit on the well head, no production tank or metering equipment was found present related to the said well and no well identification was found present. Instructions are as follows-Plug the said well within 180 days. ODNR Monroe County Inspector Bill Stolarik 740-507-6592.
1425392031 2015-03-03 Production Wells No Violations Compliance notice # 1287353237 issued on 10-03-2014 follow up inspection. Upon inspection of the well head the well was found to be commercially idle and shut in with rods and tubing and no pumping unit present. A 1 flowlines was found present with the valve in the open position. No commercial sales meter related to the said well can be found or identified. On this day I spoke with Bill Arnholt with Northwood Energy who indicated they had not been able to locate the well location and no meter or tank battery is present related to the well. I described where the well was located and explained that the well had been plotted wrong on the mapping system. Mr. Arnholt indicated that now that they understand how to locate the well they will send a contract pumper out to evaluate the well and reply back to me with their findings. The violation due date is 04/03/2015.
1456191776 2016-02-23 Production Wells No Violations Compliance Follow up inspection in response to complaint #5258 filed by the landowner a Ernest Abbott who indicated there was crude oil from the said well in His pond. Upon inspection of the area indicated by the landowner at a GPS point of 39.85646 x 81.12150 it was observed that gas was migrating up from below the earths surface. The fluid present was titrate tested for chlorides with 157 parts per million chloride levels found and PH tested with a PH level of 7 found. No oil product was observed. Notification to Northwood Energys field representative a Mike Corp on 02/22/2016 . Mike Corp and Ron Corp responded on this day and found the 2 sales servicing the well was leaking gas, the issue was isolated a shut off point 2 riser in the field near the gas leak, the gas leak issue was resolved at 12:09 pm on this day 12/23/2016 with my investigation finding that a 2 check valve in the gas shut off riser had failed and was allowing gas to back feed into the production flow line for the said well, explaining how gas was leaking with the production valve in the closed position. The Division Geologist Jake Glascock was contacted and slated to respond the next working day and sample the pond water. The landowner Earnest Abbott was on site and informed that well in question was inspected and the commercial production valves found to be in the closed position and no production facility was present associated with the well. The landowner was also informed that the Division Geologist would follow up with a environmental assessment of the area. Corey McCudden with Northwood Energy was contacted to determine the status of the well and unresolved compliance notice# 1287353237 and upon discussion Mr. McCudden indicated that the landowner that the well resides on was not cooperating and would not grant them permission to enter the property. We did however discuss the fact that there was a domestic hook up present and the well appeared to be providing domestic house gas to a near
