Mr. Fleeman informed me that they had indeed drilled to 1,995 and while pulling out of the hole with the drill string became stuck at 1,699. A copy of 1501:9-1-08 well construction rules was provided to Mr.Fleeman and the proper cementing or zonal isolation using a packer completion was explained on this day. |

Upon arrival, Monroe Drilling had 1700 of drill string in the hole and were hung in the hole. Jet Wireline Service Inc. arrived on location at 10:00 am and began rigging up to log the hole through the drill string. The bottom of the drill string was tagged at 1699 and the top of the floats/drill collar was found at 1668. A shot was run into the drill pipe to 1698 and detonated to allow fluid pressure off of the drill string from yesterday when Formation Cementing attempted to pump on it. Water flowed to surface for an hour and finally stopped. Jet Wireline then ran a ball of primer cord into the drill string to 1668 and detonated it while Monroe Drilling was pulling back-torque on the string with their rig. This allowed the drill string to spin out of the threads of the floats/drill collar. The drill string was coming out of the hole slowly when I left the location at 1:35 pm. |

The company had initially applied for a permit to deepen the well and the permit has now expired. During an attempt to deepen the well a drill collar and drill bit was previously lost in the hole. Upon my inspection a sputter drilling rig was observed rigged up on the hole with a makeshift temporary working platform erected around the well head at ground level. The casing visible is 8 diameter indicated in the well completion records to be set as the surface string to a depth of 674. Installed on the 8 surface casing is a reducing swage down to 4.5, installed on the reducing swage is a 4.5 collar and tubing head with a 1.5 diameter tubing string inserted into the well to a unknown depth. The 1.5 tubing is standing open and the outlet ports on the tubing head have 2 diameter x 8 length steel nipples screwed into the well head with no valves or well control present, no gas pressure was indicated as present. No production casing is present in the well bore. |

We discussed the casing strings currently present and Mr.Fleeman indicated only 8 was set in the wellbore. Mr. Fleeman also indicated that prior to attempting to deepen the well the well would make approximately 4 barrels of water a day. He also indicated the well was standing full of water and they were going to attempt to swab the well down through the 1.5 tubing. I informed Mr. Fleeman that he needed to get the well bore opened back up to the point where the drill bit and collar was lost below 1,600 and either plug the well or run 4.5 and cement the well accordingly. |

During my inspection, I found a small service rig rigged up next to the wellhead. Vegetation was grown around the rig. The wellhead was equipped with 10 inch diameter conductor casing, 8 inch diameter surface casing reduced down to 4 inch diameter casing. The valve is in the closed position with the top of the wellhead open without a cap. There is standing water inside the wellhead. There is also 1 inch diameter tubing laying on the ground near the well, along with a horizontal steel tank. Pictures were taken of the site, rig, and wellhead on this day. While in my truck, I was approached by well owner Theresa Fleeman. She claimed to be committed to this well and is awaiting Kerry Brown to conduct the work. She mentioned that no work had been done to the well through the summer months. She was informed that work needed to commence or it needed to be plugged. |

No production flowlines are attached to the well head and no production tubing is present inside the well bore. A service rig owned by the well owner is on location and rigged up on the hole, however no sign of recent activity is present around the well and natural vegetation has began to reestablish itself. Historically a drilling permit was issued on 12/03/2014 to deepen the well bore and on or around 01/21/2015 a drilling rig owned by Monroe Drilling was brought in and the well attempted to be deepened unsuccessfully with the drilling rig bottom hole assembly lost in the well bore at a drilling depth of 1,699. Several attempts have been made to clean out the well bore unsuccessfully and the well has not been observed to be in production since 01/21/2015. The drilling permit expired on 12/02/2016. Multiple documented attempts have been made to encourage the well owner to address the issue regarding the idle well. Instructions are as follows. The well owner/operator shall submit a written plugging schedule to Inspector Bill Stolarik at 5880 Memory Road Zanesville ,Ohio 43701 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |

James Fleeman indicated that he had spoken with Steve Opritza, with ODNR permitting, about the said well, and submitted a plan of action. It was discussed that Mr. Fleeman would have Monroe Drilling use their rotary rig to complete the well. Proper well control will be used to drill through the Pencil Cave Formation, which has collapsed at a depth of approximately 800 feet, and ensure that the hole is stable. If stable, they will clean the hole to a depth of approximately 1,650 feet, at the Weir Formation. A formation packer will be set at approximately 1,450 feet, and 2.875 upset tubing will run from the packer to surface, and be cemented to surface. If stability is not achieved, it is requested to notify ODNR, and apply to plug the well. I arrived onsite and found the old service rig that was on the well had been removed, but remains on location. Monroe Drilling has (2) water trucks, (1) D6 bulldozer, (1) 300 barrel frac tank, (1) flatbed trailer with drill-string, (1) Denver mud pump, and an Ingersoll-Rand rotary drilling rig on the hole. Additionally, an earthen pit has been dug next to the rig and lined with plastic. It will be noted that drilling will take place in close proximity to Witten Run Creek. Kerry Brown stated that they started into the hole with a 6.25 inch diameter drill-bit on 5/30/2018 and stopped at a depth of 690 feet. An inspector was not notified when drilling commenced. Today (5/31), a blow-out preventer is being installed before they begin drilling. Departed location at 12:00 pm. Arrive back to location at 2:40 pm. BOP is installed, mud pump was just started, getting water trucks in place, and preparing to start in the hole. Received another call from Kerry Brown later into the evening, and he stated that they encountered hard drilling at a depth of 780 feet. The rig owner Kerry Brown was on location and did report the well does have 8.625 casing set to a depth of 674. The rig is a Ingersoll Rand Rd 20 and the contractor also has a mud pump set up and a fluid pit established. The contractor is using a salt water based drilling mud mixture of Flori Gel Hy and brine water to condition the hole. Historically a 6.625 drill bit and drill collar was lost in the well bore below 1,600. The rig is using a 6.625 bit and is washing their way back down to where the bit was previously lost in the hole. The well bore was also collapsed between 725 and 813. The rig removed the obstruction between 725 and 813 with a show of gas below the bridge and again at 1,470 with oil present also. The rig washed down to 1,660 and was going to pull the drill string then run 2.875 tubing on a formation packer and cement the tubing in place later in the evening. |
