0332169200 2014-03-06 Production Wells Violations Noted Status check inspection of the well head found the following equipment present-4.5 production casing-2 tubing and rods with the production valves open and the well being produced by the means of a pumping unit with a 1 plastic flow line coming from the production tubing that is not attached properly to the production tank. 5-100 barrel production tanks are located in an earthen dike area that is not capable of sufficiently holding 110% capacity of the largest tank in the dike and sits in close proximity to a stream with no well identification present along with no current active gas sales chart present and no reported production in the database. Instructions are as follows- report production to the division- make the tank battery earthen dike capable of holding 110% capacity of the largest vessel in the storage area, secure all production lines & equipment standing open, properly post identification, properly attach the production flow line to the storage tank as to prevent fluid from being discharged onto the ground. Producer has 30 days to report production, post identification and properly attach the production lines, ODNR contact Monroe Co Inspector Bill Stolarik 740-507-6592.

The well is equipped with casing -tubing and rods and set up to be produced by the means of a pumping unit. A single 100 barrel production tank is present associated with the well.
