1418062133 2014-12-08 Production Wells No Violations Status check inspection per request of the Divisions Surety and Bonding section to verify the well Domestic use in order for the landowner to transfer the said well into their name as a Domestic owner. Inspection of the well found the following present at the well head- 4.5 production casing 2.875 tubing hooked into a 2 sales line with the production valve in the open position with the casing standing open and no valves present indicating a packer is set on the 2.875. ( Landowner says well was recompleted and packer is set @ 1,200) At the production fluid storage area a 100 barrel storage tank is present along with a mechanical separator and gas sales meter with no current sales chart present, all valves are open. A 1 gas line was found leading to the residence with a little joe pressure reducing valve and a panckake regulator is present on the 1 line going into residence with the valves in the open position.

1534791076 2018-08-20 Plug / Plug Back No Violations Status check inspection found a service rig owned by Viking well service out of West Virginia has moved in and rigged up to plug the well. The well is equipped with 4.5 diameter production casing and a 2.875 diameter tubing set as a liner with a 1.900 production tubing string. The State RDBMS database indicate 517 of 8.625 surface pipe was set and cemented to surface. The well has 360 pounds per square inch on the 1.900 tubing string and is flowing oil. The service company is going to attempt to blow the well down and load the hole with production brine water before attempting to pull the 1.900 tubing. Blue Ridge Mountain Resources is constructing a shale well pad in the well area. A proposed plugging plan has been received by the contractor.
1535560330 2018-08-29 Plug / Plug Back No Violations On site inspection to document the well being prepped to be plugged. Viking Well Service out of West Virginia is rigged up on the hole. They have pulled a 1.900 tubing string out of the well and a 2.875 liner string set on a tension packer to a depth of 2,370. There was a cast iron bridge plug set in the well bore at 2,701, the plug has been removed by the rig with a bit stem and jars. The well had 800 pounds pressure under the plug and is being flow into a 1/2 round steel tank. Once the bottom hole pressure equalizes the well bore will be loaded with production water before starting back in the hole with a split string of tubing made up of 2.375 on the bottom and 2.875 on the top. This will allow the service company to set their bottom plug to the State requirement of 50 below the base of the lowest reservoir rock penetrated. The bottom perforations are from 4,192 to 4,326. The company will set a 30 sack 400 bottom plug then pull the tubing up to the upper perforations shot in the Weir formation between 2,546 and 2,566. The 4.5 casing cement design initially was 485 sacks and the borehole was 7.875 diameter. The top of cement behind the 4.5 should be around 1,600. The company man on site Merle Richards has indicated they will set the bottom plug and tag it with the rigs wireline then set the plug over the Weir. Then the next working day tag the Weir plug with the wireline service while finding the top of cement and hole in the 4.5. The 4.5 may also have fill up from the red rock. The company man Merle Richards has also been advised that once the bottom plugs are set in the 4.5 that recovery of the 4.5 as close to the requirement of 100 below the bottom of the 8.625 surface casing set at 517 would be acceptable at a minimum of 617. The company man understands that the plug at the bottom of surface must be 100 in and out of the 8.625 surface. This well is being plugged inside the well pad boundary of the Blue Ridge Mountain Jackson 10 well pad.
1536346530 2018-09-05 Plug / Plug Back No Violations On site to witness Viking Well Service attempt to run tubing in well to set remaining plugs they set bottom hole plug at earlier date and cut casing off at 1990 and removed casing from well and then they decided to run tubing back into well the first attempt they sat down at 946 and they plugged tubing and then they pulled it from well and then ran it in a second time and sat down at 1014 and plugged it again and then decision was made to pull it again and locate a power swivel and mud pump so they could wash tubing back to casing cut off point and set remaining plugs.
1537461261 2018-09-20 Plug / Plug Back No Violations On site inspection to witness the final stages of the well plugging process accompanied by ODNR Inspector Tom Thomas. Previously the bottom isolations plugs were set successfully and the 4.5 diameter production casing was cut off by a wireline explosive cutter at 1,900. Due to issues with holes in the casing the service rig company identifies as Victory Well Service out of West Virginia pulled the 4.5 diameter production casing and the red rock formation collapsed below the surface pipe. After several attempts to wash back down to 1,900 a proposed plan was submitted and reviewed by myself and ODNR Supervisor Mike Moreland. The plan indicated the company would plug the well from the top of the collapsed area to surface. On this day the well was plugged from 813 to surface in 3 intervals using a total of 280 sacks of class A cement. Cement was witnessed to surface and the cement stayed up to surface.
