0146346804 2015-03-23 Production Wells Violations Noted Inspection per request of the landowner as part of complaint # 4969. Upon inspection of the well head the following equipment was found-4.5 production casing-2.375 tubing with the production valves in the open position and the well set up to be produced by the means of a plunger lift system. At the production fluid storage area 2-100 barrel production tanks were found along with mechanical separator and gas sales meter with a current active sales chart. Addressing the complaint by the landowner that the said well has a hole in the 4.5 casing contact was made with Jordan Jones with G&R contracting who indicated the well did have downhole issues that need addressed.

1403045700 2017-10-13 Plug / Plug Back No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. G & R Contracting and Formation Cementing are onsite. A 200 foot rip plug was set at a depth of 1,260 feet. The 4 1/2 inch diameter casing was then pulled up to 980 feet where a 200 foot plug was set. Then, the casing was pulled up to 200 feet to set the surface plug.
1456243700 2016-02-23 Production Wells No Violations Compliance Follow up inspection in response to complaint # 4649. On this day I met with the landowner Frank Forest and explained the well construction and how the fresh water aquifer was protected by the setting of the surface casing. I also spoke with the Divisions Permitting Geologist Steve Opritza who indicated the nearest water well was 1,600 away. Addressing the unresolved violation # 146346804. On this day it was observed that the well is set up to be produced by the means of a plunger lift system with the tubing valve in the open position and the gas sales meter showing active production, the meter is however a conjoined meter between multiple wells in the area owned by the same producer identified as G-R Contracting. On this day I also notified a Jordan Jones with G-R Contracting and advised him of the unresolved violation. Jordan indicated that they would address the issue by either plugging the well or setting an isolation packer.

1474979170 2016-09-27 Production Wells No Violations Follow up inspection to violation #146346804 and complaint # 4649 found the well equipped with 4.5 production casing and 2.375 tubing with the well set up to be produced by the means of a plunger lift system and the master control box being found to be out of service, the production valves are open and the 2 sales line valve is closed down stream from the well head. Historically the well has a hole in the 4.5 production casing located below the surface pipe at 1,700. On this day I contacted Jordan Jones with G_R Contracting and informed him of the unresolved violation and to apply for a plugging permit with the Division.
1507827056 2017-10-12 Plug / Plug Back No Violations On site inspection accompanied by ODNR Inspector Tom Thomas. Upon arrival I discovered that the company owned service rig had set the 3.275 upset 8 round tubing to a total depth 2,307 and the well completion records indicate the bottom perforating is at 2,429 so I required the company to run in 4 more tubing joints to a total depth of 2,470. The extra 4 tubing joints were not on location and had to be brought in. Once the tubing was in the hole Formation Cementing began to circulate with fresh water and the well began to unload so I then required the company to stop circulation with fresh water and bring in brine water to so the well would be properly killed and remain static. The company owned water truck brought in brine and the well was circulated until static with approximately 25 barrels of oil being evacuated out of the well before the well was static and cement was pumped down the well bore. Formation than pumped a 6.30 barrels absolute volume bottom plug consisting of 30 sacks of 1.18 yield 15.6 pound per gallon cement with gel spacer ran ahead and behind the cement. The rig then pulled the tubing and the wireline will tag the top of cement on the next working day before shooting the casing off. There is also a unresolved violation #146346804 and complaint # 4649 against the well that will be considered resolved upon the completion of the plugging process. The previous complaint and violation are considered resolved on this day.
1507828513 2017-10-12 Plug / Plug Back No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. G & R Contracting are onsite with a service rig and Formation Cementing is onsite pumping the bottom plug. The tubing was set at a depth of 2,470 feet. Well circulated approximately 25 barrels of oil into a pit and all oil was removed from location on this day. Brine water was used to stabilize the well. 30 sacks of cement, with a yield of 1.18 and 15.6 pounds per gallon were pumped for the bottom plug.
1507914248 2017-10-13 Plug / Plug Back No Violations On site at 8.00 am to witness the last stages of the plug job with Formation Cementing to set a 200 rip plug through he 4.5 diameter casing set at 1,260. The plug was an absolute volume of 13.4 barrels mix and consisted of 60 sacks of class A cement. And 1% gel cement additives, the cement was displaced with 16.8 barrels of fresh water and equalized properly upon reaching total displacement. Of 16.8 barrels. The 4.5 diameter casing was then pulled up to 980 and a 200 plug set to cover the bottom of the surface that was also 13 barrels absolute volume and consisted of 60 sacks of cement, the plug was displaced with 13 barrels of fresh water. The rig then pulled the 4.5 diameter casing up to 200 for the setting of the surface plug with cement returns to surface and the remainder of the casing pulled and the well bore topped of with cement to 30 below grade level.
1529330599 2018-06-18 Final Restoration No Violations On site inspection found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The plugging pit has been closed. The production facility is shared with other wells on the lease still active.

2055774811 2016-06-15 Administrative Inspection No Violations Follow up administrative inspection to violation #146346804 and complaint # 4649. On this day I stopped by G-R Contractings Lewisville office and spoke with Dick Sulzberger in regards to the unresolved violation. Mr. Sulzberger indicated they planned on plugging the well. I informed Him they needed to apply for a plugging permit and address the violation.
