0074486603 2015-02-05 Production Wells No Violations Compliance Notice # 1414443447 follow up inspection found at the tank battery a 3rd 100 barrel production tank has been added to store the production water. In discussion with Troy Harmon on this day he indicated the fluid knock out along with the 600 gallon storage tank and discharge line has been taken out of service and no fluid was being discharged from the tank battery. I requested the discharge lines be disconnected and removed as not to be allowed to be put back into service by the turning of a valve. The violation will be resolved when the removal of the brine discharge system is complete.
0191259957 2016-12-29 Production Wells No Violations On the day of my inspection it was observed that the producer has now set additional production tanks to handle the production fluid produced and has stopped the surface discharge of any production fluid. The existing violation is considered resolved.
0325539823 2014-10-28 Production Wells No Violations Compliance Notice # 1414443447 follow up inspection report. On 10/28/14 I reviewed the stream bed below the tank battery belonging to the well and was able to determine an area of origin that water from the tank battery is being drained into the stream from a hidden 2 plastic line. Chloride levels were taken and found over 3,300 parts per million below the determined area and no chlorides shown above. A meeting has been requested to meet with the producer Ed Crum on the following day.
0756338940 2014-10-29 Administrative Inspection No Violations Compliance Notice follow up administrative inspection to document a meeting and conversation with the said producer Ed Crum. On this day I met with Ed Crum and discussed the draining of the brine water from the said well into the stream bed below the tank battery. I explained to Mr. Crum I had determined a point that the fluid was entering the stream with chloride levels above 3,300 parts per million. Mr. Crum acknowledged this action and agreed to stop the dispersing of well fluid into the stream. I explained to Mr. Crum this is a commercially produced well and does not meet the Mississippian Requirements and dispersing well fluids into the waters of the State was illegal. This stream feeds into the Monroe County Reservoir.
0945752486 2015-09-14 Production Wells No Violations Compliance Notice # 1428412739 follow up inspection. On this day I visited the tank battery with Inspector Eric Conrad with the following observed (2) additional 100 barrel capacity production tanks have been set and the fluid knock out system is no longer constructed in such a way as to allow product to be discharged onto the earths surface. The plastic line running down to the stream still needs to be removed from the stream. Troy Harmon was contacted on this day and informed to remove the line from the stream. Also discussed was the outdated gas sales chart from 2014 present in the sales meter.
1414006723 2014-10-22 Production Wells No Violations On the day of my inspection the following equipment was observed at the well head 4.5production casing-1.5 tubing and pumping rods with the well set up to be produced by a plunger lift system. At the production storage area 2-100 barrel storage tanks are present along with a fluid separator system placed on top of the 100 barrel production tank with a 600 gallon plastic fluid storage tank placed on the surface inside the tank with a 2 discharge line leading into the earth surface inside the dike.
1414443447 2014-10-27 Production Wells Violations Noted Status check inspection with ODNR Region UIC Inspector Dave Ball. Upon review of the tank battery it was observed that a fluid separator was constructed upon the top of the 100 barrel production storage tank with a 2 pvc drain line running down into a brine storage tank inside the dike area. The brine storage tank has outlet lines constructed in such a way to allow the brine to be disposed of into the ground. A line was also found leaking into a fluid pond of 10 diameter inside the dike area. Field tests of the fluid were administered using Quantab strip tests finding chloride levels exceeded the low level reading capability of 613 parts per million, high level chloride strip tests were taken with reading of 3733 parts per million. Samples # SA 16 WJS were taken on this day for lab submittal and photographs were also taken. The storage tanks are shared between 2 wells with 3411123514 being the other well producing into this storage facility with fluid being observed running into the tanks on this day. 1416523996 2014-11-20 Production Wells No Violations Compliance Notice # 1414443447 follow up inspection pertaining to the production fluid water knock out drain system. On 10/27/14 a violation was issued addressing the water knock out system constructed in a way to allow production fluid to be dispersed into a stream bed. On 10/29/14 I met with well owner Ed Crum and informed Him that the system could not be used in such a way as to disperse fluids directly into the waters of the State. On this day it was observed that no changes have been made inside the production fluid facility to properly eliminate this problem.
1426017642 2015-03-10 Production Wells No Violations Compliance Notice # 1414443447 follow up inspection found at the tank battery a 3rd 100 barrel production tank has been added to store the production water. The water tank with the connection leading into the ground that fed into the stream has been removed. Mr. Crum has been notified to properly report his production. Violation resolved on this day.
1428412739 2015-04-07 Production Wells Violations Noted Status check inspection and follow up on the recent activity pertaining to compliance notice # 1414443447. The initial violation was resolved, however on this day it was observed that the 600 gallon plastic tank has been reconnected and was full to capacity and that there was recent pollution and contamination signs inside the dike area that was not present on my last inspection. The area was photographed and the fluid field tested for chlorides using both high and low range Quantab strip tests with the High range tests maximum reading 4,366 parts per million and exceeding the low level Quantab field strip tests at 611 parts per million. A sheen could be seen inside the dike are with hydrocarbon deposits present, the drain line inside the dike area was standing open also. Contact was made to ODNR Regional Supervisor Brent Bear and OEPA Ken Mettler on this day regarding the reoccurring matter. Instructions are as follows-Immediately stop the surface discharge of the well production fluids and remediate the affected area. ODNR Monroe County Inspector Bill Stolarik 740-507-6592.
1429707972 2015-04-22 Production Wells No Violations Compliance Notice # 1428412739 follow up inspection. Upon inspection of the tank battery it was observed that there was standing fluid inside the tank battery area. A field sample was taken and tested for chlorides with Quantab strip tests indicating brine present in the levels of 3,217 parts per million on the high level strip tests and exceeding the reading capability of 611 parts per million on the low level strip tests. Contact was made with the Troy Harmon who is a contract representative of CGO who indicated that no fluid had been drained from the storage tanks. I requested that the 1 line inside the dike be removed in such a way it no longer can communicate with the adjacent stream and the product once again be recovered from the dike. On 04/09/2015 I did inspect the storage facility and witness Harmons well svc on site remove any standing fluid and haul off any brine inside the storage tanks.
1797532152 2015-05-08 Production Wells No Violations Compliance Notice # 1429707972 follow up inspection. The initial timeframe set for the producer to comply was 04/17/2015 with the following observed at the production fluid facility on the day of my inspection. An addition 100 barrel storage tank has been set inside the tank battery and is being used to recover fluid from the gravity knock out system. A brine scald was observed in the pit area inside the dike and The 1 drain line inside the pit that runs to the stream has yet to be reconstructed in such a way as not to allow product to be discharged into the waters of the State. No contact has been received fro the producer regarding the violation.

On this day Harmons well service Employees were on site with the company owner vacuum truck recovering the product from within the tank battery. They had a total of 65 bbl of fluid recovered from within the tank battery along with product that had pulled out of the 3-100 barrel production tanks.
