0071006217 2014-03-11 Production Wells No Violations Status check inspection as part of complaint # 4625 follow up. On this day, my inspection found the following equipment present at the well head-4.5 production casing-1.5 tubing with the production valves open and the well set up to be produced by the means of a plunger lift system. A single 100 barrel production tank along with a mechanical separator and gas sales meter is located in an established dike with proper well identification posted, a domestic hook up is present. I spoke with Troy Harmon on this day and he indicated the separator had froze off during the extreme cold on January 23rd causing the oil to enter the domestic house gas line. No violations noted on this day.
0521040460 2012-03-21 Production Wells No Violations On the day of my inspection well was complete with a plunger lift system. The tank battery contents included one 100 barrel oil storage tank and a mechanical separator. No violations noted.
0675065016 2018-09-05 Production Wells No Violations On this day, I found the well equipped with 4.5 inch diameter casing and 1.9 inch diameter tubing with the production valves open. The well is setup to be produced by means of a plunger lift system. The production facility consists of (1) 100 barrel steel tank, (2) drip tanks, a vertical separator, and a domestic meter all contained within an earthen dike. The production records are current and the tank is properly identified. It will be noted that there is a Dominion Energy sales line tie-in in close proximity to the production facility. Multiple valves are present at the tie-in, making it unclear if gas is being sold.
0875457447 2015-09-14 Production Wells No Violations On this day I visited the well site with ODNR Inspector Eric Conrad with the following noted at present at the well head-4.5 production casing -1.5 production tubing with the production valves open and the well set up to be produced by the means of a plunger lift system. At the production fluid storage and metering area a single 100 barrel production tank is present along with a mechanical separator and gas sales meter with a current sales chart present. I was contacted by Troy Harmon who is a service contractor retained by the well owner Ed Crum in regards to my inspection. Mr. Harmon was notified that the brass ball valve on the separator was leaking and needed replaced and when questioned indicated there was (2) Block wells being metered through the sales meter. I questioned the presence of the check valve flap that has been removed from the 2 sales line on the downstream side of the sales meter. I also indicated to Mr. Harmon that all buried flowlines needed to be properly marked on surface and on record.
1230602412 2013-04-16 Production Wells No Violations Follow up inspection on Oil spill from separator valve malfunction on 04/09/13 found the oil spill area cleaned up and the ditch line containment dike still in place. The spill rags in place have no discoloration from oil.

1365544061 2013-04-09 Administrative Inspection No Violations Administrative inspection entry to document notification of a oil spill from the malfunction of a separator valve. At 8:38 AM Troy Harmon called and said he had found where the separator had a valve fail blowing oil outside the containment dike. Troy indicated He would begin clean up immediately and protect a ditch line coming from a road culvert with spill rags and boomers. The EPA spoke with Troy and Myself through out the day and agreed with the clean up and containment actions taken. I was in a meeting and will follow up on the next day. The landowner also called. And I spoke with Ken Mettler with the EPA.
1365598063 2013-04-10 Production Wells No Violations Inspection and follow up on oil spill. Inspection found the cause of the oil spill to be a 2 mechanical valve that separated at the bottom of the separator causing the separator to discharge its contents under pressure outside the containment dike. The oil had been cleaned up upon inspection and a small ditch had a secondary containment dike constructed to protect any residue that may enter it. Containment will remain for a period of time per request of the Ohio EPA. Proper SPCC is in place around the 100 barrel tank and separator with proper well identification in place. Equipment present at the well head is 4.5 production casing and 1.5 tubing with the production valves open and the well being produced by the means of a pumping unit. The valve that failed was pressure rated above the pressure rating of the pop off valve on the separator, no producer related violations found to be the cause of the spill.
1400620533 2014-05-20 Production Wells No Violations Status check & complaint # 4625 follow up inspection found the mechanical separator has not been repaired and still has a bungee strap holding the pressure lift mechanism open. There is no reported production in the database for 2011 and the gas sales meter has a current 31 day chart that shows no sales activity.
1541513666 2018-11-06 Production Wells No Violations On this day, I am conducting an inspection per request of landowner, Mr. Francis Block. I spoke with Mr. Block onsite, during the inspection. I found the well equipped with 4.5 inch diameter casing and 1.9 inch diameter tubing with the production valves open. A pressure gauge is attached to the 4.5 inch casing, reading 200 psi. The well is setup to be produced by means of a plunger lift system with a timer. The production facility consists of (1) 100 barrel steel tank, (2) drip tanks, a vertical separator, and a domestic meter all contained within an earthen dike. It will be noted that the facility is shared with permit number 2-2620. The production records are current and the tank is properly identified. Updated pictures were taken.
