The well is equipped with an 8.625 inch diameter casing head, 4.5 inch diameter production casing, and 2.375 inch diameter upset tubing with the production valves closed. No production lines are connected to the wellhead. It will be noted that the top of the wellhead appears to have been removed from above the butterfly valve. The production facility consists of (1) 100 barrel steel tank, drip tank, vertical separator, a sales meter with no current chart present, and a domestic meter all contained within an earthen dike. There is no legible identification and the production records are current, with 0s reported for 2017. Multiple pictures were taken. |

On the day of my inspection I visited the well site being accompanied by ODNR Southeast Regional Supervisor Brent Bear with the following observed upon inspection of the well head, The well was found to be equipped with 4.5 production casing and 2.237 production tubing with the production valves in the closed position and no production lines hooked to the well head. At the production facility a single 100 barrel capacity storage tank is present with no legible well identification posted, a gas sales meter was present with no sales meter chart noted as present, there is a mechanical separator also present. Also noted was the dense vegetation overtaking the production facility. The company (Northwood Energy) has been notified by both written and verbal means of the idle well and has not responded to my requests to repair the well. Also noted in prior contact with Bill Arnholt with Northwood he indicated there was lost tools in the well bore restricting the wells ability to produce from the pay zones. |

Upon inspection of the well head the well was observed to be equipped with 4.5 diameter production casing and 2.875 diameter production tubing with the tubing set in the 4.5 casing head and a butterfly valve on the tubing that is leaking gas through the valve with no sales line attached the casing head or tubing valve. On location near the well head is the production facility consisting of a steel constructed 100 barrel production fluid tank along with a mechanical separator and gas sales meter with no current sales chart posted in the sales meter. There is a gas leak inside the dike area possibly coming from a transmission sales line supporting all the wells in the area owned by the said producer. Mike Corp who is the contract pumper for Northwood over this area was contacted on this day regarding the idle well and gas leak in the dike area. Northwood has not responded to the violation in regards to the swabbing tool lost in the 2.875 string. |
