Power swivel and mud pump rented from Knox Energy arrive and are unloaded and placed as needed. Four hours dozer should be included in Mobilization or Site Preparation.
0390747591 2015-11-20 Plug / Plug Back No Violations 7:30 AM: Rig crew on site. They try to put the first (#35) of 3 joints they removed yesterday in the hole and it would not go.

9:00 AM: Inspector arrives 9:30 AM: Mud man (Donnie Strange, Knox Energy) arrives and begins mixing in 15 sacks of potash, then the first pallet of Florigel-H-Y (saltwater mud). 12:00 PM: Mud man checks mud and it tests 24 seconds/quart, he is trying to get it to 40 seconds/quart. 1:00 PM: Crew is circulating hole, slowly lowering #35 joint into well. Cuttings still are not coming up, mud man continues to mix saltwater mud.

-angular small chunks. The remaining 25% is white to tan, sub-rounded clumps of silty clay with crystals of quartz, calcite and pyrite.

  Mud weight is up to 30-31 seconds/quart and has been the same for the last hour.  The hole seems to be making fluid as judged by the mud-mixing tub gradually filling up.  This joint is drilling  slower than previous ones, and the pipe tight and dragging when pulled up, just like any additional joints we have added  since 11/16/15.  After circulating for 30--.

5:15 PM: Inspector and HWS crew leave location.

0608519947 2015-11-16 Plug / Plug Back No Violations 7:30 AM: Crew arrives, operator and one hand, plus contractor/owner. They get equipment started.

8:30 AM: Inspector arrives, crew begins going back in the hole. First 2 joints go in slowly, but the last of the 3 joints will not go, they lose circulation and pipe is stuck in hole (@ 1036- 11:00 AM: Troy, contractor/owner, goes for parts in WVA so he can go back to a single sand line and they can pull harder.

11:15 AM:  Crew eats lunch while waiting on parts.

11:45 AM: Crew takes off 1031- 3:00 PM: Drill bit is out of the hole and is cleaned. All the cones are free and in good shape, does not appear to be plugged. They are not using a bit sub which would explain why the drill water is flowing back in the tubing at each connection. 3:45 PM: Inspector leaves location, having arranged for another inspector to witness the plugging tomorrow (B. Stolarik, DOGRM inspector for Monroe County) 4:00 PM: Crew leaves for home.

12:45 PM: Power swivel is down but have a replacement part and are back running in 1 hour.

2:45 PM: Back to good, fast drilling, and not getting any cuttings, return water more clear. They are still not in the fire clay.

4:00 PM: Inspector leaves location 4:30 PM: Crew leaves for the day.

0861283129 2015-11-06 Plug / Plug Back No Violations 10 AM: Inspector and 4 man crew from HWS arrive, bringing water truck, dozer and 100 barrel tank on trailers. The portable toilet arrives on trailer with 2 more men.
11 AM:  All equipment is unloaded and we are waiting for OUPS to arrive and mark the phone lines.  Water truck begins filling tanks with brine and fresh water (a total of 4 loads). 

12 PM: OUPS Line locator arrives, marks line, and track-hoe digs a larger hole around the well to prepare for welding. -10 minutes, there is gas pressure on the well. The track-hoe is loaded on trailer, working a total of 4 hours today. 3 PM: The crew and inspector leave location.

HWS will bring the rental mud pump and power swivel out this next week and be ready to begin cleaning out the hole Thursday morning.

9:00 AM: Rig operator and 2 more men arrive after Contractor/owner calls to tell me they were running late. When get on location they find they have to take new hand back to the shop as he has not been drug-tested.

1423155877 2015-02-04 Plug / Plug Back No Violations I met on location with Bill Stolarik, Monroe County Inspector, to inspect this leaking well.

David Reid, from Hannibal Industrial Park, had initially called and complained about a strong gas odor. Stolarik had done the initial inspection,. He found that when they excavated around what appeared to be the source area of the gas odor they uncovered this well. It was equipped with a cast cement plug in the top of the 8 5/8 diameter surface casing with what appeared to be fire clay below the plug. There was also a short joint (~3) of surface casing that was uncovered near the well. Stolarik took pictures showing the well before the excavated area filled with surface water.

The hole around the well was full of water the day of my inspection and no well casing was visible. Gas odor was considerable when standing downwind from the well. Our database records show this was drilled in 1977 to the Berea and plugged in 1994. A 500 fireclay bottom plug was set, then the 4.5 diameter casing was ripped and the well filled to surface with fireclay. The plugging was witnessed by a Division inspector. Pictures taken. SEM is 237.

1446734821 2015-11-02 Orphan No Violations I arrived on location at 8 AM, expecting the crew to be arriving and opening up the well. I found instead that they could not set the rig over the hole because of an office trailer parked too close to the well. While the industrial park workers said it would be moved today, it has not been. Harmon Well Service will continue to move their equipment in and plan to start on the hole on 11/6.
1447776986 2015-11-17 Orphan No Violations On site status check inspection on the orphan well plugging process. Upon my arrival on location at 9:30 am I observed Harmons well service rig over the hole and a currently running 2.875 tubing into the well bore at a depth of 1,115 with a 7.875 drill bit on the tubing with no tubing string float in place. A mud pump and power swivel is on location to assist in cleaning out the well bore. The rig crew worked to condition the fresh water based drilling fluid with gel and lime and the workover pit is 2 metal 1/2 tanks set on the ground surface. The company has the well control surface device connected to the previously set 8.625 surface casing. The company will continue to wash out the old borehole with the 7.875 bit until encountering the top of the previously set 4.5. Off location at 12:30 pm.
1448291096 2015-11-02 Plug / Plug Back No Violations

-hoe and 300 barrel tank are on location. The crew found a temporary office trailer belonging to Stallion Well Services now parked close to the well and it needs to be moved to work on the well safely. They said it would be moved today. The track-hoe has excavated around the well casing and found gas bubbling around the outside of the casing.

7:30 AM: Crew arrives, starts back in hole to tag cement.

10:00 AM: Pump truck arrives, bulk truck at 10:30 AM

12:00 PM: Cementers are done, are washing up equipment and tubing. 12:30 PM: Formation Cementing leaves location. Rig crew begins tearing down the rig. Inspector leaves location.

1449511440 2015-12-07 Plug / Plug Back No Violations My inspection found the crew of Troy Harmon Well Service on site removing the last of the steel half-tanks. The track-hoe was working to dress up the gravelled side roads of the site and the main location. They will have to bring in possibly 3 loads of gravel to restore back to its original condition. They have contoured the surrounding bank to the west to resemble what it looked like prior to the plugging and will scatter grass seed on this vegetative areas to establish growth.
1528794539 2014-12-08 Plug / Plug Back No Violations On site inspection and response to complaint # 4875. On this day I met with property manager David Reed and we discussed the said well leaking gas to surface. Mr. Reed informed me that while excavating the area the odor of gas could be smelled and the site was uncovered and evaluated and the cast plug found still sitting on the 8.625 with gas leaking up to surface. Upon inspection it was observed that the pit around the well was under water and gas could be seen bubbiling up through the water that has acculimated with the recent rainfall. I informed Mr. Reid to leave the well under water to diminish the venting gas. ODNR Deputy Chief Dave Claus and District Supervisor Brent Bear were notified on this day.

1663664857 2016-01-07 Final Restoration No Violations My inspection this day found this location has been restored and all the equipment removed. There is new stone on the drive part of the location and the west half of the location has been contoured to its original grade and planted with grass seed, then mulched. Although the mulch was rather heavy, grass is growing under it, probably due to our warm winter. I am passing the final restoration with this inspection. Pictures will be copied to M/drive.
