0546688339 2013-03-22 Administrative Inspection No Violations I received a call form PP&Gs Pickerington Ohio office asking what they needed to do to comply with the violation issued against them on the said lease, Once again I advised them to register the said wells with the Division and cease the dispersant of production waste into the said waters of the State, I also advised them that they would have to properly dike the production tank and haul the water off from the wells on the lease. I explained to PP&G once again that these are commercially produced wells and not exempt Mississippian wells.
0809049205 2012-12-10 Production Wells No Violations Inspection of the well site found the well still in production and dispersing production waste fluid onto the ground surface with previous violation still pending and prior request being made by the inspector asking that the well be shut in and not produced until the well is registered and capable of being produced without further contamination. A attempt to dike around one of the production tanks was made with only 2 sides of the tank being diked making it an incomplete enclosure incapable of holding the required amount of fluid in the amount of a spill. The said well has not been registered. Inspector will note that I spoke with Jason Ludwig on 12/05/12 at another well location and during our conversation I did make note that the wells were to be registered and provided Him with a copy of the Mississippian exemption laws.
0927961734 2008-12-15 Production Wells No Violations WELL IS SUPPLIED WITH A PUMPJACK AND 100BBL TANK. NO VIOLATIONS NOTED.
0962186483 2012-08-27 Production Wells No Violations Inspection and investigation of 2 wells and the production storage and production fluid transfer site, in which sends oil to 3 tanks located up by Twp 505. GPS coordinates was taken on both wells and sent to Mike Williams in the Division Head Quarters and properly identified. Production fluid waste water was observed flowing from the knock out tank and into the ravine. Samples were taken by Inspectors Bill Stolarik and Andrew Corder and turned into the Cambridge lab. Ohio EPA , Division Supervisor Rocky King and Division Hydrologist Kyle Tharp were notified of action taken and findings. Also on 8-28-2012 notification verbally by telephone was made to PP&Gs main office and corrective action was agreed upon by both producer and myself, Lead inspector Bill Stolarik . Inspector will note that the permit number for the 2nd well on lease is as follows 3411165735 and inspector is unable to enter report into RBDMS database at this time for that said well due to the fact that it is not in the system yet.
1313424594 2013-01-07 Production Wells No Violations Inspector revisited site to document the discharge pollution and contamination from the knock out tank onto the ground surface. A Chloride High Range test was done with a 300-6,000 PPM High Level test strip with the test showing 8.8 units or 6,042 Parts Per Million. The well was pumping upon my arrival and videos taken of the pollution. Contact was once again attempted to be made to the company. Notification was made also to my supervisor Rocky King.
1346189818 2012-08-27 Production Wells Violations Noted Inspection of the well site found 2 producing unregistered wells dispersing production fluids into the said waterways of the State. After further investigation the well was found to belong to PP&G oil. Corrective action needed is as follows. All oil cleaned up from ground surface at the well head, the tank battery & the oil transfer pump site. Erect sufficient containment dike areas around tank batteries, taking into consideration that the 3 tank site is extremely close to Scharr road and needs set back off the public right of way. Legible identify both well heads and production fluid storage areas. Stop dispersant of production fluids into the said waterways of the State. Register the wells with the Division of natural resources and correctly report all production. Permit number for the well nearest to the roadway is 3411165735.
1347451559 2012-09-11 Production Wells No Violations Inspector spoke with pumper and once again asked for company to stop dispersing production waste fluid into the waterways of the state. And begin remedial action on fluid that is spilled and dike tank batteries. We also discussed the wells not being registered.
1351168068 2012-10-25 Production Wells No Violations Inspector was on site this day and the well still pumping dispersing production waste water into the waterways of the State. Previous requests to comply with the initial violation have not been acted upon by the producer. Inspector met with the pumper for PP&G, Carl English, and He stated that the company was preparing to begin work on the said well.
1352820601 2012-08-27 Production Wells No Violations I met with MRI 2 Bill Stolarik to aid and witness the collection of samples. On this day the well was producing and production water was being dispersed into waters of the state. There was also oil field contamination around the well head and below the point of dispersal. Violations were noted and owner was notified via Compliance Notice.

And they are to stop dispersing the waste product onto the ground surface immediately along with properly registering the wells and reporting the production with the division. Mr. Ludwig stated He had verbal conformation with Northwood Energy that they were interested in buying the wells.
1366816237 2013-04-24 Production Wells No Violations Compliance agreement follow up found that the company owning the well PP&G Oil has set a secondary recovery tank and no signs of current pollution and contamination was found as in previous inspections. The company has however not registered the said well under their current bond with the division. Compliance notice issued on 08/28/12 was number 1346189818.
1395016660 2018-05-03 Production Wells No Violations Compliance notice # 1346189818 follow up inspection found the well equipped with 4.5 diameter casing and 2.375 diameter tubing with pumping rods installed into the tubing and the well set up to be produced by the means of a pumping unit. There is a 1 production flow line attached to the tubing well head that leads to a nearby production facility. There is also a 1 flow line attached to the casing wall head that is standing open to the atmosphere near the well head and the production valve is open. On location in close proximity near the well head is a production facility that consists of (2) steel production tanks. There is a earthen dike present and the operator has reported current production. All previous violations are considered resolved on this day.
1490287761 2012-11-19 Administrative Inspection No Violations Inspector received a call from Glenn Ludwig in regards to the current violations on the Jones lease, Mr. Ludwig asked if He was allowed to sell the lease to Northwood energy? Inspector informed Him that Division HQ would have to approve the transaction and that an environmental issues or charges of neglect would remain issued to Him. Discussion in regards to what is the correct permit number of one of the well did take place with the inspector informing Mr. Ludwig that a discreprency between what He considers the well permit number to be and what the division called it could be sorted out, Inspector commented that Mr. Ludwid needed to get His wells registered and that no Mississippian exemption clause made these wells exempt from proper registration and following baseline enviomental standards.

1948877157 2013-01-18 Production Wells No Violations On this day I met with Ken Mettler from the Ohio EPA to review and discuss the ongoing Pollution and contamination onto the ground surface and into the waterways of the State. Mr. Mettler and myself observed a 100 barrel tank with a hose coming from the water separator on top leading to a small earthen pit that has been dug with oil visible in this pit area. We also observed a area affected by this product that leads to a stream with obvious signs of surface contamination in this area. Investigator Mettler understands that the Producer known as PP&G Oil does claim to own the wells and equipment and has not cooperated with the requests of the inspector. Investigator Mettler will assist the Division in resolving this matter.
