The spear will prematurely set off. The rig tried to pull 10up about 2 off bottom. They thought 10casing was on 7collar.

Production has not been submitted for two years. The production that is submitted shows a history of production under the minimum standard. This well is owned by C G O INC. Well shall be plugged or application shall be submitted for temporary inactive status.
0871579890 2012-07-16 Plug / Plug Back No Violations On the day of my inspection inspectors Pat Shreve and Chris Grimm were on location. Harmon well services employees were present. A bailer was run into the hole and they attempted to use the sputter deepen the hole. The eight inch casing stared moving so operations were shut down. We could hear the hole caving in. We will try to run an impression block the next time we meet on location.
0907341875 2012-03-14 Production Wells Violations Noted On the day of my inspection Harmon well services had a rig on location and had been working on the well. They were not working at the time of the inspection. There was a pit on location that did not have a pit liner in it. I tested the water in the pit and it was over 6900 ppm on the chlorides. I called Pat Shreve to aid me in taking samples. Ed Crum (well owner) showed up on location and I spoke with him about the issues on site. GR-Contracting showed up on site with a load of brine water and Pat pulled in at the same time. Pat and I sampled the water in the pit and also took soil samples. I took photos and GPS readings of the samples for documentation purposes.

The results did not provide enough information to determine exactly what was down hole. We will try to spear the loose casing and create a solid working string for further attempts to deepen the hole.
1332784826 2012-03-14 Production Wells No Violations On day of inspection I met with Andrew Corder and helped take samples at site. Well was plugged back in 1949.Well was bought by Ed Crum from Nova Christman.At some time well was opened back up and a newer piece of 7was placed in well. There was no reopen permit issued. Ed Crum will apply for plugging permit. pudding rig will be brought in to drive obstruction down to bottom. Andrew will issue a Compliance Notice to CGO. Several truck loads of brine have been placed into well bore as a drilling medium.
1342101084 2012-07-02 Production Wells No Violations On the day of inspection the unlined pit that had previously held brine water had been filed in. GnR contracting advised that water was disposed of by them in a proper manner and documentation was available upon request. Ed Crum has applied for a plugging permit; the plugging of this well and the pit closure satisfies the prior compliance notice. Compliance notice resolved 07/02/2012
1342744673 2012-07-18 Plug / Plug Back No Violations I received a phone call from Troy Harmon or Harmon well services and he is attempting to locate a spear so that we can try to retrieve more casing from the well.
1342797903 2012-07-16 Drill / Deepen / Reopen No Violations I was on location with ODNR inspector Pat Shreve to witness CGO cable tool rig drilling through an obstruction in the well which is to be plugged. The drillers were at a depth of 116 when they shut down for the day.
1344525026 2012-08-02 Production Wells No Violations On the day of my inspection Harmon well services were using 8 casing with a seringe type cut on the bottom to center the 7 in the hole to all for the 10 to be lowered further. The 8 went around the 7 and reached 83 down hole.

1352141354 2012-11-05 Final Restoration No Violations The said well was plugged to surface on 9-18-2012, As of this day of inspection the casing has not been cut off 30 below grade and a steel plate applied to the top of it identifying the well, all service equipment and excess casing is still on location, it appears that the owner of the equipment has been painting and servicing equipment on site, no final restoration work has yet to be commenced per 1509.072 ORC. Also noted that the landowner has made notice that the operator has commenced building a drill site on the property without a current lease or drilling permit from the Division.
1352219061 2012-11-06 Administrative Inspection No Violations Inspector spoke to well owner ED Crum on this day concerning the completion of the Final Restoration of the well and well site. Mr. Crum stated that He had no intentions of completing the restoration work until next spring. Inspector did request that Mr. Crum immediately commence with restoration work according to ORC 1509.072 and also made Mr. Crum aware that no extensions had been issued on the restoration work. The inspector also spoke with Mr. Crum concerning the work on additional wells in the area that are not registered with the Division in His name.

Upon inspection I found the well site itself does meet 1509:072 but the lease road needs restored. I have spoke with Ed Crum several times on the restoration work and will call Ed again and remind Him to complete the restoration.

1485719742 2012-07-12 Work Over Reconditioning No Violations Stopped by service rig while doing random inspections and found rig on hole with app 40 of 7 surface casing on ground that had been recovered from well bore with 4.5 production casing on location to go back into well bore, rig hand commented that extensive efforts had been made with both a power swivel and the sputter rig to get back down into the original well bore. Inspector will not that the 7 appears to be parted down hole.

The block was ran twice and did not provide clear evidence of what lay down hole. Attempts to spear pipe in hole will begin on 07/19/2012. No violations were noted.

1936031629 2012-09-18 Plug / Plug Back No Violations CGO Rig is plugging well back to surface from 126-0. Ran tubing in to depth of 126 and set down on tools in hole that were lost. Pulled 85of 8.625casing and circulated 146 sacks to surface. No oil or gas was present upon breaking circulation. This plug job was due to lost hole after a clean out could not be performed. Temperature of mix water was 63 Degrees Fahrenheit. The PH of the water was at 7.
1971672800 2012-08-14 Plug / Plug Back No Violations CGO rig crew was running in to check depth with 1.5tubing. They were only able to get to 120down and set on top of tools in hole. I talked to old cable tool companies in Muskingum county for advice on fishing the 7casing out. The rig made a tool that would go over the 7collar and allow it to come out by means of tabs bent in on the 8.625casing. The rig crew tried about three attempts and failed.
