0016483454 2017-10-02 Plug / Plug Back No Violations On this day I accompanied ODNR Inspector Pat Shreve. Please refer to his comments for a detailed report. Harmons Well Service was on location with a service rig and Jet Wireline was on location running a bond log. Formation Cementing had previously pumped 130 sacks of cement to bottom. Cement was tagged at 1,774 feet, with a wellbore total depth of 1,784 feet.

0448998150 2017-10-05 Plug / Plug Back No Violations Follow up inspection to complaint # 4951 and compliance notice # 1595482033 issued on 03/02/2015 with a compliance date of 09/03/2015. On this day I observed that Harmons Well Service had a rig on the hole. We have previously set 2 bottom cement plugs consisting of a total of 230 sacks of cement. The cement top was tagged at 1,080 and the 5.187 casing was ripped at 820 and pulled out of the hole. There is a 8 diameter string showing visible at ground level however during the pulling of the 5.187 a 6 piece of 8 diameter came up with the 5.187 diameter string and no visible 8 was visible below that point indicating the 8 was previously pulled. At that point I directed the service rig to leave 100 of the 5.375 standing in the hole as a permanent marker joint.2 diameter tubing was ran in the hole to a measured depth of 817 and Formation Cementing pumping 10 barrels of fresh water then 10 barrels of Gel spacer followed by a 15.7 barrel absolute volume cement mix consisting of 70 sacks of 15.2 pound per gallon 1.26 yield cement with well bore fluid circulating to surface during the pumping of the gel. The 2 tubing was pulled up to 300 and a 18 barrel absolute volume plug was pumped consisting of 84 sacks of class A cement with 1% gel and 1% calcium additives.
0614475830 2016-07-05 Production Wells No Violations Follow up inspection to complaint # 4951 and compliance notice # 1595482033 issued on 03/02/2015 with a compliance date of 09/03/2015. Upon inspection of the well head it was observed that the well was overgrown with vegetation and the electrical service is disconnected and laying on the ground. The well is not producing commercially or domestically with the flow lines disconnected. I have made contact with the well owner Ed Crum who indicated He would like to plug both wells on the property. Mr. Crum indicated the legal council for the landowner filed an injunction through the court system restricting his access to the property. I discussed with Mr. Crum my concern with the environmental aspect with the wells not being checked regularly and the producers obligation to follow the Ohio Revised Code pertaining to the laws of the State.
1370859299 2017-10-05 Plug / Plug Back No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. Harmons Well Service tagged cement at 1,092 feet and ripped the 5 3/16 inch diameter casing at 826 feet. The 2 inch diameter tubing was ran back in at 817 feet and Formation Cementing pumped 10 barrels of fresh water, then 10 barrels of gel spacer, followed by 70 sacks of 15.2 pounds per gallon, 1.26 yield cement. The 2 inch diameter tubing was then pulled up to 300 feet and pumped 84 sacks of class A cement with 1 percent gel and 1 percent calcium additives.

1459863634 2016-04-05 Production Wells No Violations Compliance Notice # 1595482033 follow up inspection found the well is in disrepair and incapable of production, the electric service is disconnected. The last reported production to the Division was in 2014. In conversation on this day with Troy Harmon who is the field representative for CGO Inc the plugging of the well was discussed. Mr. Harmon indicated the legal council for the landowners had filed an injunction against CGO Inc from entering the property.
1507057245 2017-10-03 Production Wells No Violations On site inspection to witness the well plugging process with the previous 130 sack bottom plug set on 09/29/2017 by inspector Pat Shreve and tagged at 1,774 with a well bore total depth of 1,784. On this day we set a second plug through 2 diameter tubing set at 1,758. The plug had a absolute volume of 22.44 barrels and consisted of 100 sacks of class A cement with 100 pounds of gel and 40 pounds of cotton seed hull additives. Due to the geographical location restrictions the plugging contractor laid a 2 steel line from a second well we are plugging approximately 500 away and the cement contractors set up at the neighboring well and pumped the plug from there. The well bore circulated during displacement and the company pulled the tubing out of the well and will tag the plug the next day. The well also has a compliance notice identified as # 1595482033.
1529973976 2018-06-19 Final Restoration No Violations Status check inspection found the well site restoration meets ORC 1509.072 with vegetation established and erosion control in place. All equipment has been removed. All previous violations were confirmed as closed.
1595482033 2015-03-02 Production Wells Violations Noted Status check inspection as a response to complaint # 4951 filed by Landowner Jane Percel. Upon inspection the following was found at the well head- 5.875 production casing- 2 tubing and pumping rods with the production valves in the open position and the well found not capable of Commercial or Domestic production upon inspection with heavy vegetation present and the electric service disconnected along with no sign of current activity at the well head. At the production fluid storage area 2-100 barrel production tanks were found inside an established dike area. RBDMS records show production last reported in 2013. Photos were taken on this day to note the condition found at the wellhead. Instructions are as follows Plug the said well within 180 days. ODNR Monroe Co Inspector Bill Stolarik 740-507-6592.
