| 1464883807 | 2016-06-02 | Production Wells | No Violations | On the day of my inspection the well was observed to be equipped with 12 conductor and 5.5 production casing with no surface casing or production equipment present. The 5.5 is standing open to the atmosphere. The well is located in the affected area of a proposed shale well pad. On this day I made contact with Lynn Secrest who represents D-D Oil, Lynn met me on site and we reviewed the well and discussed the well ownership. After further discussion it appears as though the well identification possibly may have been misidentified with 3411120702-0000, a nearby well being produced by D-D and not properly registered. Also noted 2 separate producers identified as D-D & Whitacre have reported production for the said well. The shale well operator identified as Gulfport has shown interest in plugging the well and will work with the producers to resolve the matter. |
| 1470768042 | 2016-08-09 | Plug / Plug Back | No Violations | 08/09/2016 A service rig owned by Chuck Dye was brought in and rigged up. JET Wireline was on location and ran a CCL Gamma ray log and marked the TD at 2,083 with the well showing formation constant indicating no shot hole. There is 5.5 casing in the well to 1,769. The rig will run 2.375 to TD and the bottom plug will be set before attempting to recover the 5.5. |
| 1470921472 | 2016-08-11 | Plug / Plug Back | No Violations | 08/11/2016- On site inspection to witness the plugging of the well with cement. A service rig is on location owned by Chuck Dye-Formation Cementing is the cement contractor and Jet wireline service is the wireline contractor. A bottom plug 250 in length was set through the 2.375 tubing from TD @ 2,083 to 1,833 with the Berea formation from 2,083 to 2,076 and the well having no rat hole, the bottom plug was displaced with 7 barrels of fluid total 5 barrels was 6% gel and 2 barrels was fresh water. The well bore was standing open and static prior to setting the 1st plug. The tubing was pulled and the 5.5 perforated to cover the Injun at 1,610 to 1,670 and Keener at 1,546 to 1,580 with 5 shots staggered out in 8 shot at 1,446 and at 1,720. The 2.375 was then ran back in and a 270 plug set from 1,720 up. The 2.375 was then pulled out and the 5.5 cut with a electric wireline cutter at 420; the casing was parted and cemented in place as a permanent marker with no conductor being present in the well. |
| 1476183805 | 2016-10-06 | Final Restoration | No Violations | The well site meets ORC 1509.072. The site also sits off the back corner of the Gulfport Claugus shale pad. |
