0364088987 2015-03-27 Production Wells No Violations Follow up to 1422372024. M&R investments has removed the tank from the contaminated well site and placed it by the road for removal. Some of the contaminated materials and soils appear to have been removed. The company has placed mulch on the site to soak up the remaining contaminated soils. The well has had an identification tag placed on it.
0407464981 2015-07-21 Production Wells No Violations The contamination at the site appears to have increased in volume even though the tank has been removed and the well has been shut in. The area is now 15 x 20 feet and fresh crude is at the surface where there was not any at the last inspection. It also appears a small amount of crude has moved off the site in a 3 foot swath and into the intermittent stream nearby. An inspection of the stream did not reveal any contamination in the stream of along the bank but there was a slight sheen visible on some of the rocks closest to the site. I notified the company as well as the EPA and the DOGRM response supervisor. The company responded with a track hoe and a large ditching bucket that evening. A pit was dug and lined with a 6 mil liner and roughly 15 tons of contaminated soils were uncovered and removed from the site and placed into the pit. Hat bails were placed in the creek to prevent any contamination that may migrate into the drainage during the remediation process from moving off site.

The tank has been removed from the bottom of the lease road, the contamination has been removed from the site and the affected area has been seeded and mulched, tha pit used to store the contamination has been closed. The booms and pads are still in the creek but do not appear to be collecting anymore contamination and are being left in place for now at the request of the Emergency Response Manager.

1367603369 2015-02-05 Production Wells No Violations Follow up to 1422372024. The leaky gate valve has been replaced and the nipple was closed with a swage. The contamination and the leaky tank were still located inside the dike.
1422372024 2015-01-23 Production Wells Violations Noted Asked to check on well status by landowner because RBDMS shows well as plugged and abandoned. Well was found at RBDMS location and a small leak was noted in the gate valve on the 2 line to the tank. The well was equipped with a 100 bbl steel tank to swab into that was set inside an earthen dike. The tank had overflown in the past and spilled into the dike. Well not equipped with an identification tag. The small Columbia gas tag on the well matched with the recorded number in RBDMS. I spoke with Jody Jasper and he verified the well was owned by M&R Investments. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. All leaking equipment shall be repaired or replaced. Any contaminated materials shall be removed and disposed of properly, affected areas shall have topsoil replaced and shall be seeded and mulched in order to encourage vegetative cover.
1816690090 2015-07-22 Production Wells No Violations I was on site from 1:30pm until 3pm today to witness Buckeye Water Service moving a dump truck of sawdust up to the site. A new road had to be cut to manage this. The sawdust was dumped in a pile next to the containment pit and the track hoe began mixing the sawdust in with the contaminated materials, this mixture was to be loaded back into the dump truck and taken to a landfill. The company had placed booms and pads in the drainage before I arrived on site and I verified their integrity before I left.
