0496439988 2012-07-18 Drill / Deepen / Reopen No Violations Ron Durst of Durst Oil & Gas called me on 7/17/12 and notified me the pit had over flown during drilling operations. An area 500 ft by 15 ft going down the hill and 140 ft by 20 ft at the bottom of the hill had been affected. The vegetation had already been removed and the soil had been treated with lime once I was able to inspect the site. Seed and mulch had also been laid down by the company. I spoke with my supervisor about conditions on site and he verified remedial actions that had been taken by the company were sufficient.
1001266631 2012-07-10 Drill / Deepen / Reopen No Violations 730 feet of 24 lb/ft steel casing set in 750 feet of 11 inch borehole with 248 sacks of class A cement. Circulation established and good returns stayed at the surface.
1150695945 2012-07-10 Annular Disposal No Violations Tested Surface casing for annular disposal fresh water test. Was unable to pressure up to the required 366 psi. Multiple attempts were made and all surface fittings were tightened. No leaks were observed during testing at the surface.

Formation breakdown pressure was 2080 psi and the well was treated with 992 barrels of fresh water and sand at an average rate of 14 barrels per minute. The instantaneous shut in pressure was 984 psi and the 5 minute shut in was 884 psi.
1352842583 2012-11-13 Preliminary Restoration No Violations The well site has been graded, the pits have been filled, there is no drilling equipment on site, the bare area has been seeded and mulched.
1629632324 2012-10-11 Drill / Deepen / Reopen No Violations The pits have been closed and filled for the two month restoration requirements. The seed and mulch that was laid down along the area of the pit spill has washed down the hill and grass is only growing in limited areas. I spoke with Ron Durst about having the area reseeded.
