2012-09-17 H2O 0 21891 Gal 5800 Producers Service Corporation Witnessed beginning stages of job including breakdown. Left job to show company production manager violation at another well. then returned to witness end of the treatment on this well. Roughly 3 minutes after the 1 lb sand left the pump truck, about the time it hit bottom, the well screened off. The pressure spiked from 1000psi to 2600 psi within 60 seconds. The trucks were shut down and the treatment was not finished.
2012-09-25 H2O 0 705 Bbl 0 Producers Service Corporation Well was screened off on initial frac job 9/17/12. Sand was flushed from casing and well was re fraced 9/25/12. No acid used on second frac, formation did not need to be broken.

Operation will continue after they reconfigure the returns trough location and line it before fluids return to the reserve pit. Checked out their drilling permit and geolograph in the dog house. Mike Williams and Josh Garland accompanied me on the visit.
