0716183029 2012-05-16 Production Wells Violations Noted An area outside the dike by the drain was found to be void of vegetation. The soil was affected in an area roughly 125 feet long and ranging from 12 to 3 feet in width. Standing water in the area was field tested and found to contain 2434 ppm Chlorides. The affected soils must be removed and disposed of properly. The affected area shall be reseeded, mulched, and steps shall be taken to encourage vegetative cover. The defective drain shall be repaired or replaced so as not to allow the escape of fluids.

The well is in operation and equipped with a pumping unit and a gasoline engine. The pit is still open, drilling equipment is still onsite and reclamation work has not been started yet. Owner/operator shall initiate and complete pit closure as soon as possible.

There was a large bare area by the tank battery as well as around the wellhead. Equipment including frac tanks, tubing, and a bull dozer were on site.

The owner or the owners agent shall grade or terrace and plant, seed, or sod the area disturbed that is not required in production of the well where necessary to bind the soil and prevent substantial erosion and sedimentation. Failure to comply with in the given time period will result in a material and substantial violation. Extension granted to 11/1/12.
