I called the South West Region Supervisor and was told to collect 2 soil samples from the contaminated area and 1 control sample. I also collected a brine sample from the brine pooled on the wellhead. The samples were taken to the ODNR Lab. Wes Harmon and Rick Zickefoose (both from Green Hunter Water) were told on site to remove the contaminated soil. The contaminated soil must be hauled to an approved EPA landfill for disposal.

New Baker Hughes A3 LOK Set Packer with ON /OFF Tool ran with new 2 3/8 4.7 lb tubing. Packer ran to 5532.65 (center of element). Packer set with 5000 lbs compression. Tubing was released after setting the packer and fresh water with inhibitor was pumped through the tubing and circulated to a tank at the surface. Pumped 80.2 barrels fresh water with 10 gallons inhibitor. Sting tubing back onto packer and pressure up the tubing to 2257 psi to open the disk in packer. When disk was removed the tubing pressure went to vacuum. Tested annulus at 1360 psi for 15 minutes with a loss of 60 psi. Started injectivity test at 3:15 pm.

I was notified by Wes Harmon (Green Hunter Water) on 2-26-14 at 2:09 pm that they received the Chiefs Order to inject. Wes wanted me to come to the site on 2-26-14. I told Wes that I was currently in Vinton County and could not come to the well on 2-26-14. I said that I could be there first thing in the morning on 2-27-14. At 3:49 pm. On 2-26-14 I received a voice message from John Jack (Green Hunter Water) That said that they are going to start injection tonight 2-26-14. I responded (as soon as I had a clear cell signal) that The permit conditions require 48 hours notice. I did again offer to be at the well on 2-27-14 in the morning. I received no other messages in regards to the issue until 8:11 am on 2-27-14 when Wes Harmon called and asked if I was coming to the well. I arrived at the surface facility at 9:40 am on 2-27-14. The pumps were off and I was told that they did inject 300 barrels on 2-26-14.

Wes Harmon (Green Hunter Water) told me that they removed approximately 8 tons of soil from the location. The soil was placed in a roll off dumpster and taken to landfill. Well site is graded. No leaks were observed on the injection line, or injection tubing. Injection psi during inspection was 1220 psi. I told Wes Harmon and Roscoe Mills that they need identification at the wellhead and the surface facility. Currently there is identification at the 100 barrel production tank beside the lease road. This tank and equipment are not part of the injection operations.

Average psi of treatment was 1270, with an Instantaneous Shut-in pressure of 1006 psi and a 5 minute shut-in of 943 psi. the pressure on the annulus was measured at 480 psi after the treatment.

A total of 200 gallons used for the job.

The injection operations are shut down until the repairs are made to the vault. Excavation and removal of the damaged vault are on going. All fluids from the vault are being removed by a Roscoe Mills vacuum truck. Inspected the unloading pad, containment dike, tanks, injection pumps, injection pressure and annulus pressure.

Well shall be equipped with functioning fitting and valves in order to monitor for mechanical integrity. Spoke to Wes Harmon over the phone and notified him of the violations. 2137846191 2014-01-21 Surface Facility Construction No Violations Injection line test. Installed 3 Fiber Spar injection line from the pump house (Roscoe Mills # 1 34-105-23619) to the well. One under ground connection was made near the Mills # 1 injection well. The remaining injection line is continuous. The injection line was filled with fresh water and tested to 1380 psi for 15 minutes with no loss in pressure. While monitoring the gauge at the surface facility The master valve at the well was opened and the injection line pressure was observed to drop to zero.
