000PL10670 | Preliminary Restoration | No Violations | Preliminary Restoration done information from Data Point | |
0441859304 | 2017-09-21 | Production Wells | No Violations | This well is a shallow gas well. There is a drip and a small 40 barrel swab tank near the well. This well showed production for last year. |
0460102667 | 2014-04-21 | Production Wells | No Violations | Follow up to 1383321298. The contaminated area has been fertilized, seeded, and mucled. |
0557288833 | 2013-12-10 | Production Wells | No Violations | Follow up to 1383321298. No clean up, valves on tubing closed. Trash can still on site. |
0744200839 | 2013-10-28 | Production Wells | No Violations | Accompanied Inspector Josh Garland on inspection of the well site. Collected a brine sample from an open plastic trash can located at the edge of a scald area at the wellhead. Sample GPS N39.02547 / W082.22949. Sample number JTS060. Sample taken to Jackson office for shipment to ODNR Lab. |
0917013284 | 2014-02-27 | Production Wells | No Violations | Follow up to 1383321298. A poly tank has been plumbed into the drip and placed less than 10 feet from the well. The valves on the well are closed and the well is now equipped with an identification tag. |
0936457183 | 2013-10-24 | Production Wells | Violations Noted | Asked to check on well by Supervisor. No tanks. Well equipped with 4-2 swedge fitting, and head on tubing. A 1/2 steel line runs to a drip next to the well. The drip has a 1/2 nipple out of the bottom with a closed ball valve. Upon inspection it was noticed there was a large scald area roughly 10x30 running directly from the well head into a nearby intermittent stream. Half way to the stream the scald area forks and a second scald, roughly 2x30 also runs into the same stream. The soils were field tested with Quan tab strips 10 feet from the well head and 15 feet from the stream along the second fork of the scald area. Soil samples one and two were found to have 372 ppm and just less than 296ppm Chloride content respectively. The high end Quan tab scale starts at 296 ppm and the contaminated soils in question registered one unit below the starting point. All contaminated soils shall be removed and disposed of properly. Any affected areas shall have soil replaced, shall be seeded and mulched, and steps shall be taken to restore vegetative cover. |
1243782166 | 2013-10-24 | Production Wells | No Violations | Follow up to 1764931367 issued by Steve Ochs. Close violation due to elapsed time. Will reissue new violations. |
1243808277 | 2014-04-01 | Production Wells | No Violations | Follow up to 1383321298. Well has ID, the water goes into plastic tanks on site, the contaminated soils have been removed and new soil has been placed at the site. There is no seed or mulch on the affected area. |
1383321298 | 2013-10-28 | Production Wells | Violations Noted | Well not equipped with legible identification tag. Mechanical separator is less than 5 feet from wellhead. No tanks. Well equipped with 3/4 line that connects to a T fitting. One line runs to separator and one line terminates in a garden hose that was left on site near a plastic garbage can filled with water. Samples were taken of the water in the trash can and sent to the Cambridge lab for testing. Near the well there was a trench that appeared to be caused by the flow of water that was roughly 30x5 feet and 2-3 feet deep. Half way down the trench the path of the water forks and the second fork was roughly 30x2 feet. Both forks appear to run directly into a nearby intermittent stream. High end quan tabs were used to field test the soils and standing water in the trench for Chloride contents at two locations along the scalded area. One location was roughly 10 feet from the well head, and the second was roughly 15 feet from the stream along the second fork of the scalded area. Water from the bucket was also field tested using high end quantabs. All three samples exceeded the parameters of the test strips indicating greater than 6078 ppm Chloride content at each sample location. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Mechanical separators shall be set a minimum of fifty (50) feet from the well. Brine from any well except an exempt Mississippian well shall be disposed of only by injection into an underground formation. Contaminated soils shall be removed and disposed of properly. Any affected areas shall be reseeded and mulched. Steps shall be taken to establish vegetative cover. |
1384957000 | 2013-11-18 | Production Wells | No Violations | Follow up to 1383321298. Met with producer to discuss clean up at well site. |
1386868883 | 2013-12-12 | Production Wells | Violations Noted | Well equipped with 3/4 line that connects to a T fitting. One line runs to separator and one line terminates in a garden hose that was left on site near a plastic garbage can filled with water. Waste water was being collected in garbage cans. Allowing brine to evaporate or overflow constitutes an illegal method of storage and disposal. Illegal brine storage, illegal brine disposal, and any further pollution and contamination caused by operation of the well shall cease immediately. Brine from any well shall be disposed of only by injection into an underground formation. Current brine storage and disposal equipment is not rated for expected pressures. All surface equipment must be pressure rated to withstand operating pressures to which it is subjected. Copies of the laws and rules as well as possible penalties provided. |

2062302812 | 2013-10-25 | Administrative Inspection | No Violations | Well was originally inspected 10/24/13 to closed ID violation issued by Steve Ochs. During inspection I noted evidence of pollution and contamination and evidence that brine water may have been improperly stored and disposed of at this well and 4 other on the lease. I spoke with the owner of the wells on 10/25/13 to verify brine disposal practices of the company and suggest clean up methods at the well site(s). During our conversation the owner verbally verified production water was drawn off the wells on a daily basis, was being stored in plastic garbage cans on site, and being allowed to evaporate into the atmosphere. |
