0335493782 2010-07-26 Production Wells Violations Noted A call was received from Mr. Dennis Deavers with Ohio EPA on 7/25/2010 at 10:28 a.m. Mr. Deavers stated that last night, on 7/24/2010, an oil spill occurred at this location in Meigs County. Approximately 5-10 bbl of oil was released from the bottom of a 100 bbl tank and entered a storm drain. The tank battery contained an eroded earthen dike. As a result, crude oil then flowed along the lease road and onto a county road. The oil would then enter a storm drain, however no impact to Leading Creek. Mr. Deavers states that the spill is under control and Herald Oil & Gas is performing cleanup. A site visit conducted on 7/26/2010 revealed oil spill contained and company representatives at location. The source of the release derives from a broken (rusted) nipple located on bottom of 100 bbl tank. The installed tank was not level and appears slanted forward. A company rep, Mr. Bobby Adams, feels over time this may have placed more pressure on the nipple and with it being corroded led to the release of oil. Mr. Adams had initially responded and placed pressure on fitting with a rag to stop leak. Mr. Adams has since repaired this fitting and tank emptied. The tank will be re-set and the company representatives agreed to address earthen dike. Walked impacted areas beginning at tank battery location (on hill) to end of lease road (approx 300) at CR 21, Hobson Road. At the time of inspection, the lease road had been cleaned up. Absorbent had also been applied to CR 21 (side of road). A boom and orange cones were placed around the storm drain. Walked directly across the road follow storm drain discharge leading into a ravine. The ravine consisted of rocks and crevices. On 7/25/2010, the ravine location was flushed with a water truck to bring up oil below rocks and between crevices per Dennis Deavers and Company Representatives. 1120141225 2010-08-23 Production Wells No Violations Received a call from Sue at American Natural Gas (aka Herald oil) at 10:52 am. Brine tank spill. Arrived on location and talked to Bob Adams. The pump did not shut off and overflowed the tank. This overflow was not contained with in a dike. The brine and oil sludge flowed 200 down a ditch and onto the paved road. Fluids did not reach the storm drain. Bob is excavating the contaminated soil and loading it onto a truck to take it to the landfill. Pictures were taken.
