Located with in a hay field. Well has identification. Well is disconnected from production equipment and production line. A 100 barrel steel tank is on site. A 1/2 mile radius search for water wells revealed: 4 wells: 307804, 275656,265603 and 289145. 307804 is 1245 feet South West of the proposed injection well. 275656 is 1617 feet South West of the proposed injection well. 265603 is 1577 feet North West of the proposed injection well. 289145 is 2873 feet South East of the proposed injection well. A pond is located 1085 feet to the South West of the proposed injection well.

1412619585 2014-10-06 Salt Water Injection Well No Violations Convert to salt water injection well. Well is not connected to injection line or injection facility. Checked the annulus pressure and it was on a slight vacuum then equalized to zero psi. Injection tubing has a master valve that is closed. I contacted Jonathan Reese and told him that the annulus had a slight vacuum and that they need to fill the annulus with fresh water and test the annulus. Jonathan said they will do that and schedule a test.
1413299095 2014-10-14 Salt Water Injection Well No Violations (Inspection on 10-6-14 revealed slight vacuum then zero pressure on the annulus.) Filled annulus to surface with 12 gallons fresh water and attempted to test the annulus, Annulus would not pressure up past 700 psi and would drop off while the test pump was running to 80 psi and stay at 80 psi. The pressure was let off the annulus and an attempt to fill the annulus was attempted. The annulus was full to the surface. Another attempt was made to test the annulus. The second test had the same results. Pressure would not go above 700 and would drop off while the test pump was running. Multiple gauges were used during this process. No leaks were observed on the tubing or wellhead. Tubing pressure was on vacuum during the entire test. The injection tubing is not connected to an injection line. The injection line has not been installed to the well at this time. Jonathan Reese is going to move their service rig back on the well as soon as possible.

1414502646 2014-10-28 Salt Water Injection Well No Violations Service rig pulled 10,000 lbs. over string weight and filled annulus with 20 gallons fresh water. Was only able to pressure up to 520 psi and then the pressure would decline steady. The annulus test was attempted twice and then the decision was made to release the packer and trip the packer and tubing out of the well. The injection tubing is not connected to the new injection line from the Conroy surface facility.
1414599884 2014-10-29 Salt Water Injection Well No Violations Witness rebuilt Halliburton R-4 packer (with 3 shear pins installed) and 2 3/8 tubing being installed in the well. Set packer at 2964. Pulled tubing tension 6,000lbs over string weight. Filled annulus with fresh water and inhibitor. Unable to fill annulus, attempted multiple times to reset packer and pull more tension. None of the efforts resulted in a good pressure test of the annulus. Spence Carpenter decided to release the packer and pull the packer and tubing out of the well.

Filled annulus with 32 barrels fresh water and inhibitor. Was unable to fill annulus, the fluid level dropped at a steady rate. Released packer and pulled up one joint of tubing reset packer and currently filling annulus with fresh water. Test is scheduled for 10-31-14.
1415123650 2014-11-04 Salt Water Injection Well No Violations Company ran a casing scraper inside the 4.5 casing on 11-3-14. Witnessed a rebuilt Parmaco packer (same packer that was installed new in this well on 10-30-14. The rubber element was replaced) and new 2 3/8 R-2, 10 round tubing being ran in the well. Packer set at 2951. MIT scheduled for 11-6-14.
1416240047 2014-11-13 Salt Water Injection Well No Violations Witnessed the Haliburton G-6 packer ran into hole and set at 2954 feet. Corrosion inhibitor and 32 bbl fresh water used to fill annular space. Annular space would not hold fluid, packer unset, two joints pulled and reset. Annulus still unable to hold fluuids. Tubing and packer pulled.
1421770647 2015-01-20 Production Wells No Violations Conversion to Salt water injection well. Appalachian well surveys ran a downhole camera. They were unable to see below the fluid level at 1180 feet. The camera was extracted from the 4.5 casing. The wellhead was shut in and the tubing was collar locked with a valve installed on the top. All valves were closed. Company is evaluating remedial options at this time.
1423165060 2015-02-05 Production Wells No Violations Conversion to salt water injection well. JD Drilling service rig on the well. Gravity fed 35 barrels fresh water down the 4.5 casing to get fluid moving out of the perforations. Well went on vacuum for a brief time then went static. Company currently is running swab tools in 4.5 casing. They are connected from the wellhead to a vacuum truck with hoses. The objective of swabbing is to remove fluid to allow another attempt with a down hole camera and to try and determine where the area is located that is cutting the rubber elements on the packers. Met with Spence Carpenter and Jonathan Reese on site to go over the plans for converting this well.
1423764166 2015-02-12 Salt Water Injection Well No Violations Conversion to salt water injection well. Installed rebuilt Halliburton R-4 packer (3) and 2911 feet of 2 3/8 10 round tubing. Packer set at 2914 with 26,000 lbs. tension. Packer has 3 shear pins installed. Spence Carpenter had a cement bond log for this well. I told Spence to send a copy of the bond log to the UIC section along with his letter requesting a variance in the packer setting depth. Spence explained that the problem they were having with the packers being damaged was discovered at 2928 a collar had a sharp piece of steel that was cutting the rubber elements as they passed below it. They have ran a casing scraper and swab tools down the well multiple times and it seems that this piece of steel has been sent to the bottom of the casing. After setting the packer they filled the annulus with 31.5 barrels fresh water and 1 gallon inhibitor. Tested annulus at 800 psi for 15 minutes with no loss in pressure.

Shot 4 holes in 4.5 casing between 2584 feet and 2586 feet. C&J Energy Services cement pump truck and bulk truck on site to pump the cement squeeze job through perforations. Circulate water to surface prior to pumping gel and cement. Water circulated into a lined pit during the entire cement job. Wire line tagged top of cement wiper plug at 2475 feet.

1446813527 2015-11-05 Production Wells Violations Noted Permit to convert to salt water injection well issued 2-19-2013, Permit Expired 2-19-2015. OAC 1501:9-3-06 (L) operations shall continue with due diligence or the permit shall expire. The last witnessed attempt to set the packer and tubing was on 9-3-2015. The attempt was not successful and the tubing and packer were extracted from the well. JD Drilling has 30days to submit a plugging plan for the Jessie Weber # 1 to the UIC Section at ODNR/DOGRM, 2045 Morse Road, Building F2, Columbus, Ohio 43229-6693.
1447181467 2015-11-10 Production Wells No Violations Convert to salt water injection well. Inspected the rebuilt Haliburton R-4 packer and used 2 3/8 tubing. Ran tubing and packer to 1032(preliminary test of casing while tripping in the well). Set packer with 11,000 lbs. tension. Attempted to fill annulus with fresh water. Annulus would fill up to surface then drop back at a steady rate. Spence Carpenter and Jim Diddle decided to release the packer and let it hang in casing. Job has been rescheduled for 11-13-15.
1447418289 2015-11-12 Production Wells No Violations Convert to salt water injection well. Continued running the used 2 3/8 tubing and packer from 1032 (ran on 11-10-15) to 2550. Set the Halliburton R-4 (rebuilt) packer at 2550 with 20,000 lbs tension. Started filling the annulus with fresh water and inhibitor. Approximately 40 minutes after setting the packer and while filling the annulus, the 2 3/8 tubing jumped up at the surface. The rig weight indicator showed less weight than the entire tubing string. Company pulled 1536 of tubing out. The last joint of tubing had 1/2 of threads left on the joint. The threads broke off in a collar. Ran tools in the 4 1/2 casing and TD top of the parted tubing at 2469. Company is planning on retrieving the tubing and packer next week.

He wants to install the packer and tubing on 2-12-15 and set the packer at 2928. I told Spence that they will need to request a variance to set the packer at 2928.

Packer was set at 2917 with 26,000 lbs. tension. Variance was granted on 3-6-2015. The annulus took more than 31.5 barrels and was not filling up. Spence Carpenter decided to release the packer and pull it up to 2549. Packer was set at 2549 and an attempt was made to fill the annulus. The annulus fluid level was dropping at a steady rate. The well was shut in and an attempt will be made to fill the annulus on 9-4-15.
