1354126946 2012-11-28 Production Wells Violations Noted Witnessing an acid job. Three Barrels of a 28% Iron Acid -20M was mixed in the tubs with 6 Barrels of fresh water for the acid treatment. The diluted acid mixture was allow to sit in the mix tub too long and corroded through the welds on the tubs. The mixture then mixed with the limestone along the lease road creating a caustic foam that impacted the area. Roughly 480 square feet of limestone lined lease road was impacted under the truck and roughly 40 square feet of soil was impacted near the well head. The acid ran along a ditch roughly 45 feet before encountering a culvert. The acid does not appear to have traveled through the culvert. Contaminated soils and gravel shall be removed and disposed of properly. If a significant amount of impacted soil is removed topsoil shall be replaced to restore the surface to its original grade. Impacted are shall be seeded and mulched and steps shall be taken to encourage vegetative cover.
1354287102 2012-11-30 Production Wells No Violations Follow up to inspection 1354126946. The limestone gravel has been replaced. The contaminated soil has been replaced and fresh top soil has been laid down. The area has been seeded and mulched in order to encourage vegetative cover. Landscaping mesh has been laid down over the mulch to ensure the mulch remains on site. The culvert has been cleaned out. The soil in the lease road was field tested for acidity and it registered a pH of 7. Pictures of the reclamation taken. The UIC numbers and log of a Viking Well Services Brine truck on site were also verified.

1355248202 2012-12-11 Salt Water Injection Well No Violations Company attempted an acid treatment on well. Three Barrels of 28% Iron Acid were pumped into tubing but were never displaced into formation. Pressure was raised to 300 psi multiple times but the acid never left the tubing. The tubing will be swabbed clean and acid will be stored in a plastic tank for future use. The annular pressure was monitored during the treatment and there was no sign of communication. During the treatment the annular space had no pressure on it while the injection tubing had 300 psi.

Both containment dikes have 3 inches of water in the deep ends of the dikes. Identification is posted at the well site.

Multiple scald areas were observed during the inspection. 1st. 15 feet West of the injection well 24 feet by 21 feet. 2nd 36 feet south oh the well 45 feet by 18 feet (area with distressed and dead vegetation.) 3rd. 6 feet east of the well 8 feet by 6 feet. There is 10 inches of oil and brine in the down slope section of the containment dike. The containment dike has two 100 barrel steel tanks. The two inch valve on the injection line is open. I called John Kent at 12:25 pm, and left a message with his receptionist about the release and the need to clean up the contamination. ODNR DOGRM duty officer was notified along with the ODNR DOGRM UIC supervisor. All contaminated material must be disposed of at an EPA approved landfill. The containment dike must be kept free of brine and oil.

The landowner Mr. Craft told me that acid was spilled on the location during the well stimulation on 11-15-13. Lime powder was observed in multiple locations. Mr. Craft told me that most of the acid went into the dike and was removed with a vacuum truck. I called John Kent and asked about the release. John said that he did not know about it and would ask the field staff about the details. I requested to know the quantity and type of acid that was released and what was done to remediate the site after the release. Both dikes are free of fluids.
