0375051475 2012-01-24 Production Wells Violations Noted Mike Sherron of Ohio EPA had asked me to revisit the site to check on clean up of oil spill. I saw no evidence of oil or any other contaminants in the creek, but I did ask the pumper to place some booms in the creek since the old ones had come loose. Upon inspection of the water knock-out I noticed the dike was full of contaminated water and had a slight oily sheen on top of it. I noticed the contamination was coming from a loose union on the annular line connected to the water knock-out. I asked the pumper to tighten the union to prevent further leaks and remove the contaminated water from the dike.
0873382239 2011-12-05 Production Wells Violations Noted Dike had filled with oil and allowed 10-15 gallons of crude oil to escape and contaminate the soil and Hysell Run. Oil had flown down hill along access road and across vegetated ground 160 feet to contaminate an intermittent stream which drains into Hysell Run. From that point it followed intermittent stream 100 feet to where it drains into Hysell Run. An area of 5 feet x 30 feet had significant ground contamination between the dike and intermittent stream. A clogged line was determined to be the cause of the leak. Water knock-out on hill had begun leaking brine since time of spill, water was tested and found to have 614+ ppm Chlorides. JD Drilling had sent a crew to empty dike, clean soil, remove oily materials, and relocate dike. Dike shall be emptied of remaining oil and water. All oily materials and soils shall be removed and disposed of properly. Contaminated areas shall be reseeded and mulched to encourage vegetative growth. Broken line shall be repaired or replaced. A dike shall be placed around the water knock-out to prevent further contamination in the future. Tank battery shall be moved, so it is no less than 50 feet from any source of water.
1323793217 2011-12-13 Production Wells No Violations Production lines have been run to tank battery across road. Area of old tank battery has been reclaimed and returned to a good grade, reseeded, and mulched. No recoverable amount of oil remains in creek. Dike has not been placed around water knock-out on top of hill yet. Booms and pads remain in creek. Spoke with operator in regards to changes still to be completed to bring well into compliance.
