0724950690 2012-09-19 Plug / Plug Back No Violations Company intended to set the bottom plug in the Ohio Shale with prepared clay from 3999 to 2342. Tools were run down the hole and the company was only able to reach 2370. I told the company they needed make an attempt to reach TD. The well was shut in for the day while they decide what to do.
0807679643 2012-12-27 Final Restoration No Violations The pits have been filled, the area has been graded off, there is no production equipment on site, there is good vegetative cover, and no problems with erosion. The road is still being used for access to other wells on the lease.
1348015961 2012-09-19 Plug / Plug Back No Violations Arrived with Josh Garland at the well site. JDs service rig and water truck were on location plus a small dozer and a small Ford tractor with a cement mixer hanging on the back attached to its PTO. The mixer will be used to mix the clay, not delivered yet, and water to create a slurry to be emptied into a trough which will empty into the open well bore. The clay will be used to plug the Ohio Shale which is not behind production csg, i.e. open hole for 4000. The bottom of the 7-inch csg covers the Berea Sandstone and formations above to the surface. JD Drilling rigged up a sounding tool with ajar and adapter attached to a cable and lowered to determine how much open hole is available below the bottom of the 7-inch csg. As the tool was being rigged up, the clay arrived in a dump truck, about 11000 lbs (3500lbs equivalent to 35-cubic feet or 25 % more than a yard). JD discovered that there was an obstruction at 2374, only 150 open in the Ohio Shale. JD will bring production tubing from their yard and with a drill bit on the end of the tubing try to drill out the obstruction. Josh and I left to retrieve my truck and travel to an injection well.
1349142837 2012-10-01 Administrative Inspection No Violations Dan Goins and Josh Garland and I met JD Oil and Gas in Jackson office on the plugging of the well. We agreed that the best way to plug the well would be to place a clay plug at bottom of well on top of the cave. Then place bottom of surface plug at 2360-2160. The surface is at 2260.The surface plug will be placed at 200-0.
