0330354203 2016-04-20 Production Wells Violations Noted Inspection of tank battery on this day found a brine scald below the dike on the lease road side. The dike drain was closed but still allowing water to drip out. Below the drain was an area that contained a large scald of about 4 by 10. I used Quantabs in a deer hoof print on the lease road side and found elevated chlorides had a reading of 4.2 or 864 PPM chlorides. I also Quantabed the area by the dike drain and higher chlorides a reading of 7.8 or 4097 PPM chlorides. 1) Identification shall be placed at the well and the tank battery since multiple wells are being produced into the tank battery. 2) The brine scald at the tank shall be remediated. All contaminated soil shall be excavated, removed and disposed of in a proper manner. Topsoil will be brought in to replace contaminated soil and area will be seeded and mulched to establish new vegetation and prevent erosion. 3) Dike shall be kept reasonably free of fluid. 4) Dike drain shall be fixed to prevent unmonitored release of fluid from dike.
0366815920 2013-11-26 Production Wells No Violations Follow up to 1171572384 and associated with complaint #4486. The sides of the dike have been remediated, there is an ID on the tanks, the transfer pump has been replaced, the 100 Bbl tank is not leaking and neither is the separator. There is still contaminated soils inside the dike and the water has an oily sheen. Water samples were taken from the discharge line at the bottom of the dike and found to have less than 296 ppm Chloride content. I spoke with the field manager and he will be removing the soils within two weeks.

1130881218 2016-04-20 Production Wells Violations Noted Last week UIC inspector Jon Scott sent me GPS coordinates of a small oily mess that seemed to have orriginated from a tank battery valve. Jon said that he called Tom Schoonover with J D Drilling and reported the mess. I stopped out today about a week later so I could verify that the mess was taken care of. Nothing had been done to recitfy the situation. 1) The oily mess shall be cleaned up and the source of the oil shall be fixed to prevent future releases.
1171572384 2013-10-02 Production Wells Violations Noted Associated with complaint # 4486. Well equipped with pumping unit and electric motor and was running during the time of inspection. Identification on well was illegible. Tank battery consisted of a 100 Barrel (Bbl) steel tank and a 150 Bbl steel tank. There was roughly 4-6 inches of fluids in in the lower side of the dike. The transfer pump inside the dike was leaking at the time of inspection. The 150 Bbl tank appeared to also be leaking a small amount of fluids from the area around the discharge line. The lower side of the dike was scalded near the dikes discharge line in an area roughly 10 x 15 feet. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. If multiple wells are being produced into the common tank each well-head shall be identified as to owner, permit number where available, well number and lease name. All fluids shall be removed from the dike. All leaking equipment shall be repaired or replaced. Contaminated soils shall be removed and disposed of properly. Any affected areas shall be reseeded and mulched and steps shall be taken to encourage vegetative cover.

1324131618 2014-04-16 Production Wells Violations Noted Associated with complaint # 4662. Pumping unit in operation at time of inspection. Identification on well head was faded and unreadable. Electric lines running to well were strung through trees and less than 10 feet in the air in places. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Electric shall be brought into compliance. Spoke with Tom Schoonover about violations.
1382541031 2013-10-23 Production Wells No Violations Follow up to 1171572384. Most of the water has been removed from the dike and the leaky 150 Barrel (Bbl) tank has been patched, the leaky transfer pump also appears to have no longer be in use. A new pump is in the dike. There is not yet an identification at the well or tank battery. The contaminated soil inside and outside of the dike still remain. The stuffing box is now leaking and a small leak in a 1 line near the separator has developed. I will notify the producer of the two new leaks but will not issue new violations due to the fact the well is still in violation for leaking equipment 9-9-05(A) 8. The compliance date will remain the same 11/4/13.
1385505584 2013-11-26 Production Wells No Violations Accompanied Inspector Josh Garland on inspection. There was contaminated soil inside of the tank battery. Hay and lime had been used to try and absorb oil. There was also fluid in the tank battery due to rainfall, but it had an oily sheen on top of it. The dike drain was open. Water was tested for chlorides. The pumper was notified via phone call.
1387376766 2013-12-17 Production Wells No Violations Follow up to 1171572384. Associated with complaint # 4486. Contamination removed from dike, tanks are no longer leaking, pump has been replaced, area has been seeded and mulched. Small amount of water in dike due to recent rain.
1490662532 2013-11-20 Production Wells No Violations Follow up to 1171572384. Associated with complaint # 4486. The dike has 4 of water on the lower side, the walls of the dike have been seeded and mulched, the leaky transfer pump has been replaced, the leaking tank has been patched, there is no identification at the tank battery, the leaky separator has been repaired, the stuffing box does not appear to be leaking. The water running along side the dike was field tested and found to be fresh.
