1005487630 2012-09-04 Salt Water Injection Well No Violations Arrived on location with Josh Garland and Jake Glascock. JD Drillings Service rig and water truck were on location. They were rigged up waiting on a tool to lift the production tubing packer, R4, to reseat it before the mechanical integrity test was performed. According to Josh they have already tried once on an earlier date to conduct this test and it did not pass. Annular space was already loaded with fresh water on top of the packer before we arrived. Josh put his 1000 psi gauge on the annular space and they used air pressure to raise pressure on the water column to 505 psi to start the 15-minute integrity test. At the end of the test, the well pressure measured 481 psi and met compliance by 1.25 psi ( 505 X .95 = 479.75 psi & 481 - 479.75= 1.25 psi). Josh then measured the injection pressure at 0 psi.

Company pulled up to 19,000 lbs on packer and tested a fourth time, resulting in failure. Company unseated packer and reset at 1628 then tested a fifth time. The company was unable to pass the test.

Retest annulus, test lost more than 5% in 15 minutes. Released packer and pulled up 18 and reset the packer with 21,000 lbs. filled annulus with fresh water and inhibitor. Tested the annulus, test failed. Lost more than 5% in 15 minutes.

Rig pulled more tension on tubing and sheared the pins in the packer. Pulled tubing and packer back out of the well. Company plans to rebuild the packer and also expose the top of the 7 well head to look for any signs of leaks.
1392917453 2014-02-20 Salt Water Injection Well No Violations Follow up to -2075986878. Packer was set on tubing inside 4 1/2 last night and 570 psi was placed in side annulus according to company. This morning I tested the annulus and recorded 575 psi. Today 10 more joints of tubing ran and packer reset at 1212 feet. A MIT was administered with 550 psi on the anulus. The well was not able to hold pressure and the MIT failed.

1398371181 2014-04-24 Plug / Plug Back No Violations Plug Job. JD Drilling service rig and water trucks. Formation Cementing. Ran tubing to 1727. Berea perfs are from 1704 to 1712. Pumped 25 sacks class A cement with 2% gel and 4% Cacl. Mixed in Cotton seed hulls as lost circulation material. Tag top of 1st plug at 1318. Attempted to rip 4.5 casing at 1298 and again at 1264 there was no telegraph at surface (holes were cut in 4.5) Pulled up 100 and attempted to find free point, continued at 100 intervals with no success. Free point was established at 272. Fluid level at approximately 400. Attempted to circulate water around 4.5 casing to surface. Pumped 50 barrels fresh water with no returns to surface (air could be felt flowing around 4.5 while pumping water). Bullheaded a 500 cement plug down 4.5 from 1298 to 798. Displaced 13 barrels inside 4.5. Locked in and waited for cement to set. Ripped 4.5 at 375. Circulate fresh water to surface. Circulated a large amount of sand and small gravel water was a red brown color. Conditioned the well with Super Mud and was able to circulate and free the 4.5 casing. Pump cement plug from 375 to surface (in two stages 1st 200 then 175 to surface). Water circulated and stayed at the surface during the entire process. Cement returns to surface.

I told Spence that He needs to remove the dike and equipment or submit a waiver form to the UIC section.

Packer set at 1629 pulled 7500 lbs over string weight to set packer. Fill annulus with fresh water and inhibitor. Test at 495 psi for 15 minutes. Lost 10 psi. Passed test.

2075986878 2013-07-17 Salt Water Injection Well Violations Noted The injection tubing was on vacuum. Gauges read no pressure on the annular space, but after opening the needle valve it was noticed the annular space was full of pressurized gas, not fluid. Pressure differential exists between injection tubing and annular space, mechanical integrity is still intact. However annular space should be full of fluid. An MIT mini test shall be performed and the well must remain shut in until mechanical integrity is proven.
