0184434388 2014-05-07 Final Restoration No Violations I was accompanied by L.J. Parkin and Josh Garland, inspectors for Meigs County, to investigate an inquiry about a barren area on the hillside on this property. Landowner Kim Bash, 740-590-7355, said she was planning on building a house on this location and wanted to know why the ground was not growing grass and covered with deer footprints. This day, we tried to locate the well that had been plugged on this property in 2003 to determine if the area of concern is related to this well. We hand dug in the affected area to a deprth of about 1 but will need to come back to excavate further with a back-hoe. We also tried to locate the well using a steel probebut was unsuccessful. We will return next week with the original plugging Contractor, Ben Altier, to see if he can remember where the well was located.

0733283946 2014-06-06 Plug / Plug Back No Violations Inspector Josh Garland and I went back to the Bash propertyto attempt to locate the well location. A search of the historic maps on Google Earth found one dating from 1997 which shows the location of the tank battery for this well. The clarity of this map made it difficult to distinguish the well itself so GPS coordinates were taken off of Google Earth for the tank battery and three possible locations of the well. The GPS coordinates for the area that Ms. Bash hand excavated, which was also the area of deer activity, was input on the Google Earth map and was found to be on the northwest edge of the tank battery. This area could have been the location of the brine storage tank and subject to brine spills which would explain the attraction of deer to this specific area. We scanned the potential well areas with our metal locator and found nothing at these GPS coordinates. We also used the metal locator to scan the area where we previously found the 8 casing and collar. We located the other end of this casing which was threaded. This pup joint was approximately 6 long. Our investigation did not find contamination or any gas leaking relating to the well in question. I spoke with the complaintant, Kim Bash, and she is satisifed that there is not a problem with the plugged well leaking. She has sold the property, and informed the new owner what has been done on the property. She is in agreement that the complaint can be closed.
0904476105 2014-05-13 Orphan No Violations Met with Benny Alteir and he was not able to recal the exact location of the well head. I also met with the land owner to have a right of entry signed. There was tanding water at the suspected site of the well and I field tested the water and found it to have 289 and 291 ppm Chlorides content.

1399487193 2014-05-07 Field Inspected, Well Not Found No Violations Inspected site with Idle Orpahn Inspector Cindy VanDyke. There appears to be an area of contaminated soils roughly 5 feet in diamter. A probe was used to attempt to find the surface casing of the well. Nothing was found.
1400185000 2014-05-15 Orphan No Violations Contaminated site inspected using a metal detector, no casing found at contaminated site. Casing was found buried about 100 feet to the west of the contamination, but it appears to just be a piece of scrap metal and not actually connected to the well.
1400602830 2014-05-20 Orphan No Violations Met Orphan Inspector Cindy VanDyke. A grid search was performed across the site in an attempt to locate any further evidence of buried casing. No further casing was found, but a 1 steel line was discovered. The 8 steel casing located last week was excavated by hand further and found to be at least 4 feet long, with evidence from the metal detector at the surface suggesting it to be 8-10 feet long total. Photo graphs taken and GPS information recorded.
1400685707 2014-05-20 Plug / Plug Back No Violations Inspector Josh Garland and I spent the morning trying to locate this plugged well, based on a complaint by the landowner, Kim Bash, of a possible leaking well. We did a grid search of the area and found 2 spots that set off the metal detector. We dug holes in each spot and uncovered in one a 3-4 long piece of 8 5/8 casing (at N39.12811, W -82.01756), angling downward deeper in the ground. The surface casing continued beyond the length of the hole and the metal detector indicated it continued for another couple feet but we felt it needed to be further exposed with the use of a back-hoe. The other hole (N39.12792, W-82.01740) exposed a 4 long piece of 1 steel tubing, likely used as a flow line, with more gravel in this hole. This tubing also continued deeper and the metal detector became ineffective to determine how much longer the piece extended. In neither hole that was dug deeper than 30 did we find the actual wellbore. We also GPSd the location with the coordinates that are found in RBDMS (N39.12754, W-82.01698) and this plotted to a spot in the woods where there had never been a location built. The holes were left open until it is decided if they will be further dug out. I conclusion, we did not find the actual wellbore that was plugged and further digging will need to be done by a back-hoe to go further out and deeper. We concluded that if the well was plugged and identified correctly, we should have located it. We also found no evidence that the well was leaking or had leaked, as was feared by the complainant/landowner.
