0675268765 2016-03-16 Administrative Inspection No Violations I spoke to the landowner and explained everything to him. I explained I left a detailed list down at the site with the emergency response inspector. He thanked me for being helpful and working to keep the oil out of the creek Monday night. I explained that I had the guy helping him out raking and pulling the grass out of the ditch and bagging it then soaking up the free oil with the oil diapers.
0934498712 2016-03-16 Administrative Inspection No Violations Tim called me say that he was going to pay Mike to clean up the mess and that he would probably just sell the well to Mike. He told me that Mike had taken samples to take to landfill.
1027008175 2016-03-29 Production Wells No Violations I witnessed the clean up of this well today. Mike started with the ditch and then moved to the top where the ground was more level. He piled the contaminated soil in the middle of the contaminated area. In his attempts at removing the tank he punctured a hole in the side of the tank with the bucket releasing about 200 barrels of brine from the tank. It was contained in a hole he had dug previously. He then had the other guy with him dip the brine from the hole using 5 gallon buckets and pouring them into the production tank.

1406396343 2016-03-22 Administrative Inspection No Violations I stopped in and checked on the status of the oil in the ditch on the 17, 21, and 22. Oil had gotten farther down the ditch but not passed the final bale of straw in the ditch.
1459421664 2016-03-14 Production Wells Violations Noted Inspection of well found a release at the tk battery. Proper identification was not found at the tank battery. The property owner owns the well and is not registered with the state, well was not properly transferred with the Division of Oil and Gas. There was no dike found at the tank battery. 1) The oil release will be cleaned up. Contaminated soil will be disposed of at an EPA approved landfill. Ground where contaminated soil was removed will be restored and seeded and mulched to establish vegetation and to prevent erosion. 2) Register the transfer of the well with the Division of Oil and Gas and file paperwork for exempt domestic well. 3) Properly ID the tank per State requirements. 4) Build a dike around the tank battery. 5) Fix the leak or leaks at the tank battery. 1773728888 2016-06-07 Production Wells No Violations I stopped by today with the intention of closing out the NOV, however I found a line hooked up to the bottom of the tank with the only purpose being to get rid of brine in the bottom of a tank. I also noted and took pictures of the line going from the separator to the tank leaking with diapers underneath of it.

2079393609 2016-03-15 Production Wells No Violations I was accompanied by 2 members of the emergency response team to this well. The well owners father had traveled from Lima, OH to come and assist in cleaning the oil up. I suggested going to find some oil absorbing pads to soak up the oil on the ground. I found a company that sells them and gave them the address and phone number. So the father and another local gentleman went to buy the products. Upon arriving back I was having them soak up all the oil on top with the pads and then started on the ditch. I was having them remove the contaminated vegetation and place the contaminated grass and straw in plastic bags that I had them double.This was to remove as much free oil and oil soaked vegetation as was possible because the clean up of the site could take several days and could be several days before they could start. Several times Trisha, the senior emergency response member said that it wasnt necessary and to just have them throw down a couple booms. After the third or fourth time she suggested this I said I was out of hours and needed to leave that she could take over.
