0005197949 2009-11-16 Annular Disposal No Violations An inspection of this well was requested by Central Office to verify Annular Disposal (AD) connection. During the inspection, it was noted that AD is not connected. Fresh oil observed inside the earthen dike placed around (2) two 100 bbl tanks. Approx 10 gallons released. Absorbent pads placed at location. Spoke with pumper at location whom, states that spill resulted from bad tank bottom. Instructed to cleanup and call when completed. No impact outside the dike.
0041619143 2010-06-15 Annular Disposal No Violations A follow-up inspection revealed earthen dike has now been constructed around the well for prevention and control of spillage from well. Lime, seed and mulch has been applied to impacted area at well location. Oil remains on surface of water at the tank battery location, but appears to be breaking down. Lime appears to have also been applied to this location. The primary inspector, Jon Scott, will perform follow-up inspectiont to ensure compliance with verbal NOV.

1312263893 2010-05-18 Annular Disposal Violations Noted An inspection of this well was requested by Central Office to verify Annular Disposal (AD) connection. During the inspection, it was noted that the AD remains disconnected. Pollution and contamination was observed at both the tank and well locations. The tank battery consists of a total (2) 100 bbl tanks. However, the second tank remains disconnected due to a previously noted leak. Old oil was observed on the surface of fluid throughout the inside of earthen dike. The overall oil cover was relatively thin with exception of the corners which were approx .25. The inner walls were oil stained and the fluid level was approx 1 from top of walls. High Range Quantabs revealed no chloride. At the well head location, approx 50 x 5 path of oil stained ground (fresh and old), and bare area would lead to a pooled location approx 10 from a creek. This location contained organic material and approx 1 of oil on surface of water. No observed sheen in creek. Rainfall event at time of inspection. Notified Mr. Jonathan Reese with JD Drilling of well site conditions at 12:58 pm. He states that a water truck will be at location within next hour and will cleanup well site. Returned to the site at 2:35 pm and witnessed pumping (one truck load). A second trip will be required. An employee on site was shown the well location. At this time, the source appears to derive from the stuffing box. An employee states that oil, in dike, had been cleaned up last year, but evidently re-surfaced around this past winter. They were unable to cleanup because of freeze. Note: Once the fluid to include oil on surface has been removed, the tank location will be evaluated for soil removal to prevent future occurrence.

1317132065 2011-09-25 Production Wells Violations Noted Response to complaint of blown line, Sunday 9/25/11. Upon inspection dike was noted to be full of water and oil. A two inch plastic line from well to separator had blown out and a large area of soil and brush 25 feet by 8 feet had been contaminated. A small amount of contamination had reached an intermittent stream 20 feet from the blown line. Water and oil shall be removed from dike and disposed of properly. Line to separator shall be repaired or replaced. All oily materials and contaminated soils shall be removed and disposed of properly. Booms and Pads shall be used in intermittent stream to soak up contamination and prevent any contamination downstream. Any area of removed soils shall be reseeded and mulched.
1339112772 2012-01-17 Production Wells No Violations Respond to a complaint lodged by the landowner who stated there was another explosion at the tank battery. Upon inspection I did not notice any signs of a blown line or oil on the ground. A small amount of contamination was noticed at the well head. This complaint is associated with Complaint 3534.
