0209354322 2013-04-03 Production Wells No Violations Follow up to 1362074380. 15 to 20 tons of contaminated soils were removed from the site and the dike. The scalded areas along the dike and road had fresh top soil replaced. The wetland area had 6-8 inches of soil removed and fresh top soil had been replaced. New gravel had been laid down on the road.
0375277458 2013-03-06 Production Wells No Violations The dike had been partially repaired as per my instructions to comply with 1362074380. After a large quantity of brush and trees had been removed from the side of the dike I noticed there had been a cut in the western side of the dike in the past. Down the hill to the western side of the dike there is a small pond roughly 30 x 30 feet . Around the edge of the pond there was a crescent shaped area of scalded soil roughly 20 long and 6 wide at its widest point. Directly above the area of scalded soil there was water leaching through the soil. The soil and water was field tested and found to have 1480 ppm Chlorides and a pH of 6. There was also an area void of vegetation to the east of the dike roughly 25 x 75 in area. This soil was field tested a not found to have any Chloride content.
0389866059 2011-04-15 Production Wells Violations Noted Upon inspection the well had no legible identification. A dike surrounding the well had filled with water and oil. Gas bubbles were seen escaping from the well through the water. The dike also showed evidence of failing to contain spills in the past, a small scalded area (5x5) next to the dike had attracted deer. Bob Adams and BJ Daniel of American Natural Gas were contacted and I requested they clean the contaminated area immediately by removing oily materials and disposing of affected soil properly. Company was given two weeks from date of inspection to make repairs to well and dike and to place ID tag on well. Company was given four weeks from date of inspection to get well into production.
0461502290 2012-09-28 Production Wells No Violations The casing head was still leaking a small amount of gas. The identification tag was in place and the old equipment had been removed from the site. I met the field supervisor of American Natural Gas at the well and we discussed correcting the well head, and putting up a fence around the well and establishing vegetative cover on the effected areas around the well.

Our divisions hydro geologist suggested emptying the pond to remove any possible contaminated waters and then continuing the remediation from that point. I discussed all this with the producer. The company will begin removing the waters tomorrow. The dike has been rebuilt and most of the contaminated soils have been removed from it. Part of the contaminated soils have been removed from the wetland area.

The dike was full of water again and the casing head was leaking gas. the fluids in the dike were tested and found to contain 1493 ppm Chlorides. The well is now in an urban township, so as repairs are made to the well it must be brought into full compliance with the urban rules. The operator was notified on 9/20/12 of the current outstanding violations at the well.

On April 27, 2011 he told me he was sending another clean up crew to go empty dike and clean up remaining oily materials. Due date from April 15, 2011 to have well reworked still stands.

1362074380 2013-02-27 Production Wells Violations Noted The tank farm included two diked areas. The first diked area (A) was 90 x 55 with six 100 barrel (Bbl) steel tanks, a 500 gallon plastic tank, and a separator. The second diked area (B) was 45 x 55 with two 100 Bbl steel tanks, one 30 Bbl steel tank, and a separator. All of the tanks were emptied, disconnected, and marked for removal. Dike A was equipped with a drainage pipe but was 1/2 full of water 6-12 deep and cattails. Dike B did not have fluids inside but was full of trees and shrubs and had trees growing on the dike wall. Dike A had leached fluids from the southern portion of its wall near the drain along and area roughly 30 in length. An area below the wall was scalded roughly 30 x 5. The lease road runs parallel to the long end of the dike along the southern edge of its wall. On the other side of the lease road there was a scalded area corresponding to the leached dike wall 60 x 1-8 in size. At that point water naturally drains south down the hill in the direction of a wetland. Scalded areas were seen along this drain intermittently up to the point where it reaches the wetland 100 away. The drain reaches the wetland and it turns east to follow along the side of the wetland. A scalded area was seen along the side of the wetland 5 x 10. The area beside the dike drain was field tested and found to have 53 ppm Chlorides. There had been high amounts of precipitation around the date the soil and water was field tested. Water shall be removed from dike and dike wall shall be reconstructed or repaired in order to no longer allow fluids to leach out the side.

Fluid had seeped through dike and into land owners yard in two places. The first is 10x10 feet and registered 1.2 on a high range quantab. The second is 25x20 and registered a 2 .0 on a high range quantab. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tanks as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Dike shall be drained of fluid and contaminated soils outside of dike shall be removed, reseeded and mulched. Dike shall be repaired to prevent further leaks. Defective casing shall be repaired or replaced.
