Cement top at 180. Took cement bond log and scanned it and sent out (E-mail) for review.
0309543284 2012-02-17 Final Restoration No Violations This well is to be monitored as part of the Chiefs Order Requirements for Roscoe Mills Salt water injection well 34-105-23619. Well casing was excavated. A 2 inch steel riser has been welded to a steel plate on top of the well casing. Well casing is covered with fresh water from rain event. No leaks were observed in the water surrounding the casing. Pictures were taken.

0463686750 2012-10-26 Drill / Deepen / Reopen No Violations Witnessed 253 of 8 5/8 surface casing (new) being ran into well (11 hole total depth 280). An insert flapper was installed inside casing at 211. One centralizer installed at 126. Nabors Completion and Production Services Co. pumped 16 barrels fresh water gel and lost circulation material (LCM) to surface prior to pumping cement. Displaced casing and landed the wiper plug with fresh water. No cement returns to surface. Unable to grout due to cement bulk truck being empty. Landed plug with 160 psi. Increased pressure to 660 psi and held for 5 minutes to test the casing.
0607175100 2012-10-28 Drill / Deepen / Reopen No Violations Surface casing was ran in hole on 10-27-12 without notification and was not witnessed. Two joints of casing were replaced from the original string of casing that was installed on 10-26-12. Baker Hughes Cement crew were on site at 8:00 am. Due to weather and mud the logistics of getting the cement trucks to the well took until 10:15 am. Circulated 25 barrels gel and lost circulation material prior to pumping Thix-O- Tropic Cement. No cement returns to surface after displacement. Grouted from surface with 3 barrels cement class A. Cement to surface then fell back to 3 feet. Cement bond log scheduled for 10-29-12.
0760045831 2012-10-26 Drill / Deepen / Reopen No Violations Cleaning out the hole, due to cave in. The casing was verified to be new casing from the manufacturer. I was concerned with the pit possibly overflowing due to the forecasted rain.

1351092299 2012-10-24 Drill / Deepen / Reopen No Violations Alpha Hunter has moved onto site today and are preparing to drill out the wall. The original conductor was found 6 feet down and is 13 3/8 in diameter, not 12 as the records indicate. Records indicate the conductor is set at 20 feet. I verified with Dan Goins that it would be acceptable for Alpha Hunter to use the existing conductor as conductor for the new surface casing to be set. The rig will be on air, I verified with the crew the blow line must be 60 from the rig and chained down. I also let them know the casing they run for the new surface casing must be pressure tested.
1351248900 2012-10-26 Drill / Deepen / Reopen No Violations 6 joints of casing on site. Rig not running. I spoke with the company man over the phone and he informed me the rig drilled to 280 and had got stuck. The cement job has been re-scheduled for noon.
1351617279 2012-10-29 Drill / Deepen / Reopen No Violations Witnessed the casing pressure test. 375 pounds of pressure was placed on the 8 5/8 casing for 30 minutes. The ending test pressure was 356 pounds. I also witnessed the bond log being ran on the surface casing. Bond log indicated almost no cement coverage of the backside of the casing. The cement top had fallen back to 180 feet. The company shut down and plans to perform remedial action by this Friday November 2nd, 2013.
1351715161 2012-10-31 Drill / Deepen / Reopen No Violations I stopped in to check on status of pits due to recent rain. The pit was intact and stone was being laid down on the lease road. A tape measure was ran down the backside of the 8 5/8 casing and a bridge was noticed around 10 feet down. A large pile of topsoil had been pushed into the creek by the dozer operator. No sheen of petroleum products was noticed in the water. The EPA was notified of the topsoil in the creek.
1352122336 2012-11-05 Drill / Deepen / Reopen No Violations One inch tubing was ran down between the conductor and the surface casing to try and attempt to tag the cement top and remediate the surface casing job by grouting cement back to surface. Less than one joint was able to be ran into the hole, a bridge was encountered somewhere between 10 and 12 feet.
1352164611 2012-11-05 Drill / Deepen / Reopen No Violations The 8 5/8 casing was shot roughly 10 feet above the cement top at 150 feet. 155 sacks of class A cement were squezed throught the shots and returns were seen at the surface and stayed up. The company will wait 12-15 hours before they go back in to drill out to TD.
1352316234 2012-11-07 Drill / Deepen / Reopen No Violations The rig is currently on air drilling at 634 feet. The hole is making a lot of water and the pits are being drained with vacuum trucks. The company plans to begin drilling on fluid once they hit the Injun Sand around 1330 feet. The cement top between the 8 5/8 and the conductor had dropped 3 feet below the surface. Gravel and redi-mix were used to top off the hole. A BOP is being run on the casing. I spoke with the company representative about maintaining the pit levels and cleaning up around the site to keep the soil from impacting the stream flow.
1352408673 2012-11-08 Drill / Deepen / Reopen No Violations I was asked to check the salt content of the pit water by the UIC section in Columbus. A high range Quan-tab was used and the water was found to have 6508+ ppm Chlorides. The drilling company has not moved over to fluid yet, they were still drilling on air. The rig was shut down for the evening.
1352814122 2012-11-11 Administrative Inspection No Violations I received a call from the company representative letting me know they hit an obstruction at 1490. After reviewing the records I determined it should be the joint (s) of 7 casing that was left on the 7 seat when the well was plugged. The company was drilling with a 7 7/8 bit, I asked them to move to a smaller bit that would fit inside the casing.
1352822438 2012-11-13 Drill / Deepen / Reopen No Violations The company was able to get the 6 1/4 bit inside the 7 casing that was left in the hole and is currently drilling at 2600 feet. A show of oil and gas was seen through the Berea Sandstone at 2070 feet. The brush and stone plug was never encountered.
1353008986 2012-11-15 Drill / Deepen / Reopen No Violations The drilling company was able to get down to 4084 feet with the 4 1/2 bit. After multiple attempts they were unsuccessful at getting the 4 1/2 bit inside the 5 1/2 casing. Upon inspection the bit was missing 4 or 5 buttons and had fresh damage to the body of the bit. The driller is currently running drill stem back into the hole to TD in order to set the bottom plug sometime tomorrow.

Circulation established before plug was pumped. 41 sacks of Class A cement with 2% gel and 3% Calcium Chloride set through drill pipe from 4074 to 3775 and 11.5 Barrels were used to displace the cement plug. A 6% gel spacer was ran.
1353166542 2012-11-17 Plug / Plug Back No Violations Witnessed the bottom hole plug being tagged at 3820. 58 joints of drill pipe were pulled from the hole in order to set the Berea plug.
1353326575 2012-11-17 Plug / Plug Back No Violations Witnessed the Bottom of Surface/Surface plug being set. Good circulation was achieved. After plug was pumped, the company pulled the pipe out of the hole and the cement fell. Cement was pumped and poured from the top.
1353356809 2012-11-17 Plug / Plug Back No Violations I arrived on location at 10:00 am. Josh Garland MRI 2 updated me on the status of the plug job. They tagged the top of cement on bottom plug at 3820. At 10:45 they were at 2040 and attempted to circulate. They were unable to clear the drill stem. Pumped over 2000 psi. At 11:00 the Drill Rig had to trip drill stem out of hole (68 joints). 4 joints of drill stem were blocked with a sand gel / mud mixture. It was not cement. Replaced the 4 joints and ran drill stem back to 2040. Achieved good circulation and pumped the Berea plug from 2040 to 1890.
1395901455 2012-11-05 Drill / Deepen / Reopen No Violations Alpha Hunter is drilling out and plugging well. After two attempts for cementing surface casing of 8.625in place there was the top of cement at 170. There was a basket ran at 40 down. Horizontal wire line brought a casing gun in and shot 6 shots on a 6 casing gun at 150as the top Perforations. Then the wireline truck was moved out and BJ Services were moved in to try and break circulation. Circulation was established with 200 psi being the top pressure. A valve with a 1orfice was installed to create back pressure and avoid over pressurizing the formation. Upon circulation cement was pumped. 35 barrel of class A cement was ran and ample amount of returns. Job was successful.
1406197895 2012-10-27 Administrative Inspection No Violations Talked to Wes Harmon, John Jack (Green Hunter), Bob Worstall Deputy Chief Division of Oil and Gas, Pat Shreve MRI 3, Andy Thomas MRI 2, and Bob Adams. (phone calls were made and received during a 3 hour time span from 11:26am to 3:28 pm.) Received notification that the casing was free and not cemented. Calls were placed and received regarding the pulling of the casing and remedial procedures to cement the casing along with the requirement for a bond log to be ran on the surface casing. Wes Harmon called me at 6:49pm and told me that the cement job is scheduled for 10-28-12 at 8:00am.
1474161228 2012-11-09 Drill / Deepen / Reopen No Violations Drill out / replug status check. 11-7-12 total depth drilled out was 814. Tripped out of well on 11-7-12. Rigged up for fluid drilling on 11-8-12. Tripped back in well and hit obstruction at 574. Drilled through cave in material to 964 at 12:00 pm. Rig crew plans to drill all weekend on day shift.

2065395340 2012-12-27 Plug / Plug Back No Violations Checking on the pit closure for the Final Restoration requirements. The pit has been closed and the creek around the site has been cleared. The landowner plans to sign the waiver to allow the road to be left as access to the site, to not require the company to establish vegetative cover at the site, and to not have erosion control practices in place.
