Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0195473862 2016-07-13 Orphan No Violations Upon inspection, it was observed that this well is idle and likely orphan. The casing consists of 12 surface casing, 5.5 casing, and 3.5 production casing which may be set on a packer. There is fluid at the top of the 3.5-5.5 annulus. No oil, gas, or brine is leaking from the well at any point. The 3.5 casing has a tee connected to it with another tee to a 3.5-2 swedge and cap on one fitting and a 2 nipple, 2 elbow, 2 gate valve, and 2 steel flow line which heads underground 10 from the well. The other tee has a 2 elbow leading to a 2 gate valve and bull plug. All valves are in the closed position. No tank battery was located for the well. There are plastic gas lines in a nearby pond dam spillway which may or may not be associated with the well. The well is located at the toe of a pond dam and is 25 from the waters edge. Several mature trees and heavy underbrush would have to be cleared in order to move a rig over the well. Access to the well is via DT Athas lease road off of State Route 681 then to the left down the landowners pond access road. There is a bridge crossing over West Branch Shade River which would need to be improved or rebuilt in order to move plugging equipment in. This well will be a low priority to plug as it is not leaking and would need significant site preparation in order to plug.
1343417976 2012-07-27 Production Wells No Violations Possible idle/orphan candidate. Pictures and GPS taken. A private bridge must be crossed to reach the well. The structural integrity may be an issue for moving equipment across it.
1625884776 2012-04-23 Production Wells No Violations Well has no identification tag. No production has ever been reported to the state. Records show no current owner.
1735321017 2013-07-10 Production Wells No Violations Review of site and attempted identification of landowner. Pictures taken.
195473862 2016-7-13 0:00: OR No Violations 2.50
